I have been on fluoxetine for 3 weeks and a half now and I don't feel any difference My other medicine made my anxeity and my panic attacks disappear but the thoughts are the same what do you suggest I do ? I am having 40 mg now and I try my best not to ruminate throughout the day but no difference

Hi - i'm sorry to hear that you've been struggling, and I hope you find the medication you are now on goes some way to helping you. Whilst I hope the video helped your understanding and proved informative for you, I need to say that I'm not a medical professional and as such can't offer you direct advice about what you should do next. Always best to see your Doctor/Psychologist. However, if possible, exploring other factors beyond biological factors might be a good idea - exploring with a therapist where your thoughts originated from, what patterns of thinking you've developed and learned over time, and how they might be able to help you not only develop alternative, healthier patterns of thinking but hopefully get to the root cause of it. Exposure therapy to help deal with the OCD/anxiety can take a long time and does not offer a quick fix.Sorry I can't be of more help.

Thanks for your videos- they are super helpful! Quick question for you- in my textbook, it talks about SSRIs, but also tricyclics, anti-anxiety drugs and 'other drugs'- is it just SSRIs that I need to know about in detail?

Glad you're finding the videos so helpful 😀 In relation to your question it does not name on the exam specification the exact drugs you need to know, so you are fine to talk about any of the drugs you mention. What I find is that there is SO much content to learn at A-level that I try to teach students enough content for them to be able to write a 16 mark essay on it, but avoid going beyond that to prevent it becoming overwhelming. So, if you just knew SSRIs really well and could evaluate them you'd be OK - but if you already know a couple more it can't hurt. Hope that helps.

hey sir at 4:25 you say serotonin "passes on a signal". I thought serotonin was an inhibitory neuro transmitter and hence has a negative summation effect and so doesn't pass on a signal?

Consider this: www.nhs.uk/mental-health/talking-therapies-medicine-treatments/medicines-and-psychiatry/ssri-antidepressants/overview/ And this (which is a VERY technical read but see the part of quoted below: www.ncbi.nlm.nih.gov/pmc/articles/PMC9627707/ "By the 1970s, serotonin was known to have both excitatory and inhibitory effects on post-synaptic neurons [66,67]. Generally, researchers thought that the excitatory and inhibitory effects required the existence of multiple serotonin receptor subtypes expressed on post-synaptic neuron membranes [66]."
