So true. I still don't get it why you not trying harder to Change your shift system. I did 12h Shirts in the nicu and drove 70km home. Wasn't the best idea. Now i Just Work 12h at the weekend..

Amen!! This gruesome shifts should be stopped... The romantization of no-sleep, long shifts should be eradicated!! It is a potential harm to both doctors and patients

I have been saying this ever since I was a medical student and even Program Directors would say stupid things such as "you're here to work" "you're in training" and so on. Hopefully this will change soon.

They build a stamina and tolerance to lack of sleep, that’s one of the whole reasons for med school and residency- to condition them to function on little sleep.

I couldn't agree more. It is a danger for patients. I wholeheartedly believe it is the wrong way to train doctors and to care for patients.

​@@AstroZombie1 just like alcoholics build a stamina and tolerance to alcohol? Still doesn't matter when they get behind the wheel, a bac means they are impaired. I don't want a sleep deprived doctor making decisions about my care just like I wouldn't want a drunk doctor making decisions about my care.

@@shaymininfernape7088 it has less to do with the amount of their responsibilities, it's more about the culture and traditions. Instead of working 24 on/24 off, etc, just work 12 on and 12 off like normal professions. Doctors do work more hours per week (and should be reduced to prevent burnout), but that's less of a problem than the extended shifts themselves.

I am a first year medical student. In the first week of university a female professor in surgery told us that 24h shifts are not the reason for mistakes when you are treating patients. She thinks the real reason is disinterest and ignorance. I was surprised how a professor can have this opinion. The healthcare-system is sick.😶

I see her point. If you think you know everything and believe you are infallible in your judgement, then you're likely to make a bad decision. I've seen this personally. I now know to walk away from such doctors. Even if you've worked 24 hours, if you are aware of your weaknesses and vulnerabilities, you have to slow down and think about what you're doing. Under such circumstances, the tired doctor may actually make a better decision than an arrogant, fully rested one. Medicine is about thinking about problems, not just operating and resuscitation.

Yeah this happened to my mom when she was a resident and pregnant with me. It was after a 24 hour shift in Chicago. Residents need breaks! She also only had one week of maternity leave during which she had board exams.

What?! In Germany when you are pregnant you even aren’t allowed patient contact… because of the risk for the mother… when there is not a safe job (for example office) or other ways, the mother to be is staying home fully paid. The maternity leave(Maternity Protection) starts in Germany 6 weeks before the birth date and last 8 weeks after the date of birth … and is Mandatory and fully paid because it is for the safety of mother and child… after the maternity leave the parental leave starts which are 14 months of 70% paid, which can be split up between the parents. And after that you can have up to 20 months unpaid leave… your job will always waiting for you to return…. So it is illegal to fire you and your job have to be there when you are coming back…

​@@Raioh. alot of people hate working 8 hours at their job already just because something is tolerable or okay for you doesnt mean it doesnt feel like lowkey torture to other people. Think what people of the future will think about us when they prob work 40hour MAX a week and not because they need to but because they want to.

@@Raioh. well, I’m not sure if it is or not but when my daughter was little, we spent a lot of time in the hospital because she needed to have some procedures done and I remember a doctor doing his rounds and telling me that if I had a question or needed anything, to tell te nurse and hed come, no matter what time it was and I was like “won’t you be home by then?” And he replied with “no, I’ll be here all night, I’m working a 24 hour shift” and I remember that because I didn’t know doctors did that and I thought it was ridiculous to make them do that, specially dealing with people’s well-being.

I have ALWAYS thought this. As a patient, I'd ideally like a doctor who has got a freaking good night's sleep and is on top of his game. 24 hour shifts are inhumane.

My daughter is a first year medical student and she’s already going days without sleep trying to keep up with all of the material and tests that are thrown at them. It’s unbelievable. It’s been her life long dream. I pray that she gets through this ok.

I was long unable to sleep .It began after my Dad commited suicide. Yet when I got the courage to bring it up with a doctor, he laughed at me, saying i wouldn't cut it as a doctor. I have always thought expecting them to do that was goofy... i know they are expected to toughen up, yet it flies in the face of what we know about how much a lack of sleep actually affects everyone. After all, we were not together in Alaska trying to survive after a plane crash. By the way, that doctor did not take me seriously. Nor did the next one. I began to realize I was mouth breathing and snoring. The second and third and fourth doctors told me that young women don't snore... maybe they all needed more quality sleep ?.

My son is military and often had to work 24 hour shifts. But when he did he always had a partner who worked with him and they would trade off doing guard duty allowing each other to take a break and sleep.

I don’t know if any nurse or law enforcement person that works this many hrs straight I worked in healthcare for over 30 years I’ve never know a nurse to work more than 16 hrs I’ve worked a double and it’s sucks

@@judyperri9496 well here I am raising my hand... hospice nurse. And my husband is federal leo- many 16 hr shifts then 4 hrs off to eat, shave and shower and on another 16.

I almost hit someone right in front of me after a 36 hr shift in residency. I literally did not see him. Fortunately I was going very slow in a parking lot and he banged his hands in the hood of my car and I stopped. I was looking straight ahead and didn’t see him because my brain wasn’t working right. It’s dangerous.

Respectfully I wouldn’t want anyone operating on me or assisting me in any fashion during a 36 hour shift. Someone could easily end up dead just because the doctor is sleep deprived. No one is firing on all cylinders after 36 straight hours. 24 is outrageous. 36 is mind blowing at least to me. I want my doctors rested, coherent, and able to do their jobs because they have all the tools to do so, especially including sleep

Back in University, I had to study for 3 final exams, but since I always hated to prepare myself for studying weeks before, I just studied 35-36h before the exams, no caffeine or drugs. When I finished them all and went back home, I just buried in my bed like a dead corpse, and did not wake up for 14 hours. I passed 2 out of the 3 exams. Not a thing that I'm completely pride of, because I promised to myself that I never going to do that again, for my health. I do not recommend it for anyone either. also, I remember that even though the exams were on summer, I felt so much cold the last hours, that my body was shivering uncontrollably, firstly for the cold of my body that was unable to produce more energy (even though I ate well), and secondly for feeling nervous of reproving all the exams and get kicked out from university because it was my last chance.

​@@sargek7When I was 28 I went back to university. I DID stay up to STUDY, CRAM, MEMORIZE. When final exams were over, I had lunch then crashed. Made all As BUT that was very, VERY difficult and I determined to NEVER do it again. SO it's possible, but NOT a good idea FOR ANYONE.

I was a passenger on a motorcycle one morning when a car swerved into us and good thing I was wearing a helmet because my head hit right onto the curb. The man who got out of the car was so distraught, and it turned out that he was a physician, who had spent all night at the hospital, trying to save the life of a baby to no avail. We were OK but it could’ve ended differently and I felt really bad for that doctor and I always have because I know that it really was terribly upsetting for him.

He no doubt had to pile that traumatic experience on top of all his sleep deprivation and the stress from his job. It's a very efficient way to ensure medical professionals get burnt the fuck out in record time.

I heard some residents will even put their ponytails in the door of their car so they can be jerked awake if they start nodding off. no one should have to do that to make it home safe

Residency is torture! And if you’re seeing patients in an ER setting - same difference! Like working with a buzz. I am a retired medical professional that is now in school to get my masters in psychology (I may continue to obtain my PhD) I stay up way to late doing my schoolwork and I find if I don’t rest and get away from what I’m studying my brain turns to mush! What is crazy is when this continues as the norm. I have worked with several surgeons who work 24-40 hours straight! I have brought it up to my department how this isn’t safe for our patients (I am an OR nurse) and you know what they do? Nothing…..who would want their family member operated on by someone who has been up that long? Mistakes are bound to happen. We have been overworked, too, which isn’t good, but the surgeon? Not ok. Hopefully change will occur sometime. I will keep fighting and advocating for my patients and the staff!

We work 12 hour shifts. We end up staying late to chart what we couldn’t chart on our shift. When I drove home, many times I didn’t remember passing certain landmarks on the way home. Very scary!!

That’s absolutely awful, the people who are of the utmost value, are mistreated so badly, no wonder the doctors aren’t as caring now as they used to be (i speak from 40 years of regular medical treatment), someone should take action to put stop to that, I will happily support the petition.

PREACH. Whenever I’m feeling burnt out as an educator and taken advantage of, you’re one of the professions I think “well at least I’m not them”. Why we make the very people that save our lives so tired that they cannot make rational decisions is beyond me.

My son worked 10 hour shifts with only 6 hours off in between during COVID to keep his hospital open. His wife did the same at her hospital. These two loved their careers and I never thought I would see the day but his wife left a month ago and he is staying until his two finish high school. This is what COVID did to hospital staff and it is sad. They have both had time in hospital to help control blood pressure and heart rate. He has had two surgeries to stop his heat and put it back in rhythm. All caused by stress and exhaustion. They are mine and I support their decisions to be healthy again.

I mean, I guess ppl don't realize how more common it is ppl r putting in more hrs. It's not always all the "honorable" ppl... I knew some classmates working in facilities that got plenty of hrs.. & here, im having to ask for more. Not really complaining tho. We work in different places & getting able to move around from client to client feels a little more like freedom to me. Even if I go to a facility, it'll be short hrs. Anywho, I remember warehouse days. And on holidays. Somexs I forget those hrs but then I remember the schedule & once it was 9am -12am. They try to call break on 12 and we already knew. We were like k no thx. Cyah. .. In the morning 😒. But we ain't doing this. My hubby (now) swears all our higher ups were on some ish. 🤔🤷🏻‍♀️ would make sense. They were pretty lenient... I think 10 hr days might be a little closer to average by now? Idk. But I know those ppl gotta be wearing their bodies out. We weren't even allowed to sit down. & the physical enviornment in it conditions sucked. I couldn't even walk out sometimes, it was a shooting pain.

I have never understood why medical professionals are expected to work this way. I certainly wouldn’t want my doctor or nurse to be in hour 22 of their shift and making life-changing medical decisions about my care 🤷🏻‍♀️

I had a cardiology doc on shift napping in the on call room. He was there for 3 days with almost no naps or food. He came over for a new admit and was dizzy and just didn't look good. We sat him down, got him some food (prior to COVID us nurses have potlucks almost nightly). He still looked like crap. He let us check his blood pressure.... 220s/110s, HR like 150. Stress and lack of sleep was killing him. We sent him down to the ER.

I often wonder when society will start caring enough to do something about these kinds of things. And these days, many jobs in general are quite demanding of their workers; where one employee has to cover 2 job positions at the same time, until they eventually experience chronic burnout. I couldn't count how many times I have walked into a convenience store and seen only one person working in the entire store (running back and forth from the cash register, back to the other end to try and finish filling the shelves, or trying to clean).

I’m a nurse who works nights and I actually do feel high sometimes after a night shift while I’m driving home. I work 12 hours so I stay up the whole night so I guess I have the equivalent of .05 blood alcohol and I can’t imagine working for longer. What the have docs do is crazy!

I got so ill working nights and then 12hr days, ICU. So not healthy. Mental & physical issues, etc. If I was in admin, I'd make certain my staff had clean/private rest areas and could use as needed. Lots of on-call RN'S & CST's, sterile processing for OR and those pt are coming from ER. Many go to ICU post-op. Our schedules are crazy.

I occasionally work 19-20 hour days, which surprisingly aren't that bad when the rest of the days of the week are under 14 hours, but when you get up to that 16 hours several days in a row...I have to slow down A LOT to make sure I don't make mistakes that could hurt someone. But this only extends my day further (nuclear pharmacist here)

I am a nursing student, and I pull all nighters studying for exams just to then go into class the next day with no sleep. Sucka, but we gotta pass lol. Can't wait to at least get paid to pull all nighters...

Year 2002, In NY, as a resident we were told that new law limits us from going beyond 100 hours per week. We easily went above it..... our shifts were 36 hrs .

In Mexico, residents have to work 36 hours in a row every 2 or 3 days. And if you are grounded, you can’t leave the hospital. I know what you talk about. You are threat to yourself and to others. Patients included.

Sameee in india and some of the hospitals in india don't even pay a penny in residency. We still have to ask out parents money to buy pen, pencil. Although we are around 26 year old. It's horrible.

As a intern doctor I have worked 36 hrs straight. After 18 - 20 hrs you just slowly start to drift away from the real world. After 24hrs you can be 100% lucid only for few minutes before falling back into a hazy world. And they expect us to make life saving decisions alone in emergency situations, while we barely can figure out whether it is day or night

@@Ink_25 No mandatory rest period. Your are oncall 24/7, but can get 12hr rest EOD. Even during that period, if there is an emergency you are supposed to be there. You can get few hrs sleep here and there during the working 36hrs but when you are already exhausted working 12-14hr straight, that kind of sleep is not enough. Best part is, during first 2 months of internship, we got only 7hrs to rest EOD instead of that mentioned 12hrs 😂

I imagine that this kind of thing will never stop because the people who came before won't allow it. "If I had to do it, so do you!" It's a powerful motivator. People who suffer want others to suffer.

Let's assume you cut the duration in half, you double your shift changes (errors related to shift changes are the most dangerous time for patients). You also half your volume of patients. Over the course of training, let's say you lose 20% of your training opportunities. Are you willing to extend the training duration by 20%? There are numerous factors as to why shifts now are shorter than they were in the past. I'm not saying a lower number isn't optimal. What i am saying is the situation is complex/ more nuanced than you and many others seem to be articulating.

Can 100 % relate. Had a 24-hr Call as a Resident. Entirely devoid of sleep. Dozed off at the wheel. Was told I was swerving in-and-out of lanes. Wound up in a ditch. Car was towed. God kept me. I'm a Consultant.

I'm in community college and will be graduating soon, looking for a 4 year school for my Bachelor's degree. I'm mostly looking into Physician Assistant school but a piece of my brain keeps telling me to try for med school. It's terrifying enough thinking about making an honest mistake, but even more terrifying to think that the sleep deprivation expected of me will be the reason I kill someone with poor decision making. Med students, just like everyone else, need to fucking sleep. Probably more than everyone else. I bet you see mortality rates bottom out if you reorganize healthcare programs in a way that allows staff to be well rested before each shift. Pilots, ATC, and truck drivers (as well as a ton of other jobs) have minimum rest requirements so their judgement isn't impaired to the point of them killing people. Why aren't doctors given that same courtesy?

@@LizzW90 honestly, I wouldn’t do it. PA is a great path in comparison, and medicine (being a doctor, I mean) is such a long commitment, it’s best to refrain if you have any sort of doubts - as a surgeon I shadowed said, “you need to want it so bad it hurts or else it’ll be more miserable than pleasurable.” Something I learned is that just because you CAN do something doesn’t mean you SHOULD. You may be smart enough to get into med school and go down that path, but that in and of itself shouldn’t justify it.

I suffer from severe insomnia and there were days where I would not sleep for a day or two. I learned very quickly not to drive during those times. it scares me to think doctors who are trying to save people's lives are that sleep deprived

As a SICU nurse I have caught many residents in medical situations that are wrong. I’ve had to call surgeons to get permission from them to follow different orders than what residents want. Residents wanting to push bicarbonate on respiratory acidosis, when all I want to do is change vent settings. I refused to push bicarb. I had to get the surgeon to tell the resident to listen to the icu nurse because she knows what she’s doing. And ortho resident on fellowship and a 4 week icu rotation doesn’t understand how much we know. But also as a nurse I had to worked 20 hour shifts which was the hospital limit for nurses. My productivity really decreased.

Driving tired is just as dangerous as driving drunk. My DJ friend was coming back from a fourth of a July weekend and drifted off briefly, bumped the car in front sending a father and son spinning. Everyone is okay, but he always made sure to have a driver or just sleep. And no, he wasn’t under the influence he is a sober king haha

EXACTLY! I’m so concerned - that’s such a hard thing to go though, but also such a hard thing to have to act with when it comes to treating patients. So much anxiety young doctors must have when they know they’re at their limits and they have to go save lives

Last year a resident dr morbidly died bc she fell asleep on the wheel after 24+8 hours shift in Turkey. Hospitals got better regulations afterwards. Still a big problem tho

Totally agree. As an OR nurse and when on call all night and I work the next day, I FINALLY got smart and rented a hotel room by the hospital. I sleep crappy when I'm on call. Hospital has rooms, but gives them to the doctors. Have tried sleeping in an empty PACU, but too many strange noises and my anxiety was really high. Once, I had a kind peer who stayed over a bit (when I was very pregnant). She circulated my room while I put my feet up and had something to eat and drink. (Most likely crackers and juice). Admin never concerned themselves with us lowly bedside caregivers!!

Good Job for spreading awareness. I know of a house officer ,who died in a Car accident after 30 hour Shift as he drove home. Doctor lives matter.

I've never understand how a person can be capable of working 24 hrs straight...it's madness..I work 8 and I end up tired and I'm just working in front of a computer most of the time. I can't imagine a doctor or resident that has to be concentrated and ready to react

Yep I just finished emergency residency. This is such an issue. It also has zero hope of changing as long as hospitals are allowed to make campaign contributions to our lawmakers..

As a nurse, I do agree with this. Doctors are being abused by the hospital systems. They’re so overworked, I feel so bad for them. Sometimes we only have one hospitalist MD overnight for a 700 beds hospital. It’s so unsafe and ridiculous.

I literally have never understood this. It’s the same with EMTs and police officers. I wouldn’t trust anyone to do anything when they’ve been working for more than 12 hours straight without a break or a nap.

Rest of the world? Look at Indian Healthcare system or should I say the lack of it. 36-48 hour shifts for Junior residents without any holidays or monetary compensation. If a patient dies you get thrashed by patient's relatives regardless of whether it was medical negligence or not and politicians or judiciary won't do anything and just stand there like a dumb spectator. People here go to government hospitals for free and expect healthcare like private hospitals. If a private hospital bills 5 lacs people think doctors are looting patients conveniently overlooking the fact that 97% of the amount written on bill goes to administration. Healthcare systems all over the world is shitty. No wonder the older generations are forbidding students from taking up healthcare as profession.

This is also true for us in EMS and the Fire department, it’s tough to make decisions when you’ve been going for some 20 odd hours. Some days are better than others and we train and rest when we can but sometimes it’s nonstop. Respect all y’all doctors.

As a doctor, I can totally relate to this. I had to do duty straight for 100+ hours a week during the COVID-19 pandemic and my condition was the same as this dude and I was 7 months pregnant at that time.

the best thing to do after a 24 hour shift is sleep in your car for a little it sucks but it’s better than nearly dying on the freeway. Also, I learned the hard way you can’t pull over on the freeway. You gotta exit and sleep at the gas station

Lol imagine working in a building filled with beds and telling your employees they have to sleep in their car. It's trash. Yeah, go ahead and add back issues to the list of why burnout is so prevalent in medical professions. I'm not criticizing what you said, for the record, I just think it's absolutely ridiculous that your solution to the issue was to sleep in your car when you work in a place filled with beds. It's gross, dude, you deserve better.

Something we did in EMS was create sleep safe rooms for providers after their shift. We knew over 12-36 hours they could be exhausted, and we didn't want them driving. So we designated bunk rooms for these providers. One of the best things a residency can do for its residents given these shifts

I have encounter this similar problem at my restaurant job. Where I work you work for 5 to 9 hours then you get one paid 20 minute break! And I still think that’s not enough break time. I can’t imagine being required to work a 24 hours shift. That is so messed up to have to stay awake that long all at once

It is not only residents. Where I am from, some doctors will be working 24hour shifts all their life. You are 32 and tired after that? Now imagine doing it when you are like 60. This is what I can look forward to 😞

I am 64 and work 36 hr shifts. It is brutal and painful. Quitting soon. Just can’t take it any more. Added to that is the devaluing of my profession (MD) and it really isn’t worth it anymore. I would make more money bagging groceries for the same amount of time with a lot less stress

I don't have any friends at the moment who are resident doctors and my nurse friends all have kids they aren't about to be away from for a full 24-hr day of work. If they were going through this, I would offer to pick them up (despite hating driving.) I have worked two full time jobs before where four nights a week I had a four-hour window to sleep. You get a personality adjustment you can barely stand. I don't wish that tiredness on anybody.

In my country shifts like that are not regular, but can happen sometimes. One time a doctor have a small car accident with my family, and when my dad went to talk things out she was in the verge of tears due to being awake for more than 24h :/
