Crohn disease and ulcerative colitis are the two main conditions that make up IBD, actually. Gastritis is inflammation of (usually) just the stomach and can cause similar symptoms as IBD conditions.

Bile acid diarrhea. I was misdiagnosed for years as having IBS and it was actually BAD. Great to have treatments that are helping so much now. Life is WAY better!

@Holly Fisher, I’m no medical expert and I’m not suggesting what worked for me will work for you… However, a combination of Scoplamine patch (changed every 72 hrs), Omeprazole (20 mg daily) and cholestyramine powder (one scoop as needed when bowel movements get out of control). Hope it helps🥰

Thank you for validating IBS. It IS real and some patients (like myself) have suffered for years--particularly because they are viewed as undeserving of treatments by certain doctors who think it's "all in their head." But it's not! While it is very different from IBD, that isn't to say it doesn't need to be treated.

I remember being 4 years old and in agony on the toilet. What moron says it’s not real. With stress I’m constipated and without stress I crap in my pants just suddenly. Doctors are morons, the vast majority. Narcissistic control freaks who have zero compassion.

Out of all the GI docs I have seen/been a patient of you are probably the only one that makes sense to me. I’ve seen at least 10 GI Docs and have been on a ton of different medications that work for the first month and then stop. I’ve given up on my GI docs near me, I think I’m just a lost cause. But I find all of your videos very informative, especially as I’m premed.

Now this explains a lot to me, especially with my mom. Since being diagnosed with a mild form of lupus, she has had high inflammation markers in her blood. She also has diarrhea daily, sometimes multiple times, and is fatigued quite often. I'll get her checked out for IBD very soon.

I appreciate your mention of amitryptiline as an option- in 20 years no one has ever mentioned this Med and it has made a huge change for me. Keep doing what you are doing.

My doctor has (in the past few years) started using fecal calprotectin tests to allow me to avoid scopes. He still wants there to be a minimum of one every five years regardless, but I'm so relieved to not need a scope far more often. Oh, Crohn's disease, btw.

As someone with crohn's disease (one of the types of IBD) I can verify that it's a major pain in the @$$. Literally. When I was diagnosed at 12, the pain medication I was given in the hospital had to be gradually increased to the max for my age/weight because my abdominal pain was so bad, and I didn't even have an appetite for popsicles. More recently, I've been dealing with a fistula causing internal abscesses intermittently for the past two years. I currently have a small drain in place just above my tailbone/rectal area and am waiting for the next step, which hopefully I should hear from my GI soon...I swear after 20 years of it I should be able to get an honorary medical degree 😂

Thanks to your channel I finally made an appointment with a GI doctor after suffering for 22 years. They're going to get a bunch of samples then go to imaging and colonoscopy depending on the lab results

As someone who lived with IBS for years, the most relief for me came when (a) I gave up wheat, and (b) later with eliminating all grains. For me it was a life-changing experience, and I have not had to use prescription medication. Having a health practitioner to consult with along the way was key in my managing this disorder. I also now take Betaine HCL with meals, to help break down foods prior to leaving the stomach. Managing stress is also key, so moderate exercise is part of a daily routine. It's a multi-pronged approach, but I've had good results.

I would love the different sub-types of IBS and more treatment options! I think some people believe it's constantly running to the bathroom, but I thought there could be constipation related to inflamed bowel. Thanks!

I realize self diagnoses can be problematic but I'm sure a few people have gone, "huh, l wonder if that's what's been bothering me" and broke down and seen a doctor. Off topic but, 6 years ago l was attacked and got a knife in the upper arm severing my radial nerve. I quickly had nerve surgery. I was left paralyzed elbow down. Was told the impulses would regrow new routes at about a inch a month released and that was it. I'm in canada (luckily) so surgery was free but rehab is not. I remember being scared/confused about the future/what to expect and found little info on my condition. There needs to be a forum where one can get advice on such things from others who have gone through similar operations/conditions.

I have no idea what I have. I've never been checked. But, I finally made an appointment. My symptoms are: I have rectum bleeding, blood in my stool occasionally, loose stool 5 to 20 times everyday, I get exhausted and tired after going 5 to 20 times a day, I think stress is a trigger where it makes me go more and or cause blood in my stool, I get clumps of blood when I wipe (have seen it on my toilet papers occasionally), leakage of drops of blood occasionally, I have constant urges to go even after I've gone, everything I eat and drink just comes right out within less than 30 minutes, even if I hadn't eaten anything nor drank anything yet, I still go 5 to 20 times, ....

I have IBD and i used to do metagenomics work (in environmental samples, not human)... but because the environmental papers were much more scant, i read a lot of human based papers, and manu focused on the gut microbiome in IBD patients. More than one published scientific paper that was specifically talking about UC or CD used the IBS acronym. For IBD cases. I get that these people were probably coming from a microbiology background and didnt have an MD training but it's incredibly frustrating to see that issue in something that is supposedly peer reviewed. IBS and IBD can both be extremely debilitating but they are by etiology quite different

I’d love to see a video on Superior mesenteric artery syndrome. I was recently diagnosed and the third portion of my duodenum has completely collapsed. (Along with the narrowing that would be expected) I’d be interested in the treatments especially as the most talked about surgeon I’ve heard of has only performed his new procedure on about 4 patients so far

May I ask which IBD you have, how long you've had it, and if you follow a specific diet and lifestyle? I've had crohn's for 20 years and I'm trying to get to the point where it's controlled with as few meds as possible (I'm on humira every week, with occasional prednisone tapers as needed).

Foods to avoid with IBS: Stimulants: such as caffeine, added glutamate (MSG, "yeast extract", many others with sneaky names), aspartame Sugar that causes ups and downs in energy and also cause inflammation and excess stress hormones like cortisol to combat this inflammation.

I have a request please. *Exocrine Pancreatic Insuffiency* Is there any relationship with long-term diabetes? (Type 2 but 30 yrs in I live like a Type 1.) Ulcerative colitis and crohn's disease ("proud" owner here) are unfortunately common bowel diseases; I'm not sure how much they're known outside of the GI community, I never heard of them before my diagnosis but of course now I hear about them more because I look for info and support groups for them. On the other hand, when I tell someone that I have crohn's, they usually ask what it is, although sadly there have been times when they were like "oh my [insert loved one here] has that!"

The medical community knows IBS is real. The difficulty is that it does not have an easy fix. As for “rather having” UC for “obvious reasons”, tell that to the one third of UC patients that ultimately have a colectomy. The public desperately needs awareness and education on these illnesses. They are relatively common, but no one wishes to discuss the GI tract.

@@conniedelio When I was first diagnosed with IBD 20 years ago at age 12, they thought it was UC. Anytime I spoke with someone with crohn's, it always sounded worse, because whereas UC affects the colon, crohn's can affect the ENTIRE digestive system, from the mouth all the way down and out. If someone with crohn's has a colectomy, the disease can still occur anywhere else in the digestive tract. As much as I hated UC, I still felt "at least it's not crohn's." Well guess what. Turns out it only started in my colon--then spread up to the rest of my large intestine. I have crohn's. For the past two years I've been having recurring internal abscesses due to a fistula, so now I have a long-term drain in place around my tailbone/rectal area.

i wish they would stop pushing anxiety as the cause of ibs. I got reactive arthritis after food poisoning along with a host of other autoimmune things, but still tested negative for ibd. I suspect there's some unknown immune damage going on.

Can you explain divetticultus. I was in er Easter ct scan showed colon is as er Dr said mad and inflamed to not eat seeds skin and easy on fiber till I see pcp. I had upper lower gi last year ( age 47 routine 1st one) nothing seen than what should I ask Dr.? The Dr gave augnentin 10 day. I've been drinking ensure and small meals any advice will be great. Thanks for all you do.

I had a colonoscopy and they saw lots of inflammation and they say "shows evolving inflammatory bowel disease" however there's multiple medical documents say I have IBS not IBD. Was initially in ER for stomach pain, actually had overgrowth of bacteria in colon and I have a hole in my colon. Doctor said I was anemic and had some form of colitis. But I have medical documents stating IBS. Do I have both?

Ibs cal also look like gallbladder or pancreatic insufficiency. I get really bad diarrhea and bloating with fatty foods and I should see a gi doc but I always put it off. On the days I don’t have diarrhea, it’s super soft and mushy from fatty foods. I can have some fat and it will cause mushiness (like melted chocolate), but if I have a lot of fat, like an ice cream cone or fettuccine Alfredo/garlic and olive oil pasta, then that’s when I very diarrhea, bloating, cramps etc.

Are you referring to getting diarrhea during your period? It’s common for women to get cramping and diarrhea during their periods because of the effects of a substance called prostaglandins that trigger your uterus to contract and expel the lining of the uterus. The prostaglandins will also cause your intestines to contract and you have that diarrhea. You can Google ‘prostaglandins during menstrual cycle’ to read up on it.

@Doc Schmidt I would love for you to do a video on how to find a licensed and competent nutritionist/ dietitian for these conditions and SIBO. I know many can be approved with FodMap diet. But, in my state anyone can cell themselves a dietitian and/ or nutritionist and be complete idiots. Please help!!!

First off, registered dietitians and nutritionists are two different careers with different training. Check your state’s website for licensure requirements.

I was told that I had IBS, I had rectal bleeding but my colonoscopy came back clean. I was told that the bleeding could be caused by fissures. But now that you mention that there is blood and mucus associated with IBD, I am wondering if that could be it, especially because I always have a lot of swelling agent's in my system whenever I get my blood tested. But none of my doctors have ever mentioned that IBD could be a possibility.

What treatment options are there for IBS? Because I've never been offered any :/

STEM: sleep, thinking, eating, moving. Proper sleeping, calm and happy thinking, clean eating, proper moving such as exercise. This plus proper breathing will cure many ailments and is the best doctor! Please don't get hooked on drugs! Best to cure it!!

Medical school students learn about homeostasis in their first year. It means steady state. Taking drugs like amitriptyline will go against homeostasis (uneven blood levels). Best to cure IBS, not mask the symptoms. ☺️

@@Doc_Schmidt ..IBS is just one manifestation of a sensitized body part and a cycle of overexcitation. Just like esophageal spasm and globus hystericus. All three are 100% curable without drugs and if the patients know this they will have more motivation to cure themselves.

Doc Schmidt, I have a question for you: Can IBS develop into IBD? I suffer from IBS, as well as my dad. He passed away in 2018 after complications with his at that time Crohn's disease. But it started as IBS 50 years prior and went from IBS to ulcerative colitis to Crohn's.

I had IBS growing up, but didnt understand it at the time what it was since I was a teen. Would just go from periods of diarrhea to periods of constipation without any warning. Now I have UC. So just from my personal experience IBS can develop into IBD.

Not a doctor, just someone with IBS, but my doctor has told me that IBS is a miscommunication and hypersensitivity between the brain and the gut, hence why pain signals and abnormal bowel movements are trigged as the brain is trying to say something is wrong when there isn't. If I'm not mistaken, Crohn's and UC are caused by autoimmune disorders. I don't think IBS can develop into IBD as they are very different in how they work. It is possible to suffer with IBD and IBS at the same time, especially as IBS is more common than than IBD. I could be completely wrong, as I'm not a doctor, but that's what I think.

@@beansforjeans I think while you are correct in saying that generally IBS and IBD are this and that, I also think there are exceptions to every rule and I've often wondered if what we currently know about these diseases is just an oversimplification based on our level of knowledge up to this point. And what's really going on behind the scenes are things we just haven't researched enough about. For example, what role do hormones play in IBS and IBD? My IBS started when after I hit puberty and my UC/IBD started after I had children. Are these possible links? If so, what will that tell us about these seemingly separate diagnoses? So like I said, while I agree with what you and the doc have said, I do think there are a lot more unanswered questions by a lot of people who experience a lot of different things.

IBS cannot turn into IBD. They occur by different mechanisms

Usually IBD is redirected to Crohns and ulcerative colitis. Lymphocytic colitis has inflammation at the microscopic level (on biopsy) but the colon usually appears normal. So no, not quite the same

Had colic at birth, continued on till the enlightened IBS. Never was tested for allergens, but cousin one year younger had colic switched to soy formula, was cured no issues. Mom and aunt had same Dr. Makes no sense.

Question: how long should a person fast before a colonoscopy and NOT drinking that stuff. Would it work to fast longer??

By not eating your gut may slow down and won’t necessarily pass all the stool in your colon. The colon might be cleared of all stool, but it also may not be cleared. If your colon isn’t clear then the colonoscopy will be incomplete. Take the prep, mix it with yellow Gatorade if you need help with the flavor. Once your bowel movements are liquid and clear you will be good to go. I understand it’s unpleasant, but it is just one day in your life. That’s how I approached labor and childbirth, and that’s how I will approach my colonoscopy this summer. You got this! From an RN and a human being.

Also, if you are having skin irritation from the frequent bowel movements a little Vaseline will go a long way to soothing the skin on your bum.☺️

@@Crystal5672cats best advice I was given before my last colonoscopy prep was to use a barrier cream like you do for baby’s - worked amazingly (wish I had thought about that before previous colonoscopies).

Unfortunately, the prep is necessary--but there are different types of preps, so talk to your doctor about which one is best for you. Also, if you get the Golytely prep (polyethylene glycol-based), here's my hack: when I get to a point where I feel like I'll vomit if I drink one more sip of the awful-tasting stuff, I mix some plain Miralax into Gatorade and drink that instead. (Miralax is the same polyethylene glycol laxative. It's important to drink as much of the prescribed prep as possible instead of just miralax and gatorade because the prescribed prep has lots of electrolytes to replace what you're losing, but if you get to a point where you can't keep any more down and you're not "pooping clear" yet, you can supplement with Miralax and Gatorade until you run clear).

I've suffered from IBS and my husband has UC and my one rule is to have 2 toilet's in my house! I was diagnosed in my teens but suffered even as a child from both issues my husband was diagnosed 8 year's ago. His GI asked him if he was gay secretly or put thing's up his butt and clearly didn't diagnose him until he went to Mayo and right away they called it and didn't accuse him of anything!

Thank god, I do not have this. Only if I take too much fiber such as 2 tb of ground flax seed. I do have diabetes.
