Im a new casual dietitian in medicine (6 months) and the ICU has always seemed so mysterious and scary to me. Your lecture taught me a lot and I realized there aren’t that many huge differences between feeding critically ill and non critically ill patients! Thanks for the info

Dr. Eddyjoe, my husband (deaf) and I were married 36 years. I really appreciate your information, I am so distraught since my husband had only one doctor (not primary care). This was for CLL blood cancer. Chemotherapy and infusion which made him sick...usually infections. Targeted therapy called Venclexta. Other oral meds, either skin or lymph area infections...one in the groin. I know only one way to treat infections because I am not a doctor. I have made hot compresses every 15 mins. Of course drinking water to clean the system is best. I don't know, but the infections did dissipate if you stop taking the medication. Venclexta did not cause tumor lysis. I need to come back to later ....Thank you so much. Patty

Excellent video! Subscribed, thumbs up, and sharing with my fellow ICU RD’s. My only comment - despite there being little difference in infection rates between enteral and parenteral access, I would still say EN is highly preferred to PN as EN prevents villous atrophy, promote local immune function of the gut, and maintains overall GI structure/function. There is also more concern for hyperglycemia and refeeding syndrome with PN. Looking forward to a gut topics video. :)

Thanks for your comments, Christina. I’d rarely reach for PN myself, but I was surprised to find that out the infection component. I’ve maybe ordered PN on three patients in my career where I was the one who decided to initiate it. I’ll work on it this week. 💪🏼💪🏼💪🏼

Another great video! I would assume that there is a difference in outcomes with the amount of protein given with RRT due to the filtration of amino acids with dialysis especially those receiving CRRT. When I worked in the ICU, any patient on CRRT would receive a protein regimen that would mirror trauma patients (~2gm/kg depending on the patient). In the facility that I worked for, we were fortunate enough to have indirect calorimetry which was a huge blessing. Also, I appreciate that you touch upon infection and TPN. I understand the risk of infection, but much of the recent data shows significant improvements in infection rates. Of course EN is greater, but TPN is a life-saving resource when utilized appropriately and I would always advocate for TPN when applicable. And for those future clinicians and current clinicians don't forget to check phosphorus in refeeding syndrome - many practitioners forgot about this electrolyte for some reason?. Great videos as always! I hope you continue to shed light on nutrition and the importance of it :) -Marco
