I had a long conversation with my dad the other day about this stuff. He has been both the patient and the doctor in massive bleed situations so I thought I’d share some of his experiences and insight. About 10 years ago, my dad cut an artery in his wrist open while sawing tree limbs in the woods by himself. He was able to stop the bleeding by wrapping his shirt as tight as he could around his wrist but he had to constantly pull it tight with his good arm. Even so, he lost a ton of blood by the time he got the bleeding under control and he passed out once or twice trying to get to the neighbors property. Eventually, he got to the neighbors and they started to drive him to the nearest hospital over 45 minutes away. He passed out again in the car and a helicopter picked him up for the transport. On the helicopter, they used a blood pressure cuff to cut off circulation. His blood pressure was so low at that point it was easy to do. He said after over two hours of constant restriction on his wrist his hand was black and he was in the worst pain of his life. They actually loosened the blood pressure cuff at one point so the bleeding was still stopped but there wasn’t so much pressure (and he didn’t die of toxic shock syndrome). They preformed surgery after about three and a half hours of the incident so he was pretty close to losing his hand, but luckily he has full function. In his case, they sewed the artery back together and didn’t have to replace any sections of it. I asked him if it was possible for a doctor to do this outside of a hospital setting and he said that it would be possible to clamp off the damaged artery and he’d be able to keep his hand but with less function due to decreased blood flow. He said, in the worst case, a doctor could amputate a limb and clamp off all the arteries to keep someone alive. I asked him if he recommended quick-clot or regular gauze and he said quick clot works great but it makes it harder for the surgeon to operate on the wound. I asked him if there was anything you could do about lower abdominal gunshot wounds and he said that if you can get to a hospital in time intestines are fairly easy to repair and you can even cut out whole sections of intestine and live (with diarrhea for life). He said a gut wound causes horrific pain because the body has a lot of pain receptors down there. He said in the field you could apply quick clot powder into a lower abdominal wound and it would probably buy you some time (but make a big mess for the surgeon). He also said if you get a lower abdominal wound that stuffing it with gauze could help (which is contrary to everything I’ve heard on KZfaq). I asked if he could preform lower abdominal surgery on someone in the field and he said without anesthetic or at the very least morphine you’d be torturing someone and they’d probably be drowning in their vomit. He said you just couldn’t preform that surgery on a conscious patient. I asked if you could sedate someone enough by getting them hammered and he said not in that case. He said for wounds to the chest cavity there’s not really anything you can do outside of the hospital other than apply a chest seal.

Great info as always. I got taught to pack all junctions with regular gauze but ideally use hemostatic gauze for the pelvic girdle. Saves me from the temptation to always use the expensive stuff.

Thanks for the series. I'm updating my Rats with CATs. A couple things that might have been worth mentioning. In one of my First Aid classes, we learned how to use a basic wrap/ACE to create a pressure bandage without Velcro or fasteners (by folding, doubling back, and then looping through the fold... not sure if the technique has a name). The other is buying/using maxi-pads as budget bandage substitutes.

They made antibiotics for the infection a person gets from anything you use to stop the bleeding. FYI, the person who is shot will actually receive antibiotics in the hospital due to the possibility of infection from the bullet, knife, pencil, or whatever is introduced into the body. Yeah, I keep ABD pads and ACE bandages in my kit for pressure dressings. I use that just last week on a GSW at work since it wasn’t arterial. An ABD, 3” kling with part of the roll over the wound, and then an ACE. My vehicle soft kit has a box of ABDs, a box of 3” kling, and several ACE bandages in it.

I was really disappointed when I got my quick clot and saw 2ft. At that point I figured, first in followed by gauze. Would that work or just use it for a nasty cut?

You may have already covered this question in other videos . but does the notion of sanitary pads & Tampons hold up? I've heard many discuss the idea of "They are designed to plug up bleeding holes", but does that hold any real truth for gunshot wounds?
