I think you should replace the ibuprofen with acetaminophen as ibuprofen does thin the blood a little considerable amount as well as make platelets slippery, not to the extent of an actual blood thinner but it’s anti inflammatory characteristics can cause more bleeding. Another thing about the CAT tourniquet, because of K9 anatomy, they aren’t super effective on dogs. For our furry friends, you’d want to use a SWAT-T or a TAC MED SOLUTIONS K9 tourniquet. I don’t think NPA’s are going out of fashion, they work great and are very compact. The SWAT-T is good for applying pressure but one downside in my opinion is that because it’s rubber, you can’t see fluid leak through which might be a problem if you’ve packed a wound because you can’t see re-bleeding. A great option for both the fanny and the IFAK that I prefer is a simple ace wrap. Rescue essentials makes flat packed vacuum sealed ace wraps which would fit great in these kits. I agree that you don’t necessarily need a tourniquet for a young child, their blood pressure is considerably lower than an adult’s BP. Also with the circumference of their extremities being smaller you can easily occlude blood flow with various pressure bandages. Also, is there any reason why you choose to carry roll gauze over compressed on the plate carrier kit?

I carry most of these items in a Coyote Burrito that I keep in my backpack. As a college student, I always have my bag so its always with me.

ARS needles among other things can’t be bought by anyone but EMS and doctors now. I tied to buy a sup IFAK while being a TCCC Marine and I got told no by North American rescue. Thanks government, y’all are really saving lives with this law….

At work, I keep an ifak on my belt from GBRS as a bleeder pouch with just kerlex, abd pads, 4x4s, combat gauze, co-band, and a couple pairs of gloves in the tourniquet holder. I wear an ankle ifak from warrior poet society with tourniquet, chest seals, chest dart, combat gauze, abd pad with co-band, x shears, swat t, and another pair of gloves.

Ironically I'm in my 40's I started wearing a fanny pack for three years. I got a few items in my funny pack, from my pistol, few roll gauze, cat, couple hemostats, npa, narcan, nitro tabs, sugar tabs "wife's diabetic" hyfan, sisscors,14 gauge needle, scaple, disposable stapler, surgical tubing and sutures. I was medical personnel, executive protection, and worked patrol in my past life and I hate going in public unless I go places with my wife.

So first, I recommend (and I think every sane physician would) the CAT, SOF-T, or SOF-T Wide TQs primarily. But the SWAT-T TQ does in fact work and I've personally stopped a brachial artery hemorrhage with it. That said, the only instance I can imagine you would actually WANT to carry it over another option is if you cannot practically carry another option. Buy a CAT, but if you are either going to throw a SWAT-T or nothing in your back pocket, carry the damn SWAT-T. In a full IFAK, carry a decomp needle or a scapel (ideally both). I'll be thankful if I'm around and so would many other trained providers.

Agreed. Only thing I would add is the SAM XT, I like the click it gives and I think it’s very easy to use for someone with zero training should the need arise. As always, I’m going to say that we should carry CoTCCC recommended tourniquets as first line interventions, but I do like having the SWAT TQ tucked away for reasons we described in the video. Cool to know that they have worked for someone. I usually keep my scalpel, bougie, and ET bundle in my bag versus on the plate carrier itself.

@@cm2973 it depends on the service, one municipality is doing simple thoracostomy while others are still using needles. My current only allow for needle D.

For decompression needles put it in the mid clavicliar line, 2nd intercostal space - just above the 3rd rib (to avoid the bundle of nerves) DISCLAIMER: I'm not a medical professional

Depending on the needle and patient anatomy. Anterior Axillary, 5th intercostal space is a good choice for some as well. Great input though!

Would you consider carrying stutures, hemostats for vascular litigation in the field? Or getting that checked off for the list to add to your toolbox

I like steristrips for wound closure. They're simple to use and work well until I get to higher care. If you know how to suture then I don't see an issue with carrying them but I would not expect to be covered by like Good Samaritan laws or anything. I do like having hemostats or forceps in my kit for choking or clearing airways and then if you need to clamp off a vessel with it then it would be handy, I could really see that in an amputation scenario but would still lean towards a tourniquet first.

TQ could be the best and the worst thing in the case of gun wounds. If it's possible to transport wounded person so he will be treated in 1-2 hours, than yes, it's OK choice. Painfull, but guy won't bleed to death. But if you're in some shithole without possibility to transport injured person, than don't use it, unless that person is missing arm or leg.

We’ve seen tourniquets get left on for longer than that in the field with minimal issues. Absolutely apply a tourniquet to any major bleed on an extremity.

@@Nomadic.Clinician agree. Some possible tissue damage sucks but bleeding to death is worse. I think the army tested up to 12 hours and beyond with no issues
