Only people who live in fear needs to be concerned about positive or negative. Somebody who doesn't have fear will take everything neutral. When you are constantly surrounded by stupid fears like what if this goes wrong or what if that goes wrong, then you need to pay too much attention to this and this won't last very long so once you get comfortable of this way which is favourable, you're looking away from reality so when it strikes, it strikes immensely affecting you much more than peope who don't care about positive or negative on surroundings. Its a blockage to know figure new things and that blockage isn't nature

Karma does not mean action. Please understand this, karma means work. The word for action is pravarthana. Naturally karma happens only when there is consciousness, if you're living you move this body of yours as intended. Similarily, universal movement happens by the intention of consciousness. This intention of karma may or may not synchronize with human karma. When this happens friction comes between existence. Karma is always supported by memory, whatever work you do, doctor or engineer. Its only possible by a knowledge which exists in the form of memory. Just like that there is memory for universal karma as well that build up its fundamental laws. Cyclical and of consequence. The rate of becoming conscious of this universal karma and its intention is different for everyone. Just like how one person walks through the thorn, he feels pain so he understands the danger, if he is walking without pain sense he wouldn't know the danger of stepping on the thorn. Being conscious of your work and unconscious of your work are like that. So an unconscious one, as he bleeds more and more his body tells him the thorn is dangerous while somebody else might have noticed it on the first strike. This realisation happens to individuals at different rate depending upon their consciousness. In every work, there is memory involved and when this work goes against the universal consciousness, naturally its laws and your work collides. This happens so because ones character is determined by their past work and when past work is not rightous, their character establishes itself to bondages which goes against existential intention. You speak about how some are not punished? Its just like how a teacher can only punish a student if teacher has taught the lesson during the test and then the student knowingly ignore it. Ones who do know both suffer and change as per truth while those who don't know do not suffer its consequences until its knowledge is given properly. Punishing an ignorant child is unfair thats why. By punish, Once the realisation of truth has been given, there is no way it can be undone, either they accept it or suffer immensely when they rejected it. it comes off as a consequence of ones own work. So yes it depends on when the realisation comes, which is dependant on their consciousness, which only revolves around their memory, which can have established influences of past which again brings about a character to which universal karmic intentions don't support so they get knocked down and learn. Its more about becoming conscious about it that decides. If these so called people who do wrong are really in the wrong, then give the damn knowledge of truth and see what happens if they don't take responsibility of possessing that realisation
