Dr. Halstead diagnoses his patient, a jockey, with an eating disorder.
Season 2, Episode 10:
Dr. Rhodes (Colin Donnell) and Dr. Charles' (Oliver Platt) heart transplant patient gets into a car crash, revealing issues that could complicate her surgery. A past arrest causes trouble for Maggie (Marlyne Barrett) when the arresting officer is brought to Chicago Med with a serious injury. Elsewhere, Dr. Manning (Torrey DeVitto) and Dr. Halstead (Nick Gehlfuss) must defend their actions when explaining a patient's cause of death. Meanwhile, Dr. Halstead attempts to treat an uncooperative horse jockey. S. Epatha Merkerson, Yaya DaCosta, Brian Tee and Rachel DiPillo also star. Kara Killmer, Amy Morton, Ato Essandoh and Eddie Jemison guest star.

In general people treat ED as an independent illness. That tends to be true for anorexia, but not always. And also not for other eating disorders. In the case of this guy, his ED is a symptom. He genuinely cares about his career to the detriment of his health. It's an extreme version of what many disposable manual workers do. He just happens to have a job that has very specific demands from his body.

It's sad that this is still how it goes. The Iceman (George Woolf) a jockey who once rode alongside the Cougar (Red Pollard) was a diabetic. He would purge himself off of food and eat high quality steaks to maintain his diabetes. He did this for years until one day he fell from the saddle mid race he died the following day. The day he died he had left his lucky kangaroo leather saddle in his car. He was 35 years old when he died.

What makes Dr Halsted different from the other doctors is that he actually engages with the patient and explains the diagnosis or what needs to happen. However, some of the other doctors kind of only do the checkups and don’t engage much with the patients. Dr Halstead is such a good guy!

I've been going throught something like a social ED, whenever I go out like a friends house or a restaurant and eat, I feel nauseaus, I feel that my chest is pressing so much and start feeling really anxious. But when I eat in my house, I feel safe, I have social anxiety and my therapist told me that it kinda mixed up with food. I believe that eating disorders should be treated tactfully ... A person suffering from anorexia, for example, cannot be gorged on stuff full of fat until it becomes fat. It doesn't work like that ... First, they need to admit that they have an eating disorder and wants to be helped ... and then they have to be looked after by specialists such as dieticians, psychologists / psychiatrists and a team suited to the situation. Forcing someone to eat is counterproductive.

Wait.... diuretics aren't drug enhancing performance, right? Why is it not allowed to take diuretics before the race?

Technically it is when you are a jockey. That makes you artificially lighter so faster. Found an article saying it was banned because jockeys abused it and thus couldn't make proper decisions in the saddle anymore... If you think about it, it's a bit as if sumos took close to death salt amounts to get more water weight just before a fight :$

Why do they always go to NG tube for calories. Yes if they refuse oral then ng but don’t always need to do it straight away (but honestly if eating disorder often will refuse ng and require capacity assessment). Just wish they don’t advertise this as first line in tv shows

I don't understand why there would even be a weight limit for a jockey. How does added weight help you? Wouldn't it hurt if anything?

idk why he said he would fix the mitral valve prolapse? I have mitral valve prolapse but they didn't do anything for it the many times i've had an EKG. I was never told it was fixable in any way through anything non invasive.

@Julie B if thats possible that’s interesting! Me and another family member have had it our whole lives so for us it’s probably nothing to do really

@Julie B ah yeah. I just figured they might try to repair a congenital defect anyways, ya know! But probably sometimes they don’t if you’re doing well etc

If he where to find out the doctors tipped them off he'll loose the last ounce of trust he has in them and won't seek treatment if he can avoid it. Dr. Charles may be partly responsible if he dies later.

They should have let the guy finish his race. One more ride to get to his derby, right? And then make him promise to come in for treatment.

He may as well never have come back. Plus, he was dying, so if he died during the race his family could have sued the hospital for letting a suicidal patient out.

@@annabees No, they couldn't. A patient is allowed to leave the hospital against medical advice. Which he did and I expect had to sign to leave. No doctor can force a patient to be treated. I assure you, in real life Dr. Charles would have not acted like that.

@@potocatepetlI don't precisely know the laws in America but for me it's kind of common sense that you shouldn't allow a suicidal patient to leave the hospital and that you *can* force treatment on them. And that family could be angry if you didn't. In France we have something called "non assist of person in life-threatening situation", and I'm pretty sure an average lawyer could say it's exactly that here. ED plays with your brain and you basically become technically suicidal (doing things to end your life quite quickly) without really realizing it... That being said, I agree it's an impossible case for a doctor here. Either you choose to not trust patient, save his life but might be let him sad (or even depressed) for the rest of his life. Either you choose to trust him and might be he'll die during the race...
