Dr. Ebraheim’s educational animated video explains the condition of knee pain and arthritis - total knee replacement.
This video describes knee pain, knee examination, knee diagnosis and treatment .knee pain relief may need surgery .this video describes knee arthritis animation, knee arthritis symptoms, diagnosis, x rays and treatment
Knee arthritis treatment will include physiotherapy, knee injection and total knee replacement.

The cartilage of the knee is complex and it is made of elastic comprehensive structure.
The normal articular cartilage is called hyaline cartilage; it provides a smooth, gliding surface to help the motion of the joint.
There is about 2cc of the synovial fluid inside the knee that helps in the motion and lubrication of the joint.
Between the hyaline cartilage which is called the articular cartilage you can see the meniscus, the lateral and the medial, the meniscus is a shock absorbing cartilage or cushion between the articular cartilages.
The hyaline cartilage has 4 layers:
1- The superficial layer.
2- The middle layer.
3- The deep layer.
4- The calcified layer
After the calcified layer you find the bone.
These cartilage cells are supposed to live forever.
Good cartilage cells are sterile; they can’t make more cartilage if these cartilage cells are destroyed.
If the cartilage is subjected to excessive wear, trauma, injury, overuse, excessive weight or improper alignment, then the cartilage will wear away leaving the bone to rub against bone.
The cartilage doesn’t have the ability to heal itself by hyaline cartilage, but sometimes it can heal itself by inferior type of cartilage called fibrocartilage, especially if the area that needs to be repaired is small.
What is the treatment of the arthritis of the knee?
1- Medication: people respond differently to medications, the doctor select the type, dosage and duration of the treatment, the medication has to be safe and effective.
Ask the doctor about the side effect of the medication.
There are several medications for example: NSAIDS.
2- Losing Weight: it’s advisable to lose about 5% of the body weight, especially if the BMI was above 25.
3- Low Impact Physical Therapy: such activities like swimming or cycling, adds less stress on the knees, life style modification to protect the knee will slow the progress of the arthritis, there is a strong evidence that physical therapy will help the patient.
The physiotherapy will decrease the pain, will improve the function, increase the strength, the range of motion; physiotherapy should be individualized with program that meets the patient’s needs, the life style, and expectations.
4- The physician will also use intra-articular injection of steroids; viscosupplementation, or hyaluronic acid, injection will relieve the patient’s pain and disability.
5- Other injection methods:
- Gene therapy
- PRP
- Growth factor
- Stem cells
6- Other treatment options:
- Acupuncture
- Massage
- Glucosamine
- Chondroitin sulfate
- Valgus directing brace
- Wedges in the foot
- Arthroscopic debridement and lavage
7- Assistance devices:
- Cane
- Shock absorbing shoe
- Shoe inserts
- Knee sleeves
- Support brace
8- Arthroscopy shouldn’t be done in arthritis unless there is loose body or meniscal tear causing mechanical symptoms of recurring lacking, catching, swelling and pain.
There are some guide lines from the American Academy of Orthopaedic Surgeon AAOS: these guidelines are suggestive, and the treatment of the arthritis patient should be individualized and based on:
- The doctor’s clinical judgment.
- The patient’s clinical situation.
- The evidence tested and published.
It is a combination of all of these factors.
If it works for you, it may not work for your neighbor.
So basically arthritis can be minimal, and anything can help the arthritis ion this situation.
But if the arthritis is moderate then the physician has many options for treatment and none of those options are predictable in the result.
When the arthritis is severe, total knee is the most predictable option for treatment of arthritis.
You have to reduce the pain and improve the function of the knee.

I’m 35 and suffering from severe pain in my knees due to arthritis… I have been taking injections but still experiencing flares. I hope oneday they can cure this disease…

I tried all the medicine 💊 But one thing really helps me it reduces the arthritis attack Usually i am having it like 4-5 times a year But now only 2-3 times a year. I tried turmeric natural Crushed it and put in a water Bring to boil then drink it Don't matter if it's hot or cold but it helps me a lot Less expensive More cure. Do it.

Excellent lecture on Dr. Ebraheim. You made the structure of the knee so clear with your talk accompanied by visual aid, as well as providing treatment options. If only all doctors/specialists offer the advice, patients would make informed decision about their treatment. I totally understand that not all treatment would be suitable for all patients. I'm 50 I just had 2 total knee replacements my were so arthritic badly I was walking around on a partially dislocated knee cap on my right knee. My orthopedic Dr told me my knee was the worst he seen ..that first surgery was excruciating painful but now I can finally walk without being in severe pain...

Hi, I’m 53 and I’m waiting to have both knees replaced too. I’ve been unable to work and I’ve waited for nearly 4 years for my ops. My surgeon likened my knees to walking on broken knees! It’s agony and it’s been very hard. I cannot wait to have my surgery. I know weight loss will ease things but as I’m so inactive due to the pain it’s very difficult. I’m relieved to hear that your surgery has worked and your quality of life has improved. It’s given me hope. 🙂

I’m 47 , this January I thought I had a simple knee injury from walking on a treadmill. The ex-ray showed arthritis in my right knee. There were no signs it just came🤷🏽‍♀️. Thanks for the video

It was simple, I didn't take medications, I just started to eat tahini paste with every meal I eat, just pour a little potion on your food . After a month the pain fades.

ASALAAM Alaikam Dr. Thank u so very much. This is great info...I am female age 88. I have bone on bone. Condition but rarely have pain. I've fallen lots in my life onto my knees. But I've always been a walker..also live in rural conditions from age 13 to 16 where I worked in fields. It consisted of walking sun up to sundown picking, choping cotton and other work. Where I was on my feet all day.. Then when back in city I walked. After age 30 or so I began walking for exercise. But now seems age has caught me. But I have little pain. Every so often my left knee has a twinge I put Salon Pas pain pad and in maybe 5 min. Pain gone. Right knee was broken in a fall Nov 2020. Never any pain. I was in hospital a few days having also broke shoulder on left side. Pain there in arm but nvr in knee. Dr told me once keep the synovial fluid going you ll be all right. I feel like I went by thar and it's help. I understand better now. Also after my fall. Due to nursing home diet for 2 mo. I lost 40 lbs. From apps 180 to 145. Fluctuating between 140 and 149. So as u say I believe that gas helped. Shukran Dr. I appreciate u. .

I’m a 22 year old male with arthritis on my left knee, I can’t bend my knee down and I can’t even use my toes to walk it hurts my front lower knee cap, when I bend it towards my stomach it stops midway and I can feel the inflammation on the side of my knee caps compared to my right knee where I bend it and it’s still squishy, not hard like the left knee

I’ve got S4 OA in my right knee and S2 in my left. I’ve seen my orthopaedic surgeon and he refused to do my knee replacement due to my other health issues and a bone fissue & because of my TBone score being nearer 3 now he said?? I’ve got lot of damage in it at 52 I feel more like 62 and on long term sick at the mo due to be finished from work soon. Working in retail made my knees deteriorate so much faster. Off years wear/tear off exercise and radical hysterectomy/osteoporosis my bones are weaker due to the latter doctor said wouldn’t risk the replacement and explained why. I’m going to be in increasing pain for life, great 😢

Very informative and clear. Indeed supplements of chondroitine and glucosamine haven't improved this chondrocalsinosis. Hyaluronic acid injections have, though on a temporary basis, but only if flexion of the joint is not to full bending! The 'pocket' will be 'popped' open, releasing the precious fluid into outlying tissue and lost for the joint. Good luck everyone !

It is an awesome one off product for learning how to fix chronic knee pain minus the hard work. Ive heard some super things about it and my m8 got great success with it.

In youtube k nuskho se kuch nahi hota maine bahut try kiya hai. Mere ek fb friend ne mujhe ek viadh ji ki medicine suggest ki thi. Fir maine us dawa k sath subah walk karna shuru kiya, ab mere knee ka dard bilkul thik ho chuka hai..

hello Dr. Nabil! can you explain to me if how a Sprain/Strain Knee can end up being Osteoarthritis( of which is bone on bone), then also have a possibility of causing hip pain on the same side of the worst knee pain. Explain why All of that happens, on top of telling me what pain reliever can help with Pain and also cause weight loss. Thank for your teaching mechanism!

Thank you for sharing this video. May I ask a questio. I had the left knee pain for almost 2 yrs. I took xray but it's result was normal. I watched on youtube about exercises to ease the pain and I do most of it, but still pain. When I walk is ok, but when bent my knee is so painful. Even food I followed some shared food for the knee pain , still the same. What other way to let the doctor know the cause of the pain? Thanks.
