Thumb Osteoarthritis is a degenerative condition that effects the small joints of the thumb, particularly the basal thumb joints (CMC and STT joints) . This video aims to provide evidence based information about Thumb Base Osteoarthritis by answering the 3 most common questions asked by people who have been diagnosed with the condition :-
1) What is it ?
2) Why do I have it ?
3) What can I do about it ?

I had severe Thumb Osteoarthritis (bone on bone) in both of my hands (left and right). I tired just about everything including laser warming and nothing helped. Finally, I had surgery to remove the Trapezium and suspend my thumb with tendons between the metacarpal and the index finger - like a suspension bridge. I had this done in Colombia (I lived there for 5 years) by a specialist doctor there (recovery time was 3 months to hand use - full recovery 6 months).. My left hand is going on 9 years now (I'm 73) and my right hand is now 4+. I play guitar and have full hand movement and strength which in my opinion is a very good test. No pain to date. I'm not recommending this to anyone, just sharing my experience. I hope you all the best in your journey to reduce / eliminate your pain.

I'm a 76 yr young Great grandmother n at age 40 i experienced such terrible pain in my left thumb joint that all I cld do at night was to lay there with my hand still as can be on my chest. Tears just rolled down my cheeks n all I cld do was pray. I saw a hand surgeon n he did surgery n replaced the cartledge. After recovery, I started catering with my aunt. Oh, by the way, I m right handed but this was in my left hand: I m ambidextrous. After 5 yrs of catering it started hurting just as bad as the 1st surgery. So my hand doctor did what he called a "capsulation". Well, it didn't last long because I don't stop. I was pretty active. The cartledge had broken in half n now I have bone on bone.😢. Anyway, I had a repeat production on my right side, same thing. He did a tendon transfer n anchored a "tiny" anchor where the cartledge went. I have flare ups now n then, but it seemed to help more. I was dx with osteopinia n I believe I have osteoporosis now. 4 fusions in my neck n in my hands my fingers have been turning n becoming disfigured. As I said before...I'm an active person, but for the past 15 years I've gotten progressively worse. I have full blown Fibromyalgia. God has given me the strength to endure. When the pain gets too bad, I stop n rest for how ever long I have to. My husband is awesome with The care he gives me n he has an autoimmune disease; Systemic Scleraderma. If you have constant pain go n see a hand specialist, mine was great. I pray God touches each n every one of you who goes through this cause it is truly painful.

Yes we mustn’t underestimate the power of prayer in our lives. Every pain I get I touch the area with holy water and it goes away or lessens a lot. I’ve managed to keep at bay any interaction with health providers that way. I’m 75 and take no meds at all.

I got this last year from eating lots of apples and pistachios. In both thumbs. The right became very painful after a injury. I seriously prayed about it and the cause was revealed. I recovered after stopping the nuts snd apples.. resting my hands and praising my Lord and my God Jesus. Be blessed and a blessing.

@@EuroWarsOrg. Agreed. ACV is tops. Have some in a covered jar near your relaxing chair. As you watch tv, use cotton pads soaked in vinegar. Drape a tea towel to hold in place, it will build up warmth. If you secured with blue painters tape, you could wear to bed. Doesn’t work unless you use it….be persistent. Keep calendar notes or make recurring email alerts. Something else that works …a pinch clip, something used on the refrigerator door. Has to be just the right one, easy to operate not too tight, don’t cut off circulation, gentle pressure, use the the V -meaty part next to the thumb. You'll know it when you've got it. Great topic. I'm 72 and have used my hands a lot in repetitive tasks almost all my life. I think I am at the stage where the small thumb splint will work. I am also at the age where various doctors are needed ( I live in the USA). I guess I'll add one more! I'll tell my sister about this video. Thanks.

Also looking for Occupational Therapist, which this Surgeon prescribed, when I balked at the offer of injections. I told him I'd like less invasive options first. He never offered any exercise & wouldn't even explain the brace & how it's supposed to work like you did.

Thank you so much for this video! You are the first to explain this problem so well. I woke up one morning in 2020 ( in central US) after months of being very ill with COVID to find my thumb laying flat to my palm & severely limited in being able to open back opening up my palm. I have worked on it on my own since as no medical person would offer any advice & now I have it 10-15 degrees yet from being able to totally open back making my palm flat. Common sense drove me to get a supporting brace & I continue to work it trying to gently flatten the palm & get the full range of motion returned. Your video has encouraged me & suggests I've been doing the best course of action for it. I am subscribing to your channel.

Glad it was helpful. That's a good result from the injection, its just a shame it didn't last longer. I have patients who I inject every 6 months for this and they are quite happy with that regime, as they aren't interested in surgery. If you have it done again, I would suggest still using the splint after but just for strenuous activities, in order to avoid a flare up and therefore prolong the life of the injection. Best wishes. I have seen different 2 different physios recently about this condition and each physio said I had to wear a different type of splint . I now use the both black and brown types. However neither physio explained why I should wear them or how they should help. All I was told is that “ at my age I have worn my joints out “. Having seen your video I now understand a lot more about OA and the why and how for splint use helps. So thank you for making OA of thumb easy to understand.

I'm a guitar player and have to practice every day. If I stop regular practice the pain in my left thumb goes away and comes back if I start up again. There's no way I could use a splint and still play and I've experienced injections for a frozen shoulder and the cure was worse than the condition. I think as I'm 81 years old that I just have to live with the pain which isn't unbearable.

Thanks for commenting. I suspect your thumb arthritis is late stage. The injection might be worth a try but do get it done under image guidance to avoid guesswork and potentially irritating the joint even further. Also, don’t discount surgery. It can give great results and age is not a barrier to it. I would ask your doctor for a specialist hand opinion. Best wishes. Stephen.

The cortisone injection was not well placed if it did not work. And being 81 you should be retired, just stop playing till the tendons heal. Massage castor oil slightly warmed not hot then wrap something to keep it warm. I got a pair of cosmetic electric gloves . They have low medium and high levels. Keep it on low as long as you can,but do unplug before you go to sleep.

Thanks for your detailed description of the problem and what do do about it. When I was diagnosed, all I was told about my splint was to hand wash it. Since i still have intermittent pain, I'm going back to using it 24/7 for a while and then try your "weaning" procedure.

I recently had the surgery for thumb osteoarthritis and am very happy I did. I tried cortisone injections, they didn’t help at all. I used the gel (Voltaren) I used ibuprofen, and splints. The day the nerve block wore off I had no pain at all. I did PT for several weeks and can now do everything I couldn’t do before the surgery. If the recommended courses of action described above work, that’s great. They didn’t work for me and surgery was the only and best option. Within the next few months I’ll be having it done on my right thumb and am looking forward to having the other thumb being pain free.

Thank you for that informative video! I have no doubt my thumb arthritis comes from being an auto worker!!! It’s embarrassing when I have to get my wife to open jars for me. I’m going to be mostly retired at Christmas and hope the thumb will improve with out all the repetitive work! I’m certainly going to keep those splints in mind, thank you!

It’s really common for thumb base pain to settle after retirement so I hope this holds true in your case. In the meantime the splints can be very helpful.

I recently had surgery to remove the small bone where my arthritis was. Its such a long healing process (12 weeks). I had tried everything but within a year of treatments and braces and cortisone shots I couldn't use my left hand, so that sealed the deal for getting the surgery. I can't wait for this to be over. I appreciate your explanation bc its the most thorough I've found.

What REALLY HELPED me with my cmc joint swelling and pain was taking turmeric capsules with black pepper in them. Pain went from 8-9 to maybe 1. I got them at Costco. I also used a cream with bee venom, called Apireven. It's greasy so I only used it at night. That helped a lot but the turmeric and black pepper capsules did wonders

I'm a gardener, who has a passion for pruning and taking cuttings. I never had this thumb pain until a few months ago (aged 57). Can't imagine having to give up gardening, but I do have a tube of that Capsicain cream (I use that on my back occasionally. Got it from eBay years ago, probably before it became a prescription medication). Don't like to keep bothering my doctor with my joint problems, but if this doesn't work I'll get a course of anti inflammatory tablets again. I had those for my feet and ankles one time because they don't flex too well any more and the tablets worked a treat.

For over a year I had pain at the base of my thumb and wrist. I thought it might have been tendonitis. I had considered a wrist brace and found some relief using an arthritis cream. I finally got rid entirely of the pain when I started sawing logs in the spring. A completely new activity for me. Within a week the pain had disappeared and I have been pain free for over 6 months now. Seems like strengthening larger muscles is what was needed.

USA here. I’m seven months out from surgery. I had this condition but I also had bone spurs where the bone was worn away. It was a simple surgery on the physicians end. It was my first big surgery so I wasn’t prepared for how much the healing would take. But u have to say I’m glad I did it. I’m about 65% healed and can do many things again. I do have one issue and is that my thumb is hyperextended. No a big deal. I’m having the doctor take a look at it. If it wasn’t for that I’d be 80% healed. That’s good because the doctor said it would take a year.

I suffered this many years ago. The pain was violent and painful, although not all the time. But carrying or holding things was really difficult. Simplest example was that it was really difficult to even carry a cup and saucer in one hand. Thankfully I was told to try magnesium oil and it's was a life changer, so much so I am now pain free. Better still, I always wiped xs oil from my hands on my knees and an issue in my knee has also now gone.
