[Patch 6] Bringing a Vlaakith-sworn Lae'zel in the Astral Prism prevents the player from transforming into a mindflayer. Either, you have to side with the Emperor (Orpheus dies), or stand against the Emperor but Orpheus is forced to become a mindflayer. Karlach can become a mindflayer when brought with the party but not the player.
There's a workaround, however, if Lae'zel is ungrouped from the party and placed far from the meeting with Orpheus-Emperor, Orpheus can acknowledge the player's intent to become a mindflayer. Afterwards, Lae'zel can be grouped back to the party--which results in a dialogue bug where she is BOTH LOYAL to Orpheus and Vlaakith.
When Lae'zel leaves Faerun, the cutscene for saving Karlach will follow and likewise, Wyll can also jump in to save her if he's the Blade of Avernus. :)
