Hydroquinone remains as one of the most popular pigment inhibitors. It is still the most effective treatment for pigmentation including melasma, post acne pigment & as a bleaching cream in many Asian & African countries. Hydroquinone was made famous by Michael Jackson, who overdosed on this (as well as propofol).
Concentrations range from 2% to 8% or higher. In Australia this can be purchased over the counter with concentrations of 2% or less. Countries in Asia such as Japan, & many African countries ban the trade & sale of HQ.
Safety concerns are founded by murine studies (rats). Not substantiated. Biggest concerns are, firstly contact dermatitis with higher concentrations, tachyphylaxis (stops working) & exogenous ochronosis (hyperpigmentation instead of fading pigment). Majority of side effects can be mitigated with appropriate use & monitoring. Most dermatologists will limit its use to 3 to 6 months maximum. We rotate other pigment inhibitors such as arbutin, ascorbic acid, licorice root, soy, flavonoids, bearberries, cranberries, & exfoliants such as AHAs.
Accurate titration and application of hydroquinone will reduce side effects and maximise its use.

Great video! Thank you! I use/ used most of the actives mentioned, but if I don’t apply sunscreen (even while in the house during lockdown) I instantly see my pigmentation surfacing again.

I agree. I can’t believe Dr Dray is not clear about it, is it in adapalene and differin? I used this on my face not realizing this may be hydroquinone! I am so mad right now.

For people like me with darker skin (FP type 6) a HQ-based product is often the ONLY resort for PIH, because most ordinary acids don't penetrate deep enough into the skin to support a complete renewal process.

Thank you so much for the info, Dr. Davin! I’ve been trying to treat hyperpigmentation from ingrown hairs on my legs with products made with alfa-arbutin and niacinamide as well as ahas, but I think I’m going to try laser treatments going forward.

Hi Dr. Lim, I would like to know your opinion on a prescription lightening product. It was developed by a well known aesthetician in LA. It’s mainly sold at her medical spa so I won’t mention the brand. The product has a lot of good reviews. Here’s the main ingredients: hydrocortisone 1%, hydroquinone 8%, tretinoin 0.1%, kojic acid 3%.

This is very helpful. Thank you so much. I got prescribed 12% HQ with Kojic acid. My skin gets very irritated, red, and flaky. I have tried skipping a night or 2, using moisturizer before and after but my skin is still very irritated. I’ll try dilution next.

Hi, not sure if this is late to reply. I was prescribed tranexamic acid by my skin doctor. Cleared my pigmentation for a while, then my skin color became way too light, kind of pale looking. And whenever it is hot, my face would feel hot and become red, even when running. I stopped using it, then pigmentation started coming back again. I was using it only during night time.

Do not use benzoyl peroxide, hydrogen peroxide, or other peroxide products when using hydroquinone. Temporary dark staining of the skin may occur.👍

My derm prescribed me hydroquinone... but also benzoyl peroxide. I keep hearing that mixing them is bad, but are they compatible if I space out when they are used? Might have to make a call tho!

I have hypopigmentation (white spots) near my chin and hyperpigmentation on certain areas of the skin because of acne. My doctor recommended using hydroquinone for the whole face for even tone. Will it not further lighten the white spots?

I used hydroquinone on my face every night for two and half month. Saw visible changes:dark spots faded away,skin lightened. However once I stopped my skin went back how it was before hydroquinone. Now I have started using it again and the results are again showing. So from what I have learned and read, you gotta take breaks when using this product and during this break you should use other products that help with pigmentation. I will give vitamin c a shot after I am done with my second phase.

I used hydroquinone for 3 months and had great results, but my doc warned that if I don’t use sunscreen religiously, it will return. So, I use a moisturizer that has an SPF, and have not had any recurrence of my dark spots!

Use for 4 months, every night, tretinoin all over the face and hydroquinone in the dark spots on my face; They decreased a lot, now I only use them once a week as "maintenance" and since I started the treatment I always use sunscreen during the day

I use Hada Labo Shiro Jyun Premium which has tranexamic acid in it and it helped a lot in decreasing the appearance of my pigmentations. Tranexamic acid also brightens my whole face complexion. It will not make your face 1 shade lighter, but brighter and healthier.

I am using 3% tranaxmic. It peeled my Skin at start. my pigmentation fading now my pigmentation faded to 70% but I used hydroquinone with it. But 30% is remaining I don't know will it fade my pigmentation completely. Oral tranaxmic acid is the king of all procedures but it may cause blood clot on safer side we have tranaxmic dermal injection also those are more effective and less risky. I am planning to go with dermal injection

I use diy unbuffered ascorbic acid solution on my face, and keep it stored in the fridge so it doesn't oxidize. My ph strips say about 1.5. I'm too lazy to add baking soda, and I like to think that my super acid vitamin c has amazing penetration properties. You would think that between the microneedling, the vitamin c, and the 20% glycolic acid pads, that I would look like a porcelain doll at this point. I filled in a rolling acne scar totally, but a little stubborn hyperpigmentation remains. Maybe I'll try hydroquinone next.

@@SweetUareDesi I have the Dr. Pen M8. Microneedling probably won't work well on protruding scars, but for pits and rolling scars it's been effective for me. I have not needed lidocaine cream for the needle lengths I use. I use 1mm on most of my face and even my neck, 0.75mm on on my nose and around my eyes, and 1.5mm on my scarred cheek area. I do it monthly. For these lengths you really need to give your skin time to recover. Definitely use a longer length on your scarring, but you could use 0.5mm length elsewhere is you are looking for collagen induction. I see that you are young, so you may just want to focus on your scarring and ignore the rest of your face. Use numbing cream if pain is an issue for you, although you could try an ice pack. Expect some bleeding with a longer needle length. Your scars have a puckered appearance because scar tissue is acting like an anchor and pulling your skin down. This is why you need a longer needle length for acne scars. You need to break up the scar tissue. Clean your face with rubbing alcohol. I like to spray my needles as well. Your skin will look awful for two or three days afterwards. I use hyaluronic acid as a slip. Don't inject a lot of foreign substances into your skin, as you may end up with unsightly granulomas. I won't use harsh actives for a day or so after I microneedle. My skincare regimen changes periodically. I did start buffering my diy vitamin c serum. I use less glycolic acid since I started cleaning my face with castile soap with tea tree oil, as my skin has gotten more sensitive to acid. My husband complimented me on how smooth my skin has been looking. Next I will switch to a retinol serum and see If I can get even better results than the glycolic acid. I just got a Mini Hifu, so now I have to figure out how and when to integrate It into my routine. Just for fun I do an occasional mud mask on top of everything else. My skin looks brighter afterwards. May your face be smooth as a baby's bottom.

Begging for advice. So this might sound silly.. but I have melasma on my forehead and I'm tempted to wear a silicone patch on my forehead under my hat to just keep it totally eclipsed from the sun. I wear a hat every single day, vitamin c & licorice root + SPF & moisturizer in the daytime, and use SPF at least every 2 hours, retinol & hydroquinone (cycle on & off) at night... my melasma isn't budging, its been years and I've been consistent pleeeease is there any advice?? I don't know what else to do.

Hi Dr Lim, I was wondering if you've heard of John Plunkett's Superfade Face Cream and what you think about it? According to the packaging, it has hyrdoquinone (20mg/g), salicylic acid (15mg/g) and padimate O (30mg/g). I bought it from Chemist Warehouse and have been using it without consulting any doctor about it 😕

Simone, Superfade cream is EXACTLY what I have been using on my elbow for the past 2 months. I only used it at night for the first 6 weeks to get used to it, then over the past 2 weeks I have been using it morning and evening. It really works well for me, I like watching the progress. I have started using the cream on my chin and neck tonight because I have very dark PIH due to plucking ingrown hairs. I have a pretty safe routine of using a Zinc oxide-based SPF 30+ and an acids-based exfoliant, so I am hoping that the Superfade cream will be giving nice results soon. Ps: I am Black African, so any change to my skin tends to be quickly visible. Things might go differently for you. ^__^

I have a question sir, around my beard area it is much darker than the skin around it similar to the darkness of your mustache compared to the surrounding area, would a hydroquinone cream fix that?

Hydroquinone ruined my bright white eyes. I was prescribed a prescription strength dosage for 6 weeks to treat hyperpigmentation . I experienced quite a bit of eye irritation but chalked it up to seasonal eye allergies. I later discovered that the product was getting into my eyes through my skin ( even though I was not applying it around my eyes). By the time I stopped using it, the damage was already done. I developed a small pinguecula in my eye and I have visible red veins now. If you experience eye irritation while on hydroquinone, discontinue use immediately. I wish someone had warned me

Why is it you don't recommend this for the 3rd main type of pigmentation- freckles/sun damage? You only mention melasma and post inflam. What do you recommend for this?

What is your opinion on a person who use hydroquinone compound via dermatologist for melasma and brown spots .... do you use retinol / retinoids at same time? And when I’m in the “ off “ time from compound ( I do 6 weeks on 3 weeks off ). What products are good to use to help keep melasma dormant. I have script for azelaic acid from Dermatologist?

Hello thanks for this video I am 43 from Scotland. This is the story of my life exactly as you have described pill in my late teens early 20s early 30s controlled and swept all under carpet. I'm in my 40s with the adult acne flare ups still chin and jaw I even get it on my chest area a little my tummy a little but also on my back and it can be tingly itchy there. I am a Bit lost on controlling it i try retinol, retinol with anti biotoc. I use ahas bha's I can't pin point what helps sometimes its good sometimes its bad, but it affects you emotionally having it always :'( xoxo

hy doc, i just wanna share my story about dangerous skin care here in indonesia, the majority of indonesian women have the beauty standard as the whiter you get the prettiest u are. so many unresponsible people use hydroquinone above the safety standard, results in they got whiter in a very short period weeks-month, but i notice that their skin got brittle by mean it got thinner, spider nevi, redness, easily bleed out even scratch minimally and uneven/very contrast color between face and neck.

I was helping my mom treating her melasma. She was using Glycolic acid + HQ 3%+ licorice root 3/week and Tretinoid 0.05% + Azelaic acid 4/week plus humectants and sunscreen... and she was good... but she forgot to use her sunscreen for a week (IDK why!!!!) and now her face is worst than ever! How is it possible that she forgot the most important part of her skincare routine?

Hi good day., I just started using hydroquinon and Tretinion , I have PCOS so I have heavy dark marks on my cheeks . Now I DONT get pimples …. But as I started to use these products , by the 2nd week I broke out , pimples all over my cheeks, as I only use the products where need and not all over my face , so I immediately stopped the use to wait for the irritation to clear up before I start back , I think I was probably using too much , I don’t know …… but I wash my face at night put on my sun screen and moisturizer let that dry for about an hour or two then I apply the hydroquinion & Retina/ Tretinion, but I still broke out , what did I do wrong ??????? Plz help

Hi davin, I have been suffering from lichen planus pigmentosus on forehead and neck areas, from past 6months… my doctor suggested the following prescription .1. Proviso tablet, 2.picon cream 30mg 3. Powder coldes… he also tried q swtich laser for 1 sitting.. but it didn't work.. Can you please suggest me a good cream or tablets or treatment for my problem… I hope you will answer my question ..

If you have olive skin don’t use any type of lasers, your skin will show spots you never knew you had permanently.

6% Hydroquinone + Retinol + Hydro. Acid , Chemist mix by tube aprox. $65.00 by Prescription , only will help to lighten for a few months ,spots gradually will return unfortunately. Edit: You can use hydroquin 6% with compounds mix by chemist which follows Dr's prescription for 3 mos , then 3 mos off and so on. It helps to live & ease your stress about spots.... been there now enjoying. God bless 🤗

Hi Doctor thank you for sharing this. I have this greyish hue on my face overall because I did a home chemical peel and got sun exposure the next day. I have used several lightening agents as prescribed by various doctors including vitamin C, kojic acid, botanical extracts. I also did a few professional chemical peels and a laser treatment. However my skin does look greyish and very very dull, and it looks strange because I have a golden tone all over my body. My doctor prescribed Hydroquinone 2% once a day (at night), and it seems to be working but PAINFULLY SLOWLY. I have been using it for almost 40 days now. I don't have any irritation or any side effects. How do I amplify this process? I asked my doctor for 4% prescription but he is skeptical as it may cause irritation. However I am absolutely broken due to my skin condition it has been 6 months and i don't go anywhere out without 3 layers of foundation :((((((((. Any suggestions would be really helpful.

I’m really sorry you’ve been struggling with your skin issues. I learned that in addition to the ingredients you have listed to prevent more pigment along with sunscreen of course, to use Tretinoin at night with the hydroquinone. It increases the efficacy. If you don’t want a prescription then use the differin or adapalene gel you can get at Target. Hope that helps. It’s comparable to .025% tretinoin.
