For your question about if Holy Card stacks: Yes, it does. All of the mantle effects stack and have a certain priority when broken, the same way revive items do. There is a mod called "Mantle Rework" that shows you the order they will break in your health bar, as well as making Wooden Cross be affected by multipliers like being golden or hold Mom's Box, plus fixing the nonsense of Holy Card and Wooden Cross mantles breaking at the same time. There is also one for revives called "Detailed Respawn" that also shows you the revives in order next to your health bar, but you can also config it to only display the next revive, hide aditional text like the 9 lives amount or the collar's 50%, can personalize the icons and transparency, and has compatibility with other mods (if the mod has it config)

I think when it comes to whether you should reroll alt room items, I think you can apply the Monty hall problem to them. So if you don't like the first choice you're given, you can reroll it. If your second visible item doesn't have an item you like u can just choose the blind and you overall increase the probability of getting a better item. Plus as rarer items are less common, doing it early game cleans up the item pool a little bit making better items more common
