Unfortunately, most of the aviation community is aware that 80% of aircraft fatalities are related to human factors, as opposed to mechanical issues. The reason being is aviation tasks that are performed, require a complex approach to problem solving and decision making. Some of these tasks include, aircraft speed, weather, air traffic communication, and external pressure from passengers who are on time constraints.
