Always check the vehicle weight and the capacity of your lift before attempting to lift any vehicle! This video is intended to help you prevent lift accidents. The folks in this video did a good job recovering from the original accident and are extremely fortunate to have not incurred serious bodily injury.
This video was gleaned from several videos posted on KZfaq by Sarahi863. The original videos total about 30 minutes in length. Thank you Sarahi863 for allowing me to use excerpts of your video.

Note the yellow oval shaped sticker on the windshield, and the lack of license plates on the truck, that would suggest that the truck belongs to the dealership. None of the lift posts or arms appear bent bowed twisted or deflected in any way all. If you look the floor plate on that post is still square with the post, as are both arm carriers. It looks like the bolts holding the one post failed, pulled out or the concrete they were in failed. Improper installation looks to be the culprit here.

Thanks for this video, I'm about to install a lift in my garage (made by Ford Smith, a defunct company). I have 4000 psi concrete and I purchased anchors with the highest pull out weight (1" diameter drop-in anchors made by Simpson). I'm going to be under that thing often, I hope anything like that ever happens !

Actually that is a challenger brand lift with tripple teliscoping arms which are not available except on the heavier duty models. You can see it when they walk around to the driver side rear. You can see that the reason for the failure is that the mounting base plate bolts on that tilted post are ripping out of the floor and the lift itself is not actually bent. Even the most expensive lift will fail if the douce bag installing it uses the wrong bolts or the concrete is weak at that point. Open top lifts like that one depend greatly on the strength of the bolts in the ground .

This is why I bought a new 12' high BendPak 10,000 lb. asymmetric close-top model lift & poured a 2' wide x 2' deep cross in the middle of my garage floor with high strength cement. You don't want to compromise on safety when you're working underneath ANY vehicle. I built my garage specifically for this lift with 12' high walls.

said this a thousand times, car manufacturers need to install standardized "female" lift points on all cars and trucks. All lifting racks need to be standardized to meet the standardization of the cars and trucks with "male" lifting points. The "male" and "female" standardization between car truck and lifting racks would prevent this. Basically the lifting arms would lock into the lift points made into the cars and trucks. I'm a retired Ford Lincoln Mercury technician.

Sorry guy, your dead wrong. The anchors pulled up out of the concrete, the floor plate is clearly visible being tilted up, and there is no abnormal bending of the lift arms. I had this happen to me once. I raised the vehicle a few inches and noticed the right side lift was swaying back and forth. I immediately lowered the vehicle and checked the tightness of the anchors and found several of them to be loose. I retorqued the anchors, raised the vehicle a few inches again and bounced it very heavily, lowered the vehicle and found the anchors loose again. They had to cut a section of the floor out, drive rebar under the existing concrete and poor a new pad for the lift. We just got unlucky that the floor was thin at that spot. This could be the same issue, of the lift might not have been installed properly.

Another possibility, the thickness, or quality of the concrete pad that the posts were anchored into was insufficient for the load. No system is stronger that it's weakest link.

It also looks as if the truck was not centered in the hoist to allow the driver to exit the vehicle. This would have put excess leverage on the toppled post causing those floor bolts to fail.

The ford looked centered on the lift that failed. So therefore it more than likely wasn't GVWR issue, it was lift integrity issue since the passenger's side is still normal, they probably haven't had an inspection on that lift since they put it together. I told my old high school votech instructor to do so and luckily as he was lifting a crown vic the side that failed, failed from the beginning so he blatantly saw it before he went a foot in the air. These lifts need to have their PM's just vehicles and techs need to understand that as well as weight limitations.

A 7000 lb. lift (still too light for an F-350 dually) would go almost to the ceiling and would have a support beam connecting both posts at the top. This was probably a 5000 lb. lift or less. These guys are idiots.

Absolutely wrong to hook to bumper. The tow driver should have known better. Sorry I’m a young guy in the industry. But the older guy is using old technology to secure this truck. Now these days they have snatch blocks to use not the rail of the truck. I’ve had many cables snap on me even using better equipment then this guy. This wasn’t thought through at all and a lot of hillbilly techniques were used. I agree with you on all aspects of the safety you point out. One thing they teach now in the industry is crowd control. Too many people around and the only people who need to be there are the professionals. Very deadly situation here and more bodies around could end in disaster. Very good of pointing out all the improper safety issues in this video. Loved the commentary good vidoe

If that tow truck operator was experienced, he would have hooked to the frame mounted hitch and when he tried to move the pickup over, he would have used a snatch block and chain and run his cable through that instead f running his cable through the bedside rail.

Couldn't see the bottom of the failed lift post but did check out the website of the manufacturer who's logo seems to be on the failed post (logo is fuzzy in the video). Their website has a prominent "buyer beware" warning about counterfeit products being sold outside their dealer net with their logo affixed. Maybe the shop owner got hosed with a counterfeit?

This is one reason shops have insurance. But too many of these slip-ups can cause the insurance companies to drop coverage or jack up the rates unbelievably. I took my Dodge Ram to a tire shop someone recommended for a puncture repair. While waiting a diesel Chevy Suburban on the rack slipped off when the left-front tire/wheel was removed. Fugly damage to the right rear body work and left front suspension. The shop tried to calm the (rightfully) irate and upset owner. But I got involved when the shop's manager wouldn't tell him the name of their insurance company and insisted that "they'll contact you by tomorrow". No good, call the owner and get the information, now (after it was lowered to the ground). Shop manager insisted that they would fix the suspension damage. Again, not a prayer pal. You f'ed it up to start with, no second chances. Owner called local cops to get leverage for the insurance information and insisted he talk to an insurance agent before the truck was moved. Agent satisfied him they would fix at another shop. That shop was looking at a minimum of $10-$15K of repairs. Advice: If it happens to you TRY to stay calm. Wait until things are safe and anyone hurt is taken care of. Then be sure to get the names of the workers operating the lift and on the vehicle. Get the name of the manager/owner too. If possible, write down any statements they make with their names ("I didn't think the lift was firmly on the frame..."). Ask the guy in charge for the name of the insurance carrier, their phone number (this info should be in the shop and easily available) and their policy number. Ask the manager to call the insurance company as soon as possible (after things are safe, injured cared for, etc.) If anyone is injured or fuel spilled, call 911 even if its seems small. The fire dept will look at it as an industrial accident and do some investigation which will give you leverage with insurance companies.

I am a tow truck driverr out of nyc, i ran into abt 5 of them in my 15yrs in towing they are really not as bad as they look. yes i do agree with you about all them people shouldnt have been in the imediate area. especially that one guy in the white shirt right next to the bed as the truck was getting winched up. that whole scene belonged to the tow driver. and just by seeing were he wraped that winch cable. told me that he was not a very safe operator. he was supposed to use a snatch block. not the end of the gaurd rail, anyway just glad that nobody got hurt. if that was my shop. somebody would of gotten there butt chewed out....

the cross piece on a two post lift isn't structural. It's there to carry the equalizing cables and (sometimes) the release cable. If you have ceiling height restrictions, a lower rise lift, with the cables run in a floor trough can sometimes be the answer. This failure wasn't because the lift didn't have an upper cross bar, the lift was improperly loaded. Many heavy pickups are heavily biased towards the front. If you've got 5000 lbs on the front axle, and 3000 lbs on the rear axle, you need a lift with a min capacity of 10K lbs, and 12K would be preferable. Guys try to lift too much with a lightly built 8k or 9k chinese lift that's really maxed out at 6K lbs.

What makes you think the lift failed. Looks like the concrete anchors failed or perhaps the concrete was not up to par. Typically a lift wont lift beyond its capacity, the pressure relief pops off and the pump bypasses. You are a monday morning quarterback who doesnt have a clue. In time of crisis you use what you have and do what you have to do. They did just fine, they got the bad situation under control all by themselves without you or the national guard. Go back to your cubical, make some calls and shuffle some more paperwork and leave the doing to those who know . And dont forget the wifes drycleaning on your way home she will kill you.

Right i think it was a lift failure. But because the truck was lifted incorrectly. You can clearly see the truck is way too far back on the lift. Causing an imbalance. also these Fords have a different height in the front and rear frame. Causes the truck to tilt to the back. This truck is around 6800 lbs easily within the lifts 10,000 lb rating.
