Pros & Cons of Residency and Fellowship for Physical Therapy School

An entry level DPT program is typically 3 years. During this schooling, you learn all the subjects of physical therapy:
Peds physical therapy
Neuro physical therapy
Ortho physical therapy
Acute care physical therapy
Geriatric physical therapy,
Vestibular physical therapy
Sports physical therapy
Musculoskeletal physical therapy
+ more
However, once you finish your entry level program, a student can decide if they want to continue their post-doctorate program and pursue further studies in one of these fields.
For example: I am thinking of pursuing an ortho residency after my 3 years of DPT school.
Residency is generally 1 year.
Fellowship is generally 1 year.
For a total of 5 years if you choose to go all the way.

PROS:
Mentorship
Full-Time Job (getting paid)
Full-Time Benefits (Medical, PTO, etc)
Prep for the Specialist Exam (OCS, NCS, PCS, etc)
CONS:
Pay Cut (60-80% of full-time salary)
1-2 years delay in working as a full-time salary PT
Have to participate in research

Residencies are a waste of your valued time and experience from PT school. Don’t take a pay cut for some letters after your name. Have confidence and get out there to find the specialty you want instead of paying others to do it for you.

Hey Justin! I love you videos man! Really appreciate all the information and guidance. You talked alot about what a residency is and all the benefits, which has made me strongly consider pursuing one. Can you make a video or talk a little more about why you would further specialize in a fellowship after already completing your residency, getting that OCS, and hopefully being offered a job?

Whats up Jacob. Thanks for sharing your input and it make me really happy knowing that you were inspired to even consider doing a residency. I could definitely get that video out - and i think I know just the way to do it. We have a fellowship instructor working at our DPT program and I think it would be awesome to have her talk about that subject. Now the trick is.... I got to convince her to do an interview with me... LOL But if she doesnt, I can for sure get that video out. Thanks for sharing and supporting, as always! @@jacobkissman3693 Yessss I hope so too! I think she would be the type who would be down, but you never know.. professors and youtube - for some reason they think its the devil haha NICE BRO! Both paths are great paths, but if your the type thats not really into medicine and more of a "conservative route" approach, then PT is the best way to get there. Im very very very glad my videos have been helpful for you - literally makes my entire week when I get comments like this to hear feedback on the impact its making and the value its giving. When doing a residency, are you paying to be in the program while getting paid for the work you do? Or are you just getting paid as you work?

ishie0196 that’s a good question! You pay for the application/initial fee, but you’re also getting paid as you work too. Different programs do it differently but usually it’s a paid internship. Most of them are full-time so you’re also getting benefits too

Hey! Thanks for this video. Can you talk more about fellowship as well? Do we also work with full benefits and get some alphabets to decorate our name after it's done? Is the pay better than residency? If you're an expert after residency, then does fellowship make you experter? lol. Sorry about lots of questions. I'm starting PT school in the fall and wanted to do some research about what to do after school.

Daniel Koo hey those are excellent questions my man! First off I wanted to say congratulations about getting into PT school. I know you worked super hard to get to this point. Second, a supplementary video on residency and fellowship was actually something I wanted to do! So I will definitely be addressing these questions. But for now I’ll try my best to answer everything I can! A fellowship is something that you get into after doing a residency. A fellowship helps you focus even more into a specific niche that you want to get into. For example there are spine fellowships and movement science, sports, manual therapy fellowships in Southern California. I heard sometimes you don’t get an increase in your salary because you’ve gone through a fellowship, but I’ve also heard that other places honor it and will give you a salary boost so I guess it depends where you apply. And most fellowship programs give your those letters too haha

Hy...I am undergrad PT student being in final year...I want to do internship in medical emergency once in life...can u plz tell me the imp of ER experience for a therapist?

Dr Physio the point of a PT seeing a patient in the ER or critical care unit is it quickly asses the safety of the patient and be able to recommend difference assistive devices, continual care, and services that the patient may need to be able to restore prior level of functions. The ER is a special place because everything is very acute. But it’s a good experience to be able to see injuries that JUST happened. And it’s also interesting to not only see ortho related cases, but also Neuro too, or a combo of both. Like motor vehicle accidents, strokes, spinal cord injuries, etc. I would recommend it!!!!

Hi Justin! I will be starting my DPT program this fall and I'm am already thinking about residency (i know, it's a little early for me to be thinking about it haha). I just have a few questions: are residency programs competitive to get in to? and are they similar to DPT programs in that they accept applicants by small cohorts?

Andrew Ruiz hey man I’m excited for you!!!! It’s never too early to think about it. I started thinking about it just a couple months after I started lol I kept going back and forth on if I should do it or not, which is why I made this video haha Residencies are pretty competitive... usually hundreds or thousands apply, and only a few get accepted. Of course it depends on where you apply, but most places usually only accept like 1-4 students a year 😱 and so that being, I’m not too sure how “cohorty” it can be with such a small size you know? At least in my opinion I think of a cohort with at least 7 students or so.. Which residency program were you thinking?

So far I am interested in specializing in either ortho or sports, but I am honestly going into school with a very open mind and I'm willing to look into other specializations too. Thanks for letting me know about how competitive residencies are, I'm glad to know about this early in my career so I can plan accordingly. Do you know if PT school GPA matters when applying to these residencies?

Andrew Ruiz yeah bro! Anytime I could help is a great time. I actually do not know if they matter.. I heard it doesn’t. But I know that most programs will have you actually do a full evaluation on a real first time patient, and your CI will be in the room grading you... DAMNNNN. That’s nerve wrecking haha Yeah bro! Keep an open mind. A lot of students thought they would have pursued ortho, but then they changed to other subjects

Great question! I dont work at a PT yet, but from my experience from rotations and from my knowledge, this is what i know: education is huge! and having the knowledge to provide to your patient is so powerful. I remember i underestimated this concept, because i just wanted to "treat my patients" but it turned out that i learned that the power of patient education can have just as much of a healing effect as treatment - my clinical pearl Bottom line, experience in the field is MUST. Having a lot of knowledge is great, but if it's not applied, then whats the point of it? the ideal world is high education and high experience, which makes an EXPERT. That being said, residency and fellowship is the FAST TRACK to being an expert. The 2 years it takes to go through post-doc schooling may take up to 10 years of experience if you did not go that route. Not saying its the wrong way, but its a very time-efficient way to be the best!! Plus, along your res/fellow you will gain personal mentorship, and attend classes to further your expertise and education

Dr Physio hey! That’s a great question!! There are a lot of options out there and it really depends on where you are looking into. Here is a website of options available: www.abptrfe.org/apta/abptrfe/Directory.aspx?navID=10737432672 But typically you have options of: acute, neuro, ortho, Peds, geriatrics, sports, and more. You can get into these right after graduating and getting licensed after DPT school! Usually in Residency you are actually “working” as a PT and on the side, you have several hours of mentorship to go over skills. They also provide education courses throughout your residency program. So all in all, you got paid while going to courses and you get clinical experience as a PT

I actually got waitlisted then rejected from residency. But from what I heard instead of making like $30-35 an hour you make about $20-25 an hour

@@drjustinleedpt That's pretty decent, what factors do you think lead to the rejection? Is there a percentage that pass and thos who don't? Just started my college career plan as a PTA, and willing to push for PT later on

@@link2442 I would say a lot of things play into the roll of rejection but I personally think that personality plays the biggest roll. Residencies want someone who can display leadership but also humility, an eagerness to learn. If you come off as arrogant and know it all, then you will for sure be rejected.

Do PTs that go through residency and fellowship always have a greater salary than PTs who didnt or does it mostly depend on how you market yourself?

Outta Breth you know, they should right?! But sadly, some places don’t really give a big enough raise for having that. But.. you can always market yourself as a specialist and also bargain for higher salary

Hey Justin! So I understand being in a residency or fellowship program you are working during that time with a pay cut, but I am curious of after you finish that program and pass the test and are now a certified specialist will that lead to you making more money when you start working??

Hey zack! Awesome question dude! Yes typically you will be getting paid more as a full time certified specialist PT. of course it depends on the location of your residency and full time job, but generally you can assume that. It may be different if you end up working in a private setting. Same goes with when you get your fellowship. Which residency field are you applying to?

Zoe Noel definitely! you can negóciate this to have you start out at a higher salary. Unfortunately however, the raise isn’t as high as you would think

Eric Pham hey Eric! It is pretty damn competitive to get into a residency program. Most programs will only have 1-3 slots available PER YEAR. and I’m sure at least hundreds apply, depending on the popularity of the program. Usually they will have the pool of applicant, then narrow it down to several interviews and mock evaluations, then they choose. So... yes, pretty competitive!!! But... residency/fellow for sure allows more of the “networking” component, compared to just academics and grades. So..... if that interests you, you better get your talking face on! Haha
