Great share. Keep sharing and never mind the peanut gallery. It’s just Pavlov’s DS Conditioning to nitpick and be petty. I love how you adapt, the true sign of greatness I look for in teammates. You were nice to the assistant, I would have benched her and call in the A team, but everyone starts somewhere.

Could you share the pre-surgical planning as well? CBCT and positioning the implant.

Thank you for sharing. What is the reason for using non hexed and non engaging abutment? Do you not recommend doing socket shield technique?

implant was seated thru the guide and touched the cylinder, not concerned about contamination?

He probably lets the guide sit in chlorhex prior to use. That's what I do in order to offset this worry.

We soak the guide in sodium hypochlorite to sterilize them prior to surgery. I do not touch the implant with gloved finger or other instruments in an attempt to be as clean as possible.

Fantastic work 👏👏 sir I have two questions first why you didn't perform shield technique in order to prevent future soft tissues drap or loss? Second question is why you did not see lateral excursion and protrusion occlusal contacts in order to prevent offset forces on implant

I’m not a proponent of the socket shield technique. A quick review of the literature will reveal the common solution to a failing socket shield is to remove the root remnant, gap graft with particulate bone and possible CT grafting. So I recommend skipping the potential failure mode and going straight to gap grafting. If the implant is properly planned and placed there will be no esthetic of functional concern with the implant. Proper thorough occlusal analysis is paramount for success. This patients overjet was so large that I was lazy and didn’t bother checking all functional movements. I do not recommend doing that and thank you for pointing it out so folks are not mislead.

Just a little festooning to the emergence profile and the concomitant use of the PREDICT method after integration and the final solution should be indistinguishable from natural teeth.
