Low Ketone Levels Don’t Matter- Here’s Why!- Thomas DeLauer
Questions that will be answered within this video:
- What is the glucose ketone index (GKI)?
- How does this differ from just measuring your ketone levels?
- Why does your GKI provide a more complete picture of your metabolic state than measuring ketone levels alone?
- How do you measure and calculate your GKI?

This was certainly helpful, I had been frustrated with trying to get higher Ketone level (not to high as it would be dangerous for me personally) there was a lot of insight and certainly given me things to think about. My only constructive criticism is that it would be nice to have mmol/L readings along side with Mg/dl as not all of us are acustomed to Mg/dl especially among the diabetic community. This would be really beneficial and to cover all basis for all who watch your insightful videos. Thanks for your efforts and keeping things interesting.

I’ve been on a ketogenic diet for over three years, never consuming more than 20g of carbohydrate and consuming large amounts of fat. I do HIT strength training and Sprint intervals for ‘cardio’. I’m 6’ and weigh 175. Physiologic insulin resistance has made it so that my blood sugar NEVER goes much below 90, and my ketones are rarely above 1. For that reason, I couldn’t care less about the GKI, because it will never be good for me. Yet I’m extremely healthy (fasting trigs under 40, HDL 58+). Don’t care about GKI, unless I end up with cancer.

Just bite the bullet and weigh yourself. I lost 90 pounds on it and then gained back 30. I had thought I'd gained it all back but I still had 60 pounds and the 30 I gained back almost all of it came off within a month and a half and back at 90 pounds. For me everyday I didn't weigh myself it just gave me one more excuse to not get back on the diet. Also, keto is very protective of your health and especially things like Covid. So even going back on it for 30 days will send your metabolic markers back to a great place and that's what you want.

Don't worry about it unless it's consistently under .5 or so. Your body could be doing any number of things (including gluconeogenesis because your beta cells aren't quite recovered yet, or you could be producing exactly the number of ketones that you're consuming, etc.) Most importantly, don't rob yourself of protein. Too many people undercut the protein thinking they're helping, and they're not. It sounds like your body is telling you that you need more. Have you tracked your protein and compared it to a protein calculator online?
@sarasayshi
@sarasayshi 3 жыл бұрын
@@CarbageMan Yes. I was eating the amount recommended for my body weight. And then I tested out a protocol that had me lowering my protein and increasing fat and carbs just slightly and that is when I started registering higher ketones. But I really just thrive better with more protein and lower carbs.
@CarbageMan
@CarbageMan 3 жыл бұрын
@@sarasayshi Sometimes, the ketones will just come from the fat you eat, not the fat you burn. If you have been eating high fat for a long time (maybe three to six months, it all varies) then your body will be more comfortable burning its own fat when you reduce the dietary intake of fat. The protein numbers are estimates. The older we get and the more exercise we do, the more stress we're under, the more protein we need. As long as you're producing ketones, and you will be if you're not eating carbs (I have to severely limit my carbs) you'll do fine. It's mostly about the actual results than about the numbers. The numbers can be used to diagnose problems you sense. Quite bluntly, most of us really only lose weight while fasting, but we feel much better cutting the carbs even if we're not losing weight. Gaining would be different, and then you'd want to know if it's muscle, which would probably be good.

Dr Boz(Annette Bosworth, MD) has come up with a ratio, Glucose divided by Ketones, to come up with a ratio which she coins the Dr Boz Ratio. Of course you still need to track blood ketones and blood glucose levels to come up with the proper ratio.

Can you speak to how physiological insulin resistance and keto-adaptation might play into GKI readings? For instance when I'm in ketosis for several months my ketosis numbers begin to fall even while doing a water only fast, and at the same time my blood glucose levels are sometimes in the 90's(still fasted). My understanding is that this could be due to physiological insulin resistance (as opposed to pathological/metabolic syndrome). I'd like your input on lower ketone numbers as a result of keto-adaptation, physiological insulin resistance and how it relates to the GKI measurement.

I was chasing high keto numbers with the Keto Mojo last year from February until March 19th. A few days before the 19th I decided to try ingest more fat to bring my numbers up. I was already on just about 10-15 grams of carbs a day mostly from broccoli 🥦. My keto #’s were from .08 to 2.5 with an average of 1.8. I think I overdid the fat with butter in my coffee a few days in a row. Turned out my gallbladder decided it didn’t like what I was doing. I had severe pain went to the ER and they removed it 2 days later. The dr said it likely wasn’t keto diet or the extra fat. I stopped keto until a few days ago and I’m happy to be back on it. Looking forward to losing some pounds and feeling better.

I’ve been somewhat frustrated with the keto diet because of my measurements. I keep total carbs under 50g a day and often probably take in maybe 30-35g of total carbs (not net carbs). I recently moved to 2 meals and 1700 calories total intake with home cooked meals (most carbs taken from vegetables like cauliflower, carrots, lettuce, broccoli) and am 3 weeks into my keto journey. I wanted that intermittent fasting experience to drive better results and it did nothing (18 hours between meals from last meal of day to first meal of next day). Best keto reading I’ve gotten from blood was 0.4 mmol and I recently had 3 urine tests at 1.5-4 mmol while my blood ketones stayed “below nutritional ketosis”. I’ve also done workouts, walking 5 days per week. I don’t drink any alcohol and don’t cheat and grab bread, cupcakes, etc. I’ve lost no weight on keto. Wonder if I just need to throw in the towel. Lost more weight last month on a total calorie tracked diet 😑

measure or feel your way? I'm 54. Got interested in low carb diet because my recent blood work indicated fatty liver disease. I rarely ever drink and if I do it's one or two lite beers. I'm clocking four weeks in. I lift weights and walk every day. I vary my work outs between heavy and light, with longer emphasis on recovery as I've gotten older and recover slower. The consistent work outs let you keep in touch with your body on a higher sensitivity. I am a quick learner, continuous learner. And scout the comments section for experience that I can relate to and learn from. I'm dedicated to this change, having been previously indoctrinated by the false narrative of high carb low fat. Because I have experienced the exact opposite. My first positive was my sinus clearing. Within a couple of days I could breathe easier. That had significant improvement on my sleep. That was the second positive. And my body was leveling from radical high and low swings. The third positive has been more consistent better work outs feelings. I have to hold back more because I feel like going heavy more often. I've done OMAD once. But it's more natural for me to go two and 18:6. Just adopted easy and naturally so far. And I'm very active and have some difficulty getting enough calories. I've been dropping weight like crazy even though it's not my real intention. Down ten pounds but seems to be leveling out.

My ketones are always between .5 - .7. It’s rare for me to see anything higher. My blood glucose is also always low. Seems like whenever I test, the GKI says I’m not in ketosis. I’ve been keto for years. I lost 165 pounds but the last 40 is so freakin hard to get rid of. Im losing like 2 - 4pounds a month. It’s just a lifestyle now so I know that eventually I’ll get there. But seeing that GKI say I’m not in ketosis is frustrating. Footnote: I eat about 1200-1400 calories a day (about 20-23 net carbs). I have an approximately 4hr morning eating window - typically 8am to 12noon. I literally never break it and I’ve been on this IF schedule for about 3 years. Maybe I should do a fat fast for a few days.

@jewelsbarbie 2 жыл бұрын
1200 calories a day is a starvation diet (whether you’re eating keto or not). Your body has slowed down your metabolism to accommodate for such low energy input. I suggest upping your calories, so your body feels safe to let go of the last 40lbs you’re trying to lose.

When it comes to weight loss, low levels don’t matter because it doesn’t matter if you’re in ketosis or not. Being in ketosis doesn’t speed up weight loss. I was in true ketosis for 4 months to give it a fair shot and not a fan. Too much fasting, not enough food. Workouts were the worst I’ve ever had. I switched diets literally in the middle of my cut and didn’t make one bit of difference. Kept my cut going and ended it way more happy than when I started.

I think keto is good only when one is morbidly obese. It should shed the easy fat off in twice the speed. But once you get back down to the visceral fats and your weight is closer to 15 percent body fat, it stops there. From there I think it's the training that will get you in shape. And real training needs carbs.

I've been on 20g carbs for 3 months now. The first month I was .7 to .9 Now at 3 months it is .3 all the time, even after fasting for 24 hours. The weird thing is even if I have night out with friends and I can't get a keto meal it drops to zero for about 4 hours then straight back to .3 I can shift it to .4 sometimes by eating 100g of macadamia nuts or coconut oil but it goes straight back to .3 a few hours later. I went 3 days into a fast without showing ketosis on the pee strips this week. I was coming off a "cheat day" that was essentially garbage and carb laden, but i thought it was weird to literally not eat for 3 days and not have depleted the stored sugar and to not have been burning fat..particularly considering rather exhausting 2 hour workouts each day. Im fairly confident that my body obeys the laws of thermodynamics and that something was consumed nevertheless, and have chalked it down to probably overindulging on a cheat day plus pee strips being bad at displaying the big picture.

That can happen. You have to reduce your glycogen stores. If I need a kickstart, I fast and do low-level cardio. If you overdo cardio, you can cause gluconeogenesis, and most of the ketones can be converted to sugars. So you burn fat, but you don't see it.

Is that keto in mmol/l and blood glucose in mg/dl,? So you have to ÷ the mg/dl by 18 to convert to mmol/ then ÷ by keto? To get ratio. Is that right? What is a good ratio range ? I found this to be not very detailed in explanation.

I am in the same situation. It has not been long enough to be a stall (3 wks) for me, so I am going to be a bit more patient before making any drastic changes. Fasting is always a great addition to foster weight loss and definitely has helped me. I began with 16/8 but some people do 12/12 for a few days (just to get used to it since the real impact doesn't begin until your atleast fasting 16 hrs) and then move on to increasing it depending on their results and how it feels to them. Good luck!

I've been using KETO Mojo since Thomas introduced it around 9 months ago, somehow my blood Ketone levels are always much lower than my wife's (we usually measure just before we sleep) but I really have no or very little body fat on my body, no matter how much or how little food i eat, and i usually eat tons. My wife, even though she does much longer fast than I do, has way more body fat than I do, so I don't think the blood Ketone level really matters that much personally.

Gluconeogenesis is a demand, not supply, driven process. Fear of protein has been debunked.
@CarbageMan
@CarbageMan 3 жыл бұрын
@@pmfith Generally, this is true. If your beta cells are still damaged, you may find, however, that your body "demands" a high level of gluconeogenesis. There are clearly confounders here and it's still being researched. In general, people new to ketosis will do better with moderate protein, but a lot will "graduate" to carnivore. Personal experiences vary greatly.

What timing, I just upgraded to the new meter and I need help understanding my Blood Glucose Because I don't see much discussion of peripheral insulin resistance and how it will effect GKI. I am fasted for 34 hours (after a heavy junk food carb heavy cheat day) and my current blood sugar is 105 and ketones is 0.4 GKI is 14.8. I'm more concerned about the 105. Even after over 10 days of moderate keto, after 20 hours of fasting I'm still reading at over 100mg blood sugar. How can I use GKI effectively if even with keto and fasting I'm running above 100mg. I would need ketones of 5.5mmol to have a GKI of 1, Even after 6 days of water fasting the highest I reached was 2.7. I assume if I did that though my blood sugar would drop but I'm just curious thoughts on how to deal with peripheral insulin resistance when using keto mojo.

Insulin resistance takes A LONG time to fix.
@Sheikdaddy
@Sheikdaddy 2 жыл бұрын
@@laurasc84366 I was talking about adaptive glucose sparing. Do keto or fasting long enough and the body will downregulate insulin receptors to preserve the glucose the body has and can make for the cells that need it. Then, have a bunch of carbs, glucose or whatever, and your blood sugar will spike huge like someone with insulin resistance. This is temporary. If you eat carbs for a few days the levels will return to normal as the body upregulates insulin.
