Upon first seeing The Doctor. I had the impression he was attached to some sort of railway system along the ceiling. But he's literally just crawling on it. It really is an interesting design choice that makes this antagonist a lot more intriguing.

I think she didn’t want to eat him, at least not until the true ending of the game. When she leaves the tv, it shows a static figure like six, and if you listen closely you can hear six’s stomach growl. I think it signals that although she is reverted back to her normal figure by the end, it shows that her soul is long gone and is replaced by hunger. I think this is due to the fact she has the power of absorbing power from others like mono, which is why she was able to escape, but it was not used until after she reverted from being a monster.

5:25 the first time that I saw this scene I was really uncomfortable because I noticed that the patient only began to panic right after the Doctor entered the room. So I originally interpreted it as if he was panicking because he thought that the doctor was going to perform another surgery on him of which I doubt that there would have been any anesthesia or anything like that. So that's how I interpreted it at first XD. But I know now why the patient was most likely freaking out because of life support being turned off. But like I thought for a moment there that mono had basically alerted the doctor to go and operate on the poor patient.

That's because he's still human. Sure, he's like 500+ pounds and can crawl up on the walls and ceiling like Spider-Man, but he's still a human, and thus, can be killed. Same thing goes for everyone in both Little Nightmares 1 & 2. Sure, some have overtly long arms, some are old women that can somehow breathe underwater, as well as swim at high speeds despite said old age, some are taxidermist gone mad with itch trigger fingers, and some, as mentioned before, are 500+ pounds and can slink around the ceiling and walls like a spider. But, they can easily be amputated, electrocuted, shot, and burned.

Something that bugs me about this boss is if he is really evil, the guy kinda treats his mannequins patients in a odd way but mostly because he doesn't know how to properly deal with them, wail in the other hand with the one real patience he has, he rushes over to him to help him as soon you shot down the machine that us keeping him alive.

Maybe the game is trying to make some kind of commentary about cosmetic surgery and body dysmorphia idk

Maybe not, as theres an achievement titled "First do no harm," which implies that he is trapped inside the incinerator, and most likely will starve before dying. (I know achievements don't usually contribute with the story and what not, but I found it interesting.) If that's the case then I'm kinda sad because the doctor was one of my favorite antagonists :(

Remember how the first game had that big pile of victims' shoes, and some players saw that as Holocaust imagery? That makes it a lot more disturbing to see Six & Mono kill a guy in a furnace. I'm sure that's unintentional and they were probably going for a Hansel & Gretel vibe here... but it does make for a very disturbing interpretation.

This dude i feel bad for. Not sure what his deal is with snatching mono and six, but you can hear him panicing like it's all up to him to keep his "people" alive. And he just does not know the resources he needs. He's just panicing trying to keep his breath steady. And his crying really shows he is trying to push through trauma.

Least favourite part of the game. The flashlight is so flimsy when controlled and doesn't stay stationary while you move, meaning you have to constantly rotate the right stick in the direction of the left stick and control both, which you can't do if prosthetic dolls are attacking you from every direction, it's also very gimmicky with the hands. This chapter dragged on for too long and just became outright irritating
