I think that pen touch should also be used to check his pupillary light reflex. Also, I guess tone and DTR should have been checked as well.

if he opened his eyes to pain, then it is a 2 not 3. so GCS is 2+2+4=8

If intracranial pressure is high which kight be suspected by CT then this is contraindicacy for LP .
