Whenever I watch Japanese baths, they have a shower area that they use before going into the water. I'm not sure that's really a thing in America though, so I would assume this is a combination bath/shower, and it seems like Suisei just filled the tub up instead of cleaning herself first, so is the shower first not that big a thing?

Many hotels, everywhere, have a separate shower and bath tub area (the higher class hotels, usually). The showering first before entering the bath thing is culture brought over from common courtesy in public baths (which in the past were the only baths available), but in private I don't think it matters.

You don't need to shower if you only filling up the bath tub, but if you want to soak in the tub, you have to clean yourself properly ( brushing with soap, wash your hair with shampoo ) and the shower usually for washing off the soap or shampoo, then you soak in the tub.
