Desvenlafaxine is the active metabolite O-desmethylvenlafaxine (ODV) of venlafaxine and is formed as a result of CYP450 2D6. It shares many of the same properties as venlafaxine. This video covers the important points that separate Venlafaxine from Desvenlafaxine.

I ended up in emergency detox from this med. I had never been actively suicidal, this poison put me there. It might work for some, but it almost killed me. I understand that for every body there is a different chemistry. I think that patients need to be very aware of the possibilities and be very careful. It was nearly impossible to get off of for me.

I'm sorry to hear that, although any mental health physician worth their weight would inform their patient of the side effects, and they likely do but because of the nature of those consultations the patient may be willing to say yes to anything

I have anxiety, panic attacks and a little depression due to anxiety/panic attacks, my Dr. RX me pristiq due to my genetic testing results, my question is why would she RX me that and not try Effexor (which is also on my use as directed list)?

For me was my salvation, I'm on pristiq for a month already I was taking 100 mg but now I'm taking 50 mg... I didn't have almost any side effects.

@@yardartistry4121 now I back at 100mg I was having more side effects with 50mg than with 100mg ... And had more anxiety with 50mg, but pristiq change my life. At least my mental state.

pristiq has helped me somewhat but it made me have high blood pressure. it was the only thing on my genetic testing i can try it should work. everything else didnt work for depression and anxiety. so i live with my issues. and make the best of it. exercize is helpful and eating healthy. having good sleep helps. it is what it is.

Can you do a video on hydroxyzine hcl? I'm taking 50-100 mg a day. It helps me sleep when I'm manic but I'm staying on it when I'm well too. I'm concerned about building a tolerance and it no longer working. I don't want to try any more drugs. It also appears to have gotten rid of my panic disorder and agoraphobia, I'd been not leaving home for years and now I'm out just about every day, still in just the nearest town and not farther but it's a HUGE improvement for me.

yes, I would be happy to cover this medication. Tolerance should not be an issue, the main thing I caution people on is QTc prolongation at doses over 100 mg/day.

I can see the rational for the combination it will theoretically increase all three neurotransmitters serotonin, norepinephrine, and serotonin. I would usually make sure an adequate dose is reached with one medication for a long enough duration before adding an additional medication. If this has already been done and you are augmenting it's a reasonable combination. There is a high risk for withdrawal syndrome with desvenlafaxine. It also has more norepinephrine activity than venlafaxine, I have a video detailing that information if you are interested. There are no drug interactions with this combo. Thanks for another informative video. How do the norepinephrine characteristics of Pristiq compare with Wellbutrin? Is one considered stronger? Is the approach to increasing norepinephrine different with each of these drugs?

This information mostly comes from animal studies (rats I believe) and clinically I'm not sure how much it translates to effectiveness in the real work setting but Pristiq has much greater affinity for the NET transporter than bupropion. Hope this helps

Pristiq is better to treat anxiety than Wellbutrin in my humble opinion. On Wellbutrin I worried a lot more than on Pristiq 50 mg. To get off pristiq, Prozac can be extremely helpful from what I’ve heard Hope this helps

I’m on Effexor XR 75 mg. I’ve been on it years. I still have days when the depression is very bad and I can’t function. My APRN raised the Effexor XR to 88 mg. I became very hyprometic. So she dropped me back down to 75 mg. I am frustrated because she doesn’t seem to want to change anything. I take Lamictal I was on 100 mg twice a day now she raised it to 200 mg twice a day. I don’t feel any better. I don’t know what to do

Try pristiq 50 mg. It is more affective than Effexor xr. It’s not metabolized In the liver either. They are just about the same drug

Your presentations are excellent . I'm currently taking Desvenlafaxine 100 . After four weeks I feel my depression is much less " heavy " and more manageable . However , sometimes my anxiety is higher , which has led me to drink a few glasses of wine at night , even though I know that this may be exactly what I should not do . I've tried 9 different antidepressants but none have worked - Amitripiline came close but my family doctor told me to stop due to my age , 67 . My impression is that these medications can have the effect of increasing desire for alcohol . Is there anything to that ?

This is not medical advice, it’s for educational purposes only. Always talk to your doctor prior to making any changes. I’m not aware of any data indicating increased risk of alcohol consumption while taking these medications. Anxiety is an antidepressant impediment. People with comorbid anxiety will have more resistant depression and will be less likely to respond to medication. If a person has had 5 or more trials of medication without remission, medication is unlikely fully control symptoms. This does not mean people should lose hope, but it does mean the expectations of what medication can do has to change. The expectations should shift from achieving remission to, how can I tolerate these symptoms while still living a meaningful life. Being a good psychophramacologist means knowing the limits of medications. With each successive medication failure, the likelihood of the next medication working decreases. If there is true treatment resistance then ECT, pramipexole, and ketamine remain options, with ECT having the best efficacy for depression. Have you heard of good results of pristiq for ADD? do you think sexual side effects might be lower than with other SNRI?

sexual side effects are still an issue with this medication. Some people have used pristiq off label to treat ADHD, but I tend to stick will medications specifically approved for ADHD. In short yes this medication may help with ADHD and you need to balance the risks and benefits with your doctor

I take ADHD medication and I've been on this a month and I finally feel a bit of the old me has returned and I haven't cried in the morning upon waking like I have for the past year at least! I wish you well 👍🇦🇺

Hi, my doctor prescribed 25mg Pristiq to add on to my 45mg of Mirtazepine for anxiety. Just wanna ask is the loss of appetite/weight temporary? I don't wish to lose the healthy weight I've gained after recovering from anorexia

I would have your doctor monitor your weight. Some people do experience decreased appetite and in some cases it is transient. The hope would be mirtazapine would balance this out but I think talking with your doctor and careful monitoring are essential given your diagnosis. All the best

Mirtazapine usually makes patient gain weight, and the pristiq doesn’t have as strong weight effect, so hopefully it was ok

Depends on the medication and the patient preference. If we are talking about sedating medications like dopamine blockers then dosing it all at night is a better option. I also dose lithium all at night. With SSRIs and SNRIs they can be activating and best taken earlier in the day. Hope this helps. Question.. if I take Clomipramine 25mg at night, is it safe to start Pristiq in the morning or should I be discontinuing Clomipramine completely first? I’m getting mixed messages from everyone.

I can't provide direct medical advice so I can only talk about the medications in general. There is always a risk for serotonin syndrome and other complications when combining medications with a similar mechanism of action. I do not see any benefit in combining these two medications together. I prefer to use mono therapy at an adequate dose and for a long enough duration to properly assess effectiveness. I take 50 mg of Pristiq for bipolar disorder type two. It’s great three days after I started taking it I stopped waking up crying and wish I was dead I wake up now I have a better mood all together. I still have anxiety though.

The pharmaceutical and medical device industry is constantly making payouts for all types of medications and devices. We talk about the side effects of the medication and present the information. The main concern with the SNRIs is worse withdrawal symptoms, this can be avoided if a proper tapering schedule is monitored by the doctor who prescribed the medication. The people who I see have problems are those who abruptly stop without a plan.

I know Effexor turns into pristiq, but pristiq works sooooo much better. I know that doesn’t make sense. They also say there’s no evidence that anything above 50 mg does anything but 100 mgs works sooo much better than 50 for me.

It does make sense actually. interestingly, pristiq has a stronger affinity for norepinephrine at the starting doses, so it definitely doesn’t have the same effects as Effexor. There are differences even if minor. That strong norepinephrine effect might be why it works better.

There seems to be a sweet spot with many medications where the benefits are maximized and side effects are minimized, I would say 50 mg in this case is accurate. Some people do have benefit over 50 mg but it may not be as robust and comes with more risk of side effects. maybe we will do a video on the sweet spot for each medication

I have been on 50 mg of pristiq for about a year and have started to attempt tapering off (with the approval of my GP).. Currently the taper plan I am attempting is the first week, on one day 25 mg and the other six days at 50 mg. Then the 2nd week, two days at 25 mg and the other five at 50 mg. Hence I would continue this pattern for seven weeks. My question is, am I tapering too quickly? I do not want to attempt any crossover AD's if I don't have to, but what would you recommend? That schedule you described sounds reasonable. I expect you will do well. Despite what people say, I have patients who abruptly stop medication and do not experience a withdrawal syndrome, sometimes other comments make it sound like you can never safely stop the medication which is simply not true. It's always a good idea to do an appropraite taper either way

Side affect??? I cry at the drop of a hat! My girlfriend who took it was a cryer too. I’m going off because of that. I can’t deal w/ the hypersensitivity anymore. I took it for 2 years. Do you see that as side effect? I don’t see it on any info about it.

Desvenelafaxine......as a fellow mental health professional I have always been told by my professors and other collegues to pronounce Venlafaxine as VenLA-Faxine and you are pronouncing it venalfaxine???...I knwo it is a small thing but you are pronouncing it totally different than every other mental health professional I know?

@@ShrinksInSneakers please my first time watching your video. I just got this medication today the 1 June 2022. I don't know if I should take it. I'm afraid it damage my body. I went to the doctor because I'm always feeling depressed because of what I'm going through. St times I feel suicidal 🙃 but not all the time. But I didn't like how my doctor was talking to me making it look I'm crazy. People go through things differently at times. I'm frm jamaica 🇯🇲. Do you think I should take them? If yes , can I still drink my natural fruits juice that contained leaf greens, pineapple 🍍 ginger, beetroot, carrot 🥕 lettuce ,string beans etc etc. Please I hope you reply.

The increased side effect profile is part of the tradeoff for a slightly more effective medication. It can be intolerable for some patients and should be addressed on a case-by-case basis. Many side effects are transient, and others can be effectively treated. My hope is that you found a treatment that works for you. @@ShrinksInSneakers well I started prestiq to finally try it and I have gotten better with the depression and I have tons of energy but I shake like tremors and nervousness. Will those side effects go away in time of usage?

@@ShrinksInSneakers thank you for responding 🙏 😊. My psychiatrist told me to cut it in half but I'm scared to cut it cause it's time released and she was aware of that. She said well it couldn't hurt you. But I am no longer lying on the couch all day with crippling depression 😪. So, I am going to keep taking them I've been on 50 for 3 weeks and 25 for about 4 or 5 day's now. I was always thinking I don't have a mental illness but it is in my family. My mom is possibly bipolar among much more. But, she's too stubborn to go to a shrink and too stubborn to take meds. She doesn't think she has a mental illness but she said grandma did her mom. Well she has it to and my other family has mental health issues. But, thank goodness my son got placed on Prozac and low dose it's helping him. But, he may need 10 more mgs. We're sensitive to alot of meds it seems
