I wanted to offer a quick update, I’ve been reading on hydroxyzine, and it appears that the medication is not anticholinergic. Dr. Rossi, thanks for taking my request. I am doing well on hydroxyzine at this time. I watched this while waiting for my dose to kick in. Interestingly, sunflower lecithin in somewhat large doses (scrupulously avoiding any additives such as inositol in the lecithin) helps me avoid cognitive deficits while taking hydroxyzine. Great video, loving your channel. Most you tube psychiatrist/psychologists these days seem to me to enjoy making fun of the mentally ill, or just making click bait. I agree completely, that other ways of coping with anxiety are more helpful in the long term than relying on medication alone. I use EFT tapping, meditation, cold showers, breathing exercises, running, and weight lifting to help my overall health and my anxiety.

Sarah J 1 second ago I was prescribed this last month, one pill 50mg a night before bed. I’ve tried many different meds and muscle relaxers for restless leg syndrome, insomnia, and anxiety. Other meds seem to just make my eyes tired and itchy but hydroxyzine actually helps me fall asleep after about an hour and stay asleep for at least 5+ hours! This is the first medication that has worked for me but just remember that it works differently for everyone. I recommend to try it 👍🤞

I've had nothing but good results from hydroxyzine for anxiety, sleep problems, and agoraphobia/panic attacks.

They give these to minorities cause they don't want to give us benzos cause they think we abuse them,they will give a Karen in the suburbs Klonopin but not someone from the inner city going threw ptsd or real trauma

This med changed my life. I sleep most nights. I no longer require a narcotic to manage the anxiety. If insomnia is an issue give this a try for a couple months.

This medicine has helped me with my social anxiety and nerves and depression. I still feel a little still but it has helped. I’m on my second week.

@@cordyone It depends on the person but the mechanism of action antihistamine tends to cause drowsiness. I've found that some are more susceptible to this side effect and others are not. I like using it only as needed for patients and not as a standing medication and no more than two times per day.

Something quite interesting happened to me. I had agoraphobia and panic disorder so bad that I didn't leave the house much for about 4 years. Started taking hydroxyzine and after a while of taking it I was able to leave the house and do more things than I was able to in years. Then my doctor stopped seeing me and I stopped taking it but the progress with leaving the house stuck with me and now I don't take the medicine but I'm still able to leave the house and don't have panic disorder or agoraphobia nearly as bad. It seems like the effects of the medicine work even when I'm not taking it or maybe it's just the fact that I know I can handle the situations that frighten me. I still have some of my medication left and I'm saving it I don't know what for but I'm tired of doctors because they've all treated me like crap.

I like to think of many of our medications as symptom management. We lack a true disease modifying medication with the exception of possibly lithium. Some commenters get upset and feel like all we are doing in pushing medication that does not take into account the underlying cause of the anxiety or mood disorder. Theses are the current tools we have at this time, as more research comes out we will begin to find underlying causes. My point is that the medication helped you get past a specific symptom you were dealing with, and once you have the new reference experience and change the way you are thinking about leaving the house, then we need to reassess the need for continued medication treatment. I think it's important to consider situations where medications can be stopped and not just counting without thinking about this possibility

It’s a sedating antihistamine that is not that great a treating anxiety. I think a lot of anxiety treatment depends on how it manifests. Is it mostly internal tension due to anxious thinking or physical symptoms such as muscle tension. Both can be treated by CBT and the effects will be longer lasting without this risk of addiction from medication like benzodiazepines. Half the video helped

You have to let your body adjust to it. First time I took 25 mg and it put me straight to sleep. I now take 50mg every evening as it helps me relax and get tired enough to sleep.

I get panic attacks and was previously prescribed 20 .5mg Xanax pills for them. They worked like a charm and made 20 pills last for 6 months. Now I have a new doctor and she refuses to prescribe Xanax whatsoever. She only prescribed me hydroxyzine and it's useless for me. All it does is just knocks me out and it's useless if I'm out or working. I'm just frustrated that i'm being denied medication that works well for me and being prescribed something worse instead.

sounds like you were using them responsibly but many doctors are now antibenzodiazepine which I don't agree with but it's fairly common

@@thimasrehder9364 I was prescribed xanax in the past and used it responsibly, never got physically dependent on it. You only get withdrawals if you take them everyday. Nothing to be thankful for.

I have taken this a few different times in the past and it never did very much for me either time. I was given it for anxiety and also sleep. I feel like another viewer’s comment that it seemed to make me more anxious so I didn’t stick with it very long.

Yeah after the first time taking mine all went well so to speak but then the next night seemed like I was just feeling drowsy but anxious couldn't stop moving my legs rolling around just couldn't get to sleep I thought about taking another one but bc of heart probs. I am to scared to take more then one even though I can take two

This medication is heavily pushed by doctors and FDA because of the obvious, that it doesn’t cause addiction but it’s not a great option for anxiety in most people. If you have heart palpitations ( a main anxiety symptom), it makes them worse, causes severe drowsiness (can’t be used during the day) and lastly it’s an antihistamine so it causes dehydration. Not a great medication imo.

It’s not perfect for everyone and all medications will have their risks. I work on an inpatient unit where this is a standard PRN medication, and some people find it effective others do not. Sedation, hypotension, and QTc prolongation are things to watch out for. I have not heard of palpitation as a major side effect outside the context of QTc prolongation. It can be used during the day, and I have many patients who have no issue using it that way. Many medications cause dehydration educating patients about remaining appropriately hydrated should mitigate this concern.

@@ShrinksInSneakers I just don’t understand the sudden push for this medication. It’s essentially Benadryl with the 5-HT2A factor, causing it to require an RX but they are almost identical in what they’re used to treat. Day time effects wouldn’t vary much considering the drugs are so similar (increased tiredness, lethargy, etc.) I haven’t met a single person that had a positive opinion about this medication and most of them wish they would’ve just bought Benadryl and skipped the co-pay.

@@jacobrowe9226 fair enough, you could use benadryl and I will often use regular old benadryl at 12.5 mg or 25 mg in child clinic. We attempt to make anxiety too simple. You need to ask what is the cause of the anxiety, is it part of the personality structure, is it related to temperment, is it due to severe depression, in the social context, because the treatment will be different in each case. I do not see much value in medication for patients with neurotic type depression where it's mild/moderate chronic and a part of their personality. Therapy is much more effective in the long term and keeps those patients well longer. You should know I do not push any medications, I try to objectively look at the current tools we have and use them in a safe effective manner. Appreciate the comment

This medicine does not make me sleepy with daytime use, it effectively blocks my anxiety and helped me recover from agoraphobia. Benadryl does not help my anxiety. This does not cause heart palpitations for me either. I disagree with the dismissal of this medicine.

This drug isn’t in my stahls nor my Davis’s drug guide yet needs to be in both because of its use in psychiatry primarily. Thank you for this. It must be irony that I was looking to prescribe this yesterday and the lack of information stopped me. My patient has a hx of htn and is a woman over 50….something said no- although qtc is checked using every corrected method. Just didn’t feel right. Didn’t know about the heart thing. Thanks doc…very much. Keep it coming!

I love this med; for me the drowsiness is a plus (helps sleep and definitely takes anxiety down). I’ve found tolerance builds easily though so keep it to a couple times per week max.

I’ve been taking a small dose 12.5 mg every night to help with insomnia due to anxiety. It’s really been helping me sleep and when it kicks in I feel the calmness rush over me. I’m now afraid I won’t sleep without it… do you think 12.5 mg a night long term is dangerous? I’m a healthy 35 yr old with no known heart issues but the risk of heart problems with this medication is kind of scaring me.

I would talk with your doctor about the concerns. It’s a relatively safe medication with the same risks as other antihistamines. People do experience some tolerance to the medication when used daily, I usually recommend using it on as needed basis. I would start with sleep hygiene techniques and move on to CBT for insomnia if that does not work to help with your sleep issues. In summary the medication at that dose is relatively safe but I would recommend looking into other options as CBTI has longer lasting effects over time and sleep hygiene can always be used. Great video, I have the occasional anxiety fist time I was prescribed any type of meds for it and for sleep as well. Just waiting for it to kick in I will provide and update to let you know how it went.

Such a great video dr!! Im 52 and experiencing insomnia for 2 years. Fall asleep wake after 4 hours can't get back to sleep. Tried everything. My Dr wants Mr to try this. I have no history of arrhythmia. Do you feel this is a safer option than Trazadone for insomnia? Thank you!

Both options are relatively safe. Doses of trazodone should be kept between 50 mg and 150 mg and hydroxyzine should be limitted to 50 mg twice per day. As always talk with your doctor about the situation before making any changes. Appreciate the comment

Trazodone is an antidepressant! It's addictive and a horrible pill. The side effects are miserable.

Interesting video!! What do you think about building tolerance and loss of efficacy, I have read it happens fast with hydroxyzine. Could you imagine doing a video on pregabalin / gabapentin ?

I'm not sure it's tolerance in the same way as things like benzodiazepines, but the way to avoid this is to only use it on an as needed basis. It's not designed to be a long term anxiety solution. Gabapentin and pregabalin are good topics we can do them. Appreciate the comment

@@ShrinksInSneakers anecdotally, I've never built a tolerance to it in several months of usage. Some nights I just don't need it and don't take it. I really like it, which is saying a lot because I grew up in a household that was philosophically against medicine pretty much all kinds. But it's been good, no crazy side effects, lots of benefits, and not addictive or habit forming. Don't even have the dry mouth.

This medication is similar to Benadryl and has worked for some people to treat their anxiety. It’s more of an as needed medication and people need to be mindful to keep doses around 100 mg per day total to avoid QTc prolongation. Every person is different it’s difficult who will respond well. I always encourage my patient’s wit anxiety to start talk therapy as well. Hope this helps, best of luck with everything

My doctor knowing I am a heavy user of alcohol (knowing it is going to be hard if not impossible for me to just stop) still gave me this. I am worried about any interactions with the drug and alcohol. I will work on cutting back and getting to the point of quitting but In the mean time I still am prescribed this. What do you know about the drug and use of alcohol? Like if I have a couple drinks and a 25 MG Hydroxyzine am I putting my life at risk?

As it might be causing anxiety. Not to mention the toxic effects of long-term use of large amounts of alcohol. I have a few videos talking about potential medications to cut cravings and reduce heavy drinking days. I will link them here. I literally just had a heart monitor on last week and go for an echo on Monday with my cardiologist and my primary just prescribed this 2 days ago. I have a small amount of arrhythmias on a regular basis but the cardio doc isn’t worried abt them as I’ve had a history of an “irregular regular heart” .. I haven’t taken this med yet but now I’m def terrified to & I have pretty bad anxiety .. should I call him tomorrow & ask for something else?

This is not medical advice, always talk to your doctor before making any changes. As long as the dose is limited to less than 100 mg/day it does not have a great effect on QTc. I would defer to your doctor as they have reviewed your EKG and know the details of your history. 50 to 100mg is insane - I just took 10mg and its now three hours later and sooo hard to stay awake! Yes I am relaxed. But I dont want to sleep!

On the inpatient unit people will take 50 mg 4 times per day, I agree it's too sedating which is why I like to limit the dose to 100 mg total for 24 hour period.

Pamoate comes in capsules and Hcl comes in pill form and at lower doses (10mg).

The anticholinergic properties can cause constipation which can be a good thing in the case you are describing. Another popular use of this medication is for sleep. I will stress this medication should only be used as needed and people do report benefits decreasing with regular use. Appreciate the comment

what if i have a TBI and im given this 100mg as needed ever 6 hours.my brain bleed in the frunt right and all back. craced my skull. but i was told not to not take any blood thiners. and i didnt understand when u said it works threw the blood system. coukd this mess me up.? cuz as soon as my dr put me on this and my other 3 anxity meds i have only gotten worse and worse.

I live in anxiety everyday. Your calls me anxiety but not looking into the screen looking at us you're reading something look into the screen like you're looking at us I feel like people do that on screen I feel like you don't want to look at me because I'm ugly or something

It will make you wanna sleep and sleep and sleep and then sleep a little longer. I went from not sleeping at all to now just wanting to sleep 20 hours a day. I wouldn’t know if it helps anxiety because it just knocks me out.

These are better than benzodiazepines. Works fast. Last a long time. I use benzos even now sometimes for panic disorder but this is better.

It will not help with alcohol withdrawal, I usually use benzodiazepines for alcohol withdrawal or gabapentin. I would talk to your doctor about the options. I hope this helps, and if you haven't subscribed to the channel please do, and spread the word about what we are doing here

My Doc has discontinued clonazepam and put on long-acting Beta blocker (propranolol hydrochloride) 10 mg. And continuing on 100 mg of Fluvoxamine

I thank you 😅 I went to the ER my bp was 188/111 an HR was 210 when they hooked me up to ekg 10 lead the sheet came out red but in the end my HR went down to 71 an bp went to 134/93 so they ruled it as a panic attack an gave me vistaril butttt I’m scared that I’ll get a (QT) interval. What would you recommend take it once see what’s up or just wait the 2 weeks to get more sertraline. 😊 hope you see this

I would stay away from antihistamines in general if you have a heart problem but that’s something your doctor will have to be the judge of.

Hydroxyzine blocks H1 receptors thats the primary MOA. H3 receptors are inhibitory auto receptors located on the presynaptic neuron. There are several drugs being designed to block these receptors which will increase histamine release, this would increase alertness and may be good for ADHD and narcolepsy. The H1 receptors are located in the periphery and when blocked in the brain lead to sedation which is essentially the anti-anxiety effect of this medication. Hope this helps

Does hydroxyzine help you to STAY asleep? I've been waking up every single hour for five nights straight and was prescribed this but I'm having anxiety about taking it and then still not getting enough sleep.

It does the job in the short term but often results in long term consequences. When you take Tylenol for a fever and don't treat the underlying infection you end up worse off. Benzodiazepines do exactly what they should but not getting to underlying cause of the anxiety through psychotherapy ensures anxiety will return when not using the medication. This goes for SSRIs as well. The genetic component of anxiety disorder is much lower than schizophrenia and bipolar and are more environmental than genetic in their underlying cause. It goes beyond the sedation aspect, independent of sedation there is modest serotonin-2 receptor antagonism, histamine H1 blockade, and muscarinic blockade all resulting in a cumulative effect on the physical effects of anxiety. Even medications like benzodiazipeniens which work on GABA-A receptors primarily ameliorate the physical symptoms of anxiety often patients anxious thinking continues unless of course you change the way you think. I believe this is the one that can negatively affect your eyesight.. was on it for awhile.. my vision became blurry & can't even see TV clearly.. I looked up side affects..I told Dr & stopped immediately..

I've never had the blurry vision from hydroxyzine, and if I did I would not take it. That's why I wonder if taking supplements of phosphatidylcholine counteracts the anticholinergic action. I take both sunflower lecithin which is high in phosphatidylcholine, and hydroxyzine. My experience: taking the lecithin removes the tiredness and foggy brain that I often get in the morning after my dose of hydroxyzine the previous night. Wish I was more scientifically savvy, to understand the exact science of brain chemistry better.

This is not a common side effect. Although I never rule anything out because each persons is an n=1 and strange things can happen. However, the literature actually has evidence to support the opposite, hydroxyzine may DECREASE tinnitus. People are certainly on this medication for longer periods of time. When I offer this medication, I stress that it should be used on as needed basis. On days where anxiety is running high it can be helpful, if there are days where the anxiety seems tolerable it may not be needed. This prevents tolerance. Hope this helps, appreciate the comment

It can be tried in panic attacks but not FDA approved for the that disorder. As far as mixing it with herbals it depends on the herbal and you should check drug interactions and consult your doctor about any CAM treatment you are considering. I do mix hydroxyzine with herbs such as 5 HTP, yarrow, turmeric, ashwaghanda, rhodeola, holy basil, valerian, melatonin, etc with no ill effects. Do ask a Dr first.

I know in Europe they limited the dose to 100 mg/day for QTc prolongation but I'm not sure why it's not approved

If you are having blurry vision from vistaril it's likely due to anticholinergic side effects. You can combine the medication but I think the clonazepam should be enough unless the goal is to get you off the clonazepam. Anticholergenics such as Vistral are contraindicated in a lot of people that are genetically predisposed to NAFLD and low choline. There is a percentage of people genetically predisposed to NAFLD. The study of genes is in its infancy. Heavy use of anticholergenics besides the histamine blocker combo can lead to M.S. and NAFLD. Medicine needs to be more specific. Insurance should pay for testing of specifics rather than generalizing these medicines that are effective but have unintended long term serious health consequences down the road. It becomes more profound when combined with over the counter common anticholergenics.

This should work Immediately 15-20 minutes after dosing. However, it can take several weeks of daily dosing before the full effect is seen. Hope this helps, appreciate the comment

@@johntrevy1 I think of medication as a good pair of running shoes. You can run a marathon without any shoes but with shoes it's a lot easier. In some cases the combination of therapy and medication is best. There are other options as well things like rTMS may be resourceful.

Hair loss is not a common side effect of this medication and I have not seen a case where hair loss was an issue clinically although this medication is not intended to be used every day or as a prolonged solution for anxiety. A tremor that is medication induced has no impact on the long-term development of Alzheimer’s disease or ither neurocognitive disorder. I have mostly placebo with that but cool thing about placebo is it works good we should all invest in placebo. In all seriousness a recent meta analysis did find hydroxyzine to have an effect size of 0.45 which beats the hell out of antidepressants for anxiety disorders

Sure does, I spent two years in a child outpatient clinic and saw a lot of hydroxyzine prescriptions. This is one of those medications that helps some people but not others. It's not designed to be a long term solution to anxiety or taken indefinitely.

Benzodiazepines do what they are intended to do, but in the end the anxiety comes from somewhere and figuring out the underlying causes goes a long way towards reducing symptoms. I would say lunesta is the better of the two options. However, I recently covered hydroxyzine in my latest anxiety videos and it works modestly well. The effect size is 0.45. There is some increased risk for sedation, dizziness and some of the common side effects but on an as needed basis I may consider using both. Medications are going to work differently for different people. The research tell use hydroxyzine has a moderate effect size in GAD which isn't too bad. It does not cure anxiety, in fact no medication cures anxiety. In my mind thinking differently and psychotherapy help people the most and medications should be thought of as lubricant to help make that process a little easier. Addictive medications are not the only effective medications but I understand the struggle

Everyone is different and responses to medications are variable. For anxiety disorders this is far from an ideal treatment, I think psychotherapy should be the foundation of the treatment.
