In this video, Toronto Physiotherapist James Braithwaite describes a great alternative exercise for remodelling a damaged proximal Hamstrings tendon. This type of injury is characterised by pain in the crease of your butt when you run.

I don’t have a treadmill, what is the best way to recreate this movement? I have been working with a PT for several months and I think this addition will help me.

HI James - thanks for the video. I have this injury. I'm going to read the blog, but it would have been GREAT if, in the video, you explained why/how doing this treadmill exercise might help the injury. You say people should feel the discomfort come on during the exercise. I'd love to know how that helps heal the tendon. Thank you!

Another thing u can do is with the treadmill off is face forward it and straight leg push it back . Or a glute kick back with ur leg straight. These would all help this injury. And wrappung the hamstring up tight during physical activity will help too

Thanks for the help. Just to confirm, am I working with the treadmill and guiding my leg along the belt, or should I be trying to resist the movement of the treadmill?

Resist it. Eccentric loading of the hamstring with this injury helps strengthen the tendon in a different state as oppose to concentric loading. Same technique is used for psoas Achilles etc etc.

I’ve had proximal hamstring tendonopathy for a year now. I’ve been incorporated physio into my gym workout. I’ve been running too. The pain got much better than 7 months ago but it’s still there. I feel it during and after running. I feel it for a few days after doing legs in the gym. I think what makes it worse is yoga and stretching the hamstring. How long will It take to fully recover?

How you getting on buddy? I have also been plagued by this for a year. Sometimes feel like its healing and then out of the blue goes all cranky again. I just cannot shift this. Tried pretty much everything short of surgery.

stejeliteK I’ve had it for 1.5 years from lifting. Only complete rest induced from this corona virus situation seems to be helping.

stejeliteK I suggest looking into BPC 157 and TB 500. That in combination with 6-8 weeks of rest and foam rolling seems to be the fastest way.
