I always thought it was more that it shows his struggle of killing his ''father'' which he was tortured to believe and remembering his old life, saving Batman in the process, which causes the duality with crying/laughing at once. You basically see both personalities there.

He being shot by his “son” and the actual projectile probably hitting a place where it would hurt a lot, he falls unconscious and impales himself even more. Hence the answer to OPs question would be, it's a comic book world made for children and that's why they don't work like real worlds, hence joker was never given a death penalty.

@@aust2354 Dynamic liquid made for realtime is hard to create in games but they could have preanimated the liquid out as high poly animated geometry with a screen space refraction element for the ingame cinematic. They could have simulated it using a high poly vial and later swapped it for the low-poly game model. I don't see any reason why UE even from 2010 couldn't do it in the way I describe. Bottomline, the liquid doesn't need to be dynamic.
