This is not surprising but sad none the less. The NHS does have a funding crisis, but it need not have. It is a political choice, and the result of that choice is that people in need of long term support are being denied it, replaced with a temporary fix, which just leads back to needing the support again. As you say adult care has also had its funding cuts so very little there also. Damned fi you do, damned if you don't. The people doing the work are generally helpful people who are having their hands tied, consequently lots of people leaving the NHS and Care services to work in other jobs.

The level of supervision and/or intervention needed determines eligibility. The key words here are primary health need,that are beyond the remit of social services to provide. Step 1 after hospital discharge is the completion of the initial checklist. if you score high risk in those domains,then you go to DST assessment, if it's low then they look at supported living/care at home/ self funded residential(with/without nursing) Having been through this obstacle course recently,I would back up what stu says,and make sure you REALLY do your homework if you have to go through the DST process. Make sure you know the LEGAL definitions of primary health need,and do study the relevant case law(ie coghlan case).You will need it because it is not uncommon for the severity of the ailments/risks to be underplayed.

CHC funding decisions are based on what medical treatment is required and whether nursing care is required so it will depend on those requirements rather than her conditions or care needs. Everything you have said so far only reflects as being care needs, but of course that situation may change. Rest assured that if CHC funding is viable it will be brought up with you by either the medical professionals or social services.
