This is my study guide for students looking to PASS Step 1 of the USMLE as quickly as possible and outlines a 2-3 month schedule that mostly uses UWorld, First Aid and Dirty Medicine / KZfaq videos. As a Canadian medical student in my final year of clinical rotations, I needed to come up with a plan to pass one of the hardest exams in medical school on my first attempt and while studying part time... and I'm confident in saying that for me, this was what worked best! The Step 1 exam is basically a harder version of the MCAT, so you're going to see a lot of crossover between how I approached both exams. Just wanted to leave my pre-exam stats for people heading into the exam - you've got this! UWorld first pass - 53% correct and 90% complete UWorld Form 1 score - 190 (17th percentile) - 2 weeks before exam date UWorld Form 2 score - 176 (8th percentile) - 1 week before exam date NBME full length exams (not done - I would highly recommend AGAINST skipping these!) Step 1 Test Day (April 1st, 2022) - Felt nervous afterwards but pretty good in reflection :) Step 1 Score (Released April 13, 2022) - PASSED (No score breakdown available) Happy to take questions if you have any. My practice test scores were a product of me doing the forms at 3 AM after working but also I think that UWorld is actually just harder than the Step 1. Best of luck!

I follow around 25+ med school channel, I truly believe YOU ARE THE BEST FROM FAR, I don't know how you're not nb1 in the game. The way you talk, and your whole vibe is just great. congrats on passing! and thank you for the transparency regarding all of the life events you had to juggle while studying. it's not as frequently mentioned how hectic and stressful life is during step studying especially if you don't have the luxury of just devoting several months to only studying. when do you plan on taking step 2? what is your approach, and are you going to make a video for that?

Hi Katya :) Thank you so much for the well wishes and I'm glad that the transparency was helpful. Step 2 has been booked for June 21st and my Canadian MCCQE 1 board exam is May 16. I will make a video for both of those exams - or probably a few videos on those. My approach is to do the UWorld Step 2 Q bank and spoiler alert - I've found it very helpful so far. Truthfully, I'm kinda burned out writing all these exams back to back but I believe that doing step 1 in relative close proximity to step 2 is probably very beneficial. I'll provide more updates in the future via video - see you around :)

Hey thanks for the video, really well done! Which Anki deck(s) do you recommend specifically? Also thanks for the transparency with your % correct in Uworld. Mine look very similar (although I don't do nearly as many blocks as you a day) and like you said, sometimes seeing those LOW numbers can make you nervous.

What I loved the most is you sharing your Uworld comulative score. I am doing Uworld, and my cumulative score is 51% (after 78% of questions, with a slight overall positive slope). I was so depressed until I saw your video (best ever). Thank you so much! I am pretty much using everything you used except I rarely annotate FA, and I have no idea where I need to go to find the ANKI flash cards. I googled my ass off, and I have no idea where this database is. Any advice on that would be greatly appreciated!!

Since you’ve made A LOT of progress as a content creator I think it would be a great video topic to go over again. Thanks so much Jorge, I really appreciate it :) I think that this is definitely something that I may get around to again in the summer if students have particular details that they need me to clarify. What I can say for now is that the MCAT plan video has almost crossed the 50,000 view mark which is incredible to think about. I routinely (maybe on a monthly or biweekly basis) get emails or messages from students who say that the plan allowed them to be successful and have even spoken with 2 students who have achieved perfect MCAT scores of 528 while using the plan. There might be room to add things in (Anki cards and UWorld questions if you have more than 3 months to work with) but I would still strongly recommend my original MCAT plan at this time. My Step 1 plan also draws on inspiration from the MCAT plan, but is designed mores for students looking to pass... whereas on the MCAT, we were shooting for the gold LOL. I am planning to start my studying journey soon after I finish my fifth year finals , I do have a question regarding the time frame it should take me to finish . So I haven't really done any Anking flash cards in previous years and I regret it, I feel like I am pretty decent in system reviews but as you said , basic science like immunology, biochemistry and pharmacology is my biggest weakness . Do you think I can finish in 3 months doing your exact study plan but more like 5-7 hours of studying daily ??

Hi Basel :) Glad it helped. I think that it can totally be done in 3 months with 5-7 hours. I assume that being in 5th year means you've finished your clinical rotations - this should be helpful as well. Get a solid question bank and just start plugging away... that's my recommendation anyways :) Good luck!

I am a 32yo IMG back at '15 and I recently started studying for Step1. Truth is what kept me from doing it during med school and afterwards during residency years (mocrobiology) was both mental and financial issues. So my age is worrying me too much having to work simultaneously and most importantly what you said about memorisation I tried anki and seems too superficial and just rote learning and so thata applies for first aid it is impossible to rote learn first aid that said of you get a question implying prader willi and multiple choices ask for chromosome not remembering the chromosomes out of heart is rote rearning not understanding. Whats your opinion? I would appreciate your time for my concern? TIA

First, It was an absolutely enlightining video for me and thank you! Second, yesterday I took the exam and before the exam I was so nerveous and questioned myself if I should delay the exam or not bcs last week, correct ratio of my free120 was 65%, nbme30 was 52%(which was disaster), nbme 29 was 59% and uworld stats were similar to your's. I watched your video and saw so many similarities with your story so decided to attend the exam... In the exam, everything felt like it was like having a test exam in my home at last days, nothing more difficult or easier than that. The exam was not easy at all and yes I found myself in a dilemma for many questions but whatever I didn't feel like I did a disaster exam.. Anyways I really wish to pass this exam and would like to here some motivational words from you hahahahh

Hey Burak :) Huge congrats on finally being done this thing! I had a similar feeling coming out of my exam - not disastrous but just a little uneasy (which id say is totally understandable after an 8 hour exam LOL) You did the prep, put in the work, and if your story was similar to mine - absolutely grinded it out the last few weeks. I think that the best thing to do now is trust the process and keep your fingers crossed 💪 The next 2 weeks until you get the score back are gonna SUCK... unfortunately 😅 (speaking from my experience). 😅 I forgot to update here, and here is the good news.. Passed! Although I couldn’t reach to the confidence interval at the preparation time, it went well. I believe it’s about courage around these correct ratios. So believe in yourself!

I think the most important question is how you can function on so little sleep for such a prolonged period of time? The sleep deprivation would have gotten to me!!! And that would snowball into not having those 3 - 4 hours of study time per day.

Hi Elsabe :) I think that's a very valid point... Truthfully the last 3 months haven't been the healthiest for me and I know that the sleep deprivation was taking its toll. I've really been trying to catch up on some missed sleep in the last 2 weeks since finishing the test and before starting to study for the step 2 and Canadian boards. Personally, my key to functioning off of little sleep is a combination of lots of caffeine, strategically placed naps and daily physical activity. I would like to make a video on it maybe at some point in the future, but it's one of those unwritten rules of medical school that I don't necessarily want to perpetuate. Unfortunately though, sometimes there's only so many hours in a day so you need to give things up. All the best!

Thanks Nurkali :) The screenshots shown at 5:41 were from my UWorld account after having completed >90% of all questions. There are way less questions in the bank on Biostats, biochem, histology and immunology then there are for pathology (as an example). This is because of the relative representation that these questions will have on the actual exam. As a result, when I completed more than 90% of questions - I had already completed all of the available questions on those topics 😅 Hope that clears it up

Hey :) From my understanding, it depends on the specialty and the state where you intend to practice. Having the Steps completed makes the process much easier, but in some cases, even Canadian trained doctors may need to complete some form of additional training. I suspect that relatively speaking, you will not have a very difficult time making the switch to working in the US if you want! Goodluck

They say to be safe over 70% on NBMEs.

How were you able to do around 140 QS per night (3-5 hours)? I am having trouble in doing more than 50 QS. My percentage is the same (53%), however I have not done as many QS as you. Any advice in how to push myself? I really want to tackle in 3 months or less ( if possible). Thanks. Great video by the way.

Hi :) Please see my Step 1 VLOG if you're interested in my personal reasons - its easier for me to explain in the video. For most other Canadian students though, writing the steps helps keep options open for fellowship training and practising in certain states.

Hey Jerry, sure thing :) Take a look at this document if you haven't seen it already - www.usmle.org/sites/default/files/2022-01/USMLE%20Step%20Examination%20Score%20Interpretation%20Guidelines_1_10_22%20%28PF%20Transition%29.pdf Statistically, only 5 percent of students who write step 1 will score below a 200 (keep in mind that the passing score is 196 this year). Because Step is a scaled score (your score is adjusted based on the difficulty of your test, etc.) then my rational with the UWorld tests were that so long as I was beating out 5% of the students who did the forms, then I would have likely scored at least 200 points on the actual test. So for example, when I scored in the 17th percentile on UW form 1 and we apply that percentile to the official document that I linked - we get a score of around 215. I did not do any of the NBMEs due to time constraints but would highly recommend people do them :) Hopefully no-one watching this video is studying for step while applying for residency and can be afforded with a little more time. Hi Shreenidhe :) Everyone will be different depending on their education and how what they've learned will cross over to the American board exams. I recommend 2-3 months of part time studying - but your best bet would be to take a NBME practice exam and see how you do. Then you can make a rough time table for how long you will need. what do u think of kaplan q bank??? This is what the school gives. If you dont have uworld , can kaplan q bank do the job??

Hi there :) personally, I have no experience with Kaplan so I can't give them an honest review. From my experience and from talking to other students, UWorld is the most commonly recommended but I suspect that man students have passed using other products

Great question :) Do you mean for medical school or residency? From a residency perspective, Niagara is an amazing training location for family med imo because it combines a varied patient population with great training from specialists. Because St. Catherines General is a community hospital, we don't have any specialty residents working there (except for general surgery). This means that when I'm on OB/GYN for example... all of those consults are ours - and greatly increases our exposure and 1 on 1 instruction from specialists. This is the same for emergency medicine, psychiatry and everything else. Also Niagara offers a longitudinal post grad training instead of block training. This means that I will get to be in the family medicine practice for 2 years straight even when I'm rotating in the specialties. All this to say that I believe the Niagara program produces very strong family docs - second probably only to the rural programs where you get a lot of responsibility from the very beginning. Point 2 - Niagara has great job prospects from a family medicine perspective and has recently been voted I believe #2 in the country for best places to practise as a new doctor. Hope that helps!

Thanks for these tips! You mentioned a 53% success rate after completion of 90%, but I was wondering if you could share what was your success rate closer to the exam, let's say 2 weeks prior to the exam. Thanks!

Hi Basma :) Please remember that I am a Canadian medical student and soon to be resident. The step 1 and 2 exams are not required for us in Canada.

I say its about 35% backwards compatible from step 2Ck to step 1 imo. You will very likely pass, but I do still recommend working on step 1 focused practice questions and using your performance their as a guide for how hard and long you will need to study 🤙 Congrats on that step 2 score though 🔥

Why are you writing Step 1? I thought a Canadian trained doctor can compete for residency in the USA without Steps.

Hi there :) Any medical student wishing to match to residency in the US after medical school needs the step exams...regardless of state I believe. For doctors who have completed residency, whether or not you need the steps to transfer will depend on the specific state. Finishing the steps keeps doors open in the future, especially since guidelines are subject to change

Actually I think the biggest predictors of USMLE score performance are 1) MCAT score 2) which medical school you attended (especially US based vs IMG) 3) your medical school grades There’s a few studies published on this if I’m not mistaken - I had read about it in passing a number of months ago

Hi Belan :) this is from the USMLE website: The total number of attempts allowed per Step is four (4). Examinees who have attempted any USMLE Step (including Step 2 CS) four or more times and have not passed are ineligible to apply for USMLE Steps.

Deciding to write Step 2 is something I'm currently juggling with right now. Because I'm not doing residency in the US, I just need to pass and don't need a super high score. My Canadian exam is May 16 (MCCQE 1) and residency starts July 1st. So the plan might be to write step 2 on the weekend of May 28, which gives me just over a month to prepare 🔥 I'm going to try and book a test date later today and will make a video at some point to explain if it all goes through :)

The exam is pass or fail. They do not show you final scores. My official score was written as “PASS”

Congrats on your passing score but unfortunately this video is quite misleading. There's no way you can pass Step 1 by only doing U-World questions, you clearly have some background from studying in medical school or rotations that would substitute the required knowledge from First Aid. I tried this method of only doing U-World and it wasn't great at all. This is a major exam for all medical students and it's important to clarify how you got all that knowledge outside of U-World that assisted you in passing the actual exam. For anyone reading this, do First Aid and U-World questions together then take the Step 1 exam. Don't risk getting a bad score over these tips that unfortunately don't reflect the full scope of one's knowledge and how they studied for a major exam...
