I feel like there's nothing you would get from doing a second undergrad that you wouldn't get form a post-bac. I was going to say that strongly, however I'm in the U.S. and I realize this is a Canada video so maybe it's completely different.

This video is THE BEST summary of family medicine finances and is completely hidden or ignored in medical schools. We need more transparency like this so false info like doctors being paid with prescriptions do not spread throughout the public. Thanks NXTgen!!

I was in a fatality causing accident in 2010, i lost my fiancè, and i was permanently disabled..... ive spent so much time in hospital and rehab, like ive a got a few years worth at this point under my belt so i have nothing but the utmost respect and gratitude to everyone whos been involved me, doctors, njrses and everything in between..... because was a **unique** and **interesting** medical case ive always supported being a guinea pig and allow people to learn from me. I did once let a student nurse give me a blood thinning injection in my stomach for his first time because id rather it be me that has a high pain tolerance and im patient. But i confess i never took that thought further as to students doing a lumbar puncture of intubation for the first time, maybe thats a slightly self perseving mindset because id rather think of all the medical health professional i interacted with are competent, but yeah theres got to be a first time for everything and everyone....... EDIT: This ended up being quite a trip down memory lane and brought up a lot of nostalgic memories. Its been such a rough decade but in the last few years things have finally, finally at long, stabalised. Well as stablised as someone whos got what ive got going (its a lot) is going to be *""stable"*..... and i certainly dont feel like its something that ive **"come out the other side of"** but at least i have prgressed far enough in this journey, thatbim realistic about, will be a life long journey..... but maybe with enough time ill kinda be able to get to a *new normal* in some ways..... but im really thankful for this moment youve given, its the first time ive been able to look back in a way that was more **peaceful**. The weird thing is i dont use social media so i dont post anything at all..... but here i am in the comments section of a KZfaq video letting out such deep and personal inormation that maybe a few eyeballs might see but i doubt anyone will read this because i know if i saw such a* horrendously* long post i would not stop to read it. So in saying that, on a whim, im going to ask that if youve read my post the press the like button so i can know youve seen it. Or comment that youve read this if you dont want to like it. This isnt for attention im just curious if anyone at all will read this...... i dont normally check back when i comment on a video, and by comment i mean **"i love you video keep up the good work"** type thing but i will come back and check on this!

What exactly are your office hours and does your office have a website? The first time I tried to get an application form was my fault, I forgot that it was Wed afternoon; and even though I know you're not in on Friday's I figured someone would be there so I could just grab a form but it was closed then too. I'm gonna try again tomorrow (tuesday) afternoon, hopefully 3rd times the charm 😄

Great video! This is an excellent summary of the models. Don't know why the information is so difficult to access here in Ontario. I'm a new attending just like you. I've been grinding at walk ins about 80 to 100 patients a day 7 days a week to pay off my student loans. While it is lucrative akin to neurosurg money, it's not really sustainable long-term. I agree that the FHO seems like the ideal balance, but it comes with its own set of worries about being dinged by your patients using walk in clinics.

@@chuksezeike3183 It's doable in a walk-in clinic but you can't really provide proper comprehensive care in a primary care setting. It also depends on the level of support you have in triaging and nurse assistance. Depending on the clinic someone could see about 10 to 12 patients an hour. Most people work about 7 to 8 hours to see 80+ patients. I really finish my notes on time before I move on to the next patient either through artificial intelligence or through no templates. Unless the patient is very complicated where I'll have to actually spend a larger amount of time sitting down and documenting every fine detail.

Hi everyone, thanks so much for tuning in to this week's video :) Please leave comments below if there's anything that I can help with. Just a few things that I'd like to discuss at the onset though. 1) These tips / discussion points involve family medicine in Ontario. Things work differently in different parts of the country, but a lot of the underlying concepts are similar 🇨🇦 2) My bias is very clearly as a family doctor who has been looking at long term practice. There are many doctors in Ontario that have gone public by saying that they needed to close their practice because they didn't make enough money to sustain the business. I'm not trying to put these people down at all. I am only trying to (hopefully) provide insight about why finance and medicine is a lot more closely integrated than I think most people imagine at first 📈 3) *For students*: I will very likely be slowing down on financial discussions here for the next few months. These talks are nuanced, highly political in nature and with a high barrier of entry. If anyone would like to learn more, many doctors and billing experts are happy to share what they know - especially with other students. Please ask questions as you go through your training and do not let a single jaded physician change your mind about a career choice. Focus on finding mentors that have made it work for them and learn from what they've found success in 💪

Hello doctor I'm so motivated by your videos and I love your patience and hardworking. I really need your advice I'm from Egypt and I'm at my fourth year in medical school. I suffer from domestic violence and my mom used to punish me by locking the door and not letting me go to exams that really lowered my GPA and I have 2 years gap because of that and PTSD. I left my parents house because that really destroyed me and my schooling, I hope you don't judge me for that as I feel like if you don't go through this you won't understand stand . My question is would the 2 years gap affects the match ? And another thing I work and I pay for my own things but that really barely enough for my rent and food , do you know any organisation or people that would help me pay for step1 , they can pay it themselves because it's okay to be suspicious and don't believe me , and I will work more to be able to pay for step2 after I graduate because my current job is giving me 3000 Egypt pound and I use them all to pay rent , food . Would you recommend one joining a multispecialty team? i.e a practice with pediatricians, Rheumatology, peds and Int med as opposed to a FHO or FHT? Thanks!!

Thank you very much :) I’m with a FHO which tends to be a very good earning model when compared to FFS. When deciding between FHO and FHT I suppose the real question is how much control do you want to have over your own office? Because FHTs are multidisciplinary, these are often relatively large offices with > 5 practitioners (NP, PA, MD, psychiatrist, dibabetes educator, etc) working in the same place. With the increased staff comes increased admin costs and the need for a building manager in many cases… it’s not a bad way to do things, but it can get complicated quickly and depending on where you go, “overhead” can be on the higher end - shop around and see what you’re being quoted. In my opinion, the FHO model has been my favorite but this is made even better if you have good access to specialists in the area that can help with referrals when you need them. Unfortunately, specialist referral is hard to come by in my area in many cases. Final recommendation - keep your overhead low, see a lot of people, do good medicine and go where you feel most comfortable working. Best of luck :)

Hi Doc, it's so nice to hear your thoughts. I am also a Canadian and an IMG but I completed my MRCGP training from the UK, now I am planning to move to USA. Can you please advise which US states are easier to get in? I have LMCC from Canada, plus ECFMG I also have USMLE 1,2, Step CK & CS currently I am working as GP in family medicine in UK and I would really like to hear your thoughts on this? Which US states are easier to get in family medicine? Please advise

Hi :) I don’t know what your experience has been like so far working in the US, but instead of making your decision based off of which state is easiest to work in, I would suggest going through the licensure process in the state that you can see yourself living in long term. Each of the states is different in terms of people and their experience with the healthcare system and working in Florida is much different than working in New York. It’ll depend on what you’d like for yourself. I would recommend looking at the process on their official websites and deciding if it’s worth it for you. As a personal example, I will at some point soon look into getting licensed in Florida - I have no plans of moving there right now but think it will be beneficial for me to have personally. @@nxtgenmd Hello once again Doc, and thanks for your response. Actually since I am not trained in Canada I am facing a lot of hurdles in transitioning from the UK to the USA physician pathway as there are only few states that accept UK trained MRCGP. But I am still hopeful and exploring, trying to find new information from people who are going through similar experiences. So far it looks like I will have to do a fellowship in the USA to get the flexibility to choose the states which I desire 😞. Anyway the UK job market isn't that great but I am still thankful since they gave me a chance. Anyway it's really nice to hear back from you. If you get any information regarding transitioning from the UK to the USA please do share it.

For a hopeful IMG who wishes to one Day practice in Canada, your vids have been helpful. Now really interested in this family practice of yours. Got many questions though - Shall u be hiring Nurses, lab guys and other assistants to help u? If so how are they compensated...and if not, I'd be intrigued about how you'll handle the work load alone. Plus how did u manage to finance the place..like the setup and all the necessary equipment

Hi :) we have 2 nurses working with us and 1 full time front desk staff. I pay their salaries directly along with the other doctors working - the government does not help us fund their salary so it comes out of our own salary. I pay my overhead costs on a monthly basis - in Ontario, doctors are usually not be paid for the first 3 months of working as OHIP billings are held for 3 months. So you cover the costs of your office in the beginning with loans from the bank or incentives paid to you by the city you choose to work in if they have a program like that in place. Hope this helps

I'm also starting my internship year here in my country in August This year...an intense working period of 12 months.. rotating in all the 4 major clinical rotations before we can be licensed to be medical officers, the would be Family Drs according to your system. We had a person bring their dog to the VA hospital for treatment. He looked up "vet hospitals" and thought we treat animals. I can't confirm or deny if the pet was treated.

Shortly after watching the farmer pain scale video, I had a female farmer check in to the ER. Later, an african american male checked in with chest pain and an elevated troponin. He looked very uncomfortable. When asked what his pain was on the scale of 1-10, he replied "Oh Jesus."
