Hi jay! Is it okay of im not specific on his medical condition and focus on what he can only perform at home? I am writing to refer Mr. Murray who suffers from leg infection, heart and renal diseases and obesity . He is medically stable but will require home assistance from your agency. Although Mr. Murray suffers from different medical issues, he is coherent and can independently mobilize with a walker. However, bladder and bowel incontinece is noted at times and he is on a low sodium diet with fair appetite. Also, i reminded him yesterday to always wear his personal emergency response gear for emergencies. Socially, Mr. Murray lives alone and drives his car every summer. He can idependently perform grocery shopping, laundry, administration of his maintenance drugs and can manage his medical appointments. Given the above, it would be greatly appreciated if you could provide a personel who can provide assistance 6 hours every week to perform house chores and washing of his lower extremities.

is it okay to use present form of verbs/adjectives when you are referring to a medical history. i noticed in the sample that you wrote ' In terms of medical history, Robert experiences...' but medical history is pointing to the past? is it okay to assume that the patient is still experiencing the illness at present?

celulitis is a bacterial infection NOT a result of obesity , however, why should I take the risk by adding information which is not mentioned in the notes ??

patient was hypertensive ,which was the main reason behind the recommendation of low sodium diet.Furthermore ,the relevance of personal emergency system is highly uncorrelated with cellulitis ,which was considered to be the main issue suffered by patient.To a great extent , emergency system is beneficial to fathom the blood pressure but it got nothing to do with cellulitis .washing the legs adequatly and avoid making contact with the belongings of other people should be recommended .owning a car is somewhat relevant because a patient with cellulitis and odema on both legs cannot walk at ease
