The drill should have been "red tagged" out of order with the damaged grounding pin. And the 4 sq outlet box been de-energized tagged until it was properly repaired. Power ought to be shut off with replacement of a 3 phase breaker, and been "red tagged" when it tested defective.

Much was made of lack of PPE in last case presented. Fair enough. Electrician was troubleshooting so had to work live. PPE would be critical. So I’m surprised little was said about it in case 1. There were numerous errors in procedure as noted but, additionally, power SHOULD have been shut off to the panel for breaker replacement; live work was likely not justified. But, as live work WAS being done, electrician should have had full PPE including face shield, gloves and FR clothing. Probably would have saved his life. Also should have put cover back on panel before energizing. Sad.

First team didn't want to stay and finish the job. Why? To save OT pay? Could have just thrown away the defective CB.

What exactly electrician in last example did wrong? I didn't even seen any actual electric use such gloves - always bare hands. Also how do you even imagine working while wearing these massive gloves?
