Tonicity only deals with effective osmoles.
Urea is NOT an effective osmole (it passes freely through the plasma membrane, so by definition it cannot cause an osmotic gradient).

The answer is D Because lung cancer causes ectopic production of ADH ,which is becoming high level ,that lead to more and more reabsorption of water which cause hypotonic plasma(low s.Na).

Amazing work !! One of the most tough topics to understand, also very very important in practice. You made it very simple. I wonder what year of med school are you in ? Also, a very important thing to add is that some fluids that we give are isoosmolar but hypotonic so they might cause hyponatremia with normal plasma osmolarity, a condition with a very narrow and specific DD.

A very famous example is in Trans uretheral resection of the prostate where surgeons use iso osmolar irrigation fluid >>> causes TURP syndrome ( hyponatremia with normal plasma osmolarity ). Nowadays surgeons use glycerine for irrigation to minimize this problem.

There is one thing that makes me quite confused. In your video about osmolality vs osmolarity you hade the following mnemonic: "Osmolarrrity is mesured in osmoles per Literrrrs" but in this video you say that osmolality is measured in osmoles per liter?

Osmolality should be osmoles per kg, but again-clinically-it doesn’t matter! Hope it helps!
