What an amazing video!!! Very engaging, thorough, and informative. 38 minutes well spent. I am 63 y/o and 1 week post pelvic floor prolapse repair surgery. I have learned so much from this video than the few minutes I spent with my surgeon. Now, when l go for my follow-up visit, I will be equipped with a million educated questions. I will also be requesting pelvic floor PT and possibly estrogen cream. Thank you so much for sharing your knowledge!!❤

I can't tell you how many videos I watched before this one. I felt so annoyed and frustrated because NONE of them answered the questions I had. YOU ARE ABSOLUTELY AMAZING! Thank you so much. I am certain you are helping more women than you could possibly know, even just by posting this video, and in your practice, you are an angel. :). 37 minutes and I listened to every single one, because I just went to the urologist I was referred to by an absolutely NON EXAMINING nurse practitioner, a urologist that specializes in prostates. The staff didn’t even know what to do with me. I literally thought that I’m irreparably broken with no hope of ever being able to be healthy again. Now to find an amazing doctor like you.

That's about 36 more minutes than my Doctor discussed with me prior to my recent hysterectomy with a cystocele bladder fix tossed in for good measure. I was only told why the mesh could not be used due to insurance regulations and risk factors. Here I am, almost 6 months after surgery, dealing with my bladder again. I think it must like living in the basement! I wish women had been given better educations regarding this phase of our lives when we were given our sex ed courses in school and college! THANK YOU for this very educational video!

My surgery failed after two months. I’ve made a couple of changes that are helping. Find me on Facebook, friend me, and private message me and I’ll tell you what I am currently doing. I don’t ever want to have that surgery again.

Bless you. Forever grateful. I’m 78, just had vaginal/uterine prolapse surgery. Just had two week check up. No one mentioned the estrogen cream. I will be getting that on Monday. This information was so helpful. This video ought to be in every doctors office.

@@llamarama111 Hello, well my surgery failed after two months. This drs information was the best I have heard. So I still have the problem. I have made a couple of changes that make it easier to live with. If you can find me on Facebook and friend me and private message me I’ll tell you what I have been doing. Thank you very much for sharing your wisdom and educating us. It's amazing how many women are affected by POP but are embarrassed to talk about it. Thank you for 37 minutes of prolapse talk! I am the mom of eight children, all vaginal deliveries. I’m in my early 60’s, not on an Estrogen cream, had a sudden weight gain of fifty (50) pounds six years ago due to a three year treatment with steroids, (I’ve since lost twenty pounds, yay!), and currently I have a bulging bladder and uterus. Thanks to this video, I now have a better idea of what decisions I want to make when I visit my GYN next week. I definitely feel more empowered. Thanks. (Even so, the Foley still scares the heck out of me!🤣) Peace out. ✌🏽

I’m wondering what you decided to do, and how are you now? I had a full repair after my last baby and it was the best thing I ever did. That was 25 years ago now, and I’m still going great, and very active. All the best to you.

This was a great explanation. The Doctor I had did not help me understand anything he said. Its too bad there aren't more women Doctors like you. After seeing him I was petrified. I am now seeing a female doctor for a pessary and she has been great. Thank you so much!!!!

Paraneoplasty deserves more airtime, IMHO. The lack of sensation a woman can feel because of vaginal muscles stretching from childbirth is incredibly depressing. And some doctors do not take it seriously at all. I had one female doctor practically scoff in my face about it, insinuating that it is for the male partner's enjoyment only. Not so. I left the doctor's office feeling disheartened. And so your mention of it is appreciated, doctor.

I had the surgery after my 4th baby and it has been amazing. I can now be active without feeling like my bladder is about to fall out . That was 25 years ago now, and I. Still good. If it’s affecting your lifestyle adversely, as mine was, then go for it.

Thank you for this video. I didn't actually want to tell anyone about what was going on with my prolapse. I had a total hysterectomy at 27 and I am 42 now. I just started looking this up because I have major bladder/kidney problems. I have had many surgeries to put stents in because I can't urinate properly which of course led to kidney infections and stones. I have noticed for over 2 years this bulging out of my vagina. Now I can't urinate or get my stool out properly. I decided to take care of this myself and research as much as I can. I found a lot about these different types of prolapse. I have seen so many different doctors and absolutely NO ONE noticed this!? I don't understand. Women are overlooked. All anyone wanted to diagnose me with is having bladder and kidney infections as well as a problem they want to leave. I am so incredibly happy I noticed this myself because I am not in a great situation now. I am going to have to have surgery. I am so scared. Watching you and the way you are so calm has made me feel more calm myself. You are a great doctor! Amazing woman! It is obvious that this is your passion. You have made me understand what the heck the drs are saying and that there are options. I need to start speaking out and being my own advocate. Thank you for the great video ❤️

I know this video is 4 years old but the information you’ve provided was absolutely essential to me. You’ve helped me not be so freaked out about what’s going on with me. I’m 53, had a hysterectomy at 42 where they left my tubes and ovaries because I had a pulmonary embolism when my youngest daughter was born in 2004. They didn’t want to put me into early menopause at 42 since they felt HRT was not an option for me. Now, I have a solid 5cm mass in the center/left side of my pelvis near my ovaries that was found incidentally during an ER and hospital stay due to a horrible UTI/kidney infection, sepsis, and kidney abscess. One radiologist thinks the mass may actually be a residual fibroid that is attached to my vaginal cuff area or maybe a broad ligament fibroid/mass. I’m scheduled for surgery to remove the mass in a couple weeks. I’ve been experiencing tailbone pain for about 5 months that I keep getting told is just aging. I’ve had no injuries or an accident to explain the pain. This week, I noticed a lump at the entrance of my vagina on the back/posterior. It’s worse later in the day or if I’ve been on my feet a long time. I’m curious if the solid mass near my ovary could be contributing to the lump/rectocele or if this is completely a separate issue. They sent me to a gyno oncologist for review and surgery on the solid mass, as a precautionary measure. I’m trying to figure out if I should let my surgeon know about the rectocele issue, before she takes out the mass or let it go and see if the solid mass being removed helps fix the issue. It wasn’t something I wasn’t even aware of during my initial consultation and physical exam with her. She likely didn’t see it during the exam because I was on my back. I don’t want extra surgery if I don’t need it, but I also don’t want two surgeries if I can avoid it.

All the way from Australia,I am sending my gratitude for this fantastically informative video. Along with your extensive knowledge comes your ability to explain things in layman's terms. Your warmth and humour to help us prolapse gals feel comforted and understood. After 5 big babies and twenty years of doing weights at the gym, I think I have given my pelvis a thrashing - hence, bladder and uterine prolapse!!!! I have confidence in my urologist but if I lived in the US I would definitely be your patient :D

I also do weights at the gym but I bought a hard foam wedge because weight machines are made for men. A woman should never have her body at more than a 90 degree angle from her legs when using a weight machine. I use the wedge to correct that - works a charm

Thank you! Started following you 6 mos ago after going to my obgyn and while up in stirrups she tells me I have prolapse and she will refer me to a surgeon for hysterectomy! I didn’t hear anything she said after that and realized when I got home that she hadn’t even done my Pap smear that I went in for. Dr. Just said it was normal with aging. I am 63 yr very healthy, active, LHS resistance training, clean eating, supplements etc. never expected this. Researched on internet for hours daily and now on estrogen cream, pelvic floor physio, estradiol gel, progesterone. Changed dr. Prolapse better, work in progress. Now Following you, dr Rachel Rubin, dr Mary Claire Haver, Dr Louise Newson, Dr V. Wright and others. Thank god for all that you do and social media

I just noticed a anterior prolapse today while I was at work (as a CMA) I knew immediately what I was feeling because of my previous obgyn experience, and it scared me to death. You're the first video I landed on after coming home, and you have helped calm me a bit. I'm guessing I'm a 3 or 4 so I'm somewhere between starting a keagle program and running to a surgeon. I began to suspect a prolapse last week when I took a „look“ due to an itching feeling. I was shocked…but then when you aren`t always looking nor feeling anything unusual, it could be that this is something normal. Anyway, found your video and I feel sooo much better, even if it is the beginning of a prolapse - also suspected because I have a chronic cough. Will talk to my Dr on my visit in April …..you made me feel like it wasn`t an emergency nor would I need invasive treatment. I`ve come away from this video very well informed. Thank you for all the good info!!!!

Thank you for posting this video. You answered my questions. I have had a pessary for seven years, which helped with my prolapse (at 3-4). But over the past two years, I have been experiencing bladder leakage with constipation. I have recently met with an OBGYN specialist and neuro surgeon to discuss bladder repair and a partial hysterectomy (remove the uterus) due to polyps and fibroids. A mass was found in my ultrasound. I feel constant pressure in my pelvic floor. I’m tired, bloated, and struggle to have bowel movements. Sex is not painful… but when I lay on my back, my uterus is enlarged and feels like a rock to touch. I don’t want mesh products in my body so I asked about the natural biological tissue to ligaments for the repairs. The sling was suggested but I have another meeting with both surgeons to pose my question about the natural biological tissue to use for the repair. I’m premenopausal and plan to keep both my ovaries for hormone balance. Do you recommend estrogen cream with keeping both ovaries after a partial hysterectomy? I know prolapse is your specialty, but perhaps you can speak to the benefits of the estrogen cream post- surgery. I’m not sure if I want to keep my cervix due to breakthrough bleeding down the road. I want to be completely done with periods and breakthrough bleeding. I want to be healthy and feel normal again. Thank you for your video and expertise in the medical field. I want to make a well informed decision over my body before consenting to surgery.

I had a vaginal prolapse operation coming up three years ago next month. I had it since the birth of my second child at age 33 doing too much exercise. It gradually got worse while I ran myself to death the following twenty years. It was the best thing I ever did and haven’t looked back since. I’m from England and it’s one thing I can thank the NHS for.

Did you transition from running as exercise to another type of exercise after you healed from the surgery? Or were you able to run again once the prolapse was fixed?

@@sandrajohnson2491 I’m thinking that genetics play a part. The consultant told me that most if his patients were overweight and had smokers coughs, which made their prolapse worse. My mum had a similar operation at the age of only 42 and she never ran in her life, wasn’t overweight and didn’t smoke 👍🏼

I’m scheduled for colpocleisis in April and I will be 82 in April. My prolapse is stage 4. I had a hysterectomy when I was younger. I did wear a pessary for years until it began to hurt me. I’m quite anxious about all this and wish I could still use a pessary. Was not using an estrogen cream so probably should have. I do understand better after hearing you and thank you so much.

My prolapse appeared last year and I was so scared... I am 73 years old, 3 children, hysterectomy...I saw my Gynecologist/ Specialist and was fitted with a peasantry to use if I wanted to. She explained that because of kids and hysterectomy that was what caused my problem. She said I looked fine and assured me that a lot of women actually had a prolapse but no surgery needed...& this this issue was hardly talked about.. I have no urinary or bowel movement problems.. Sometimes it seems to go away and sometimes I can see this pink buldg. (My bladder)I don’t panic anymore.... I don’t think I need surgery, but I did purchase a pelvic organ brace from a company called “BRACEABILITY”.. cost $60.00. I might wear it once every 2 weeks when I feel the bulge more.. Thank you this very informative video. I have a prolapse also. Haven’t had an appointment with a hospital Doctor yet. Our local nurse could see what the problem was, referred me and instantly gave me oestrogen cream. Immediate relief from the dryness. Your video answered all the questions I have wondered about. Thank you explaining this to us. X

I’ve had 3 children vaginally and at the end of last year had a hysterectomy. It was the perfect storm for my organs to begin to shift around. I’m grade one but I’m in some discomfort. Lower back pain, lower abdominal pain and some sexual dysfunction. I agree that my sexual dysfunction is more likely due to my preexisting pelvic floor issues (weak muscles, vulvodynia). I just started PT and am hoping I can prevent it from getting worse. Thank you for this information! I’m doing as much studying as I can.

Andrea, I just started a new job so I had to postpone PT until after I’m done with training. So far I just had my initial evaluation but the therapist was very knowledgeable and gave me hope. I work full time from home at a computer and have to take frequent breaks to stand or I end up with lower back pain, pelvic floor pain and lower abdominal pain. A sit/stand workstation would be ideal. I can keep you updated on progress with the PT in upcoming months if you’d like. Best of luck to you in finding some relief.

I'm on my third child now, how do you do it hahaha. I can only afford to feed two of them, thinking of sacrificing the third for a weeks feast. Anyways, any doctors you recommend? :)

​@@001joyo If you find the right pelvic floor physical therapist, they can save your life, or should I say, give you your life back. Mine is out of network (only way to get proper one on one time) Life saver!!! The way you can get a spasm.that causes pain in your back, you can get in your vagina too. They do PT inside, pushing gently to find any trigger points or spasms, any pain, gone. But read reviews. Not all are created equal. Some just have that "it" factor, like the doctor who was kind enough to make this video seems too as well:)

Thank you SO much. Just diagnosed with anterior prolapse. Your video was so much more helpful than my family doctor in terms of information and “next steps”. “Do your kegels, refer to a surgeon”. I will be seeing a pelvic physiotherapist next month (my idea 😊

Thank you Dr.Casperson for all your knowledge. I'm 63 with a rectocele but was informed last week a high one and a lower one. I was fitted for a pessary, I'm hurting from it 6 days now. I've tryed to take it out but no luck yet. So seeing a gynogolist next week. I'm not sure I can wait. Anyway I've decided not to have the surgery thus far. I think a smaller pessary might do the trick. Thank you again you certainly explained everything about prolaps.

How are you feeling now? 6 months after my 4th child I noticed my rectocele. Had a pessary a month ago. Ended up in emergency room after some crippling pain down there. Waiting for swelling to go down to see gynecologist. But not that the pessary is out, I definitely can tell it gave me a significant amount of relief with the heaviness in my pelvic area along with bowel movements.

I have a bladder prolapse diagnosed during early lockdown by the nurse at my surgery, I haven’t spoken to a doctor about it and I haven’t had my physio appointment yet, nearly 12 months later. This was a breath of fresh air. So informative and straightforward. My question is, at 55, how long can I use oestrogen cream for? It has relieved most of my symptoms but no one told me how long I can carry on with it for. I wish I could see you In person. Thank you for caring so much about a subject which is largely not spoken about.

I assume you’re speaking about topical vaginal estrogen and not systemic estrogen. Vaginal estrogen can and should be used for life. It is an extremely low dose estrogen. If you’re deficient in estrogen to the point that you have urovaginal atrophy, the atrophy will return if you stop the estrogen. If you’re talking about systemic estrogen, however, the answer varies! You would need to speak to a hormonal specialist about that.

This was awesome! I have had to have this done twice, no mesh. If I had all of this explained the way you did here, I don't think I would have had all this mess now. Now after six years it's partially back maybe a 2 as well as splinting and stress. Wish you had some charts to show what each thing looks like.

​​@@Brooklynn291I'm so sorry you're going through this. I had a pessary first but it was a 3 to 4. You are very young. Please get a few opinions if you're not at this stage or not severe, think about it. Have you looked at finding a PF physical therapist Did you have children?❤

So scary because I had no clue this stuff could happen , I’ve been worried sick , as the symptoms are getting annoying and I have really bad back pain , I just don’t want it to worsen , I am scheduled for PT so I’m hoping that helps and some healing time also , I appreciate your words ❤️

Thanks! You prompted me to see my doctor about this and I felt very empowered and informed. 😊. Had my bloods done and booked for an ultrasound next week, then I’ll be referred to the specialist and we will see what happens. With a possible 30 years left to live I think the surgery might be worthwhile.

I am 80 years old guite active and have prolapse surgery 17 years ago came back 6 weeks later am now using pessary not ideal but other than uti best I can do it has been the most information I have ever received on the subject thank you thank you so much

Why did no one tell me about this issue? I thought I was a freak. I gave the symptoms to multiple doctors. I said there was a pocket. I was finally told what a rectocele is in my mid 50s. Thank you for is video.

I am 82, had a hysterectomy at age 50,prolapsed uterus, eventually bladder hanging out, had the tie up for bladder, never stopped wearing pads, really not much bladder control ,but i dont want mesh ever,,,i am a mess....2 children not overweight, my mom had no problem.... just figure i will live with it,,,,,,,good blog !

Since 2011 I have been wearing a large cube pessary & V-Magic cream at night in place of Estrogen, it is all working great. I have full-prolapse. Thank You, you are doing a great work..

Thank you so much for this video. I’m 88 years old with a #3 prolapse.. I last saw a (male) urologist two years ago and he suggested surgery--. He spent all of five minutes with me and I felt very uncomfortable with him. My discomfort varies, at times I feel the prolapse and then will go for some time with no discomfort. I think at my age I would opt for the vagina to be closed. Thank you so much for all this information.

Hi, I actually had a complete prolapse of the uterus with my 8-week old fetus inside. The uterus fell outside of the vagina into the toilet, after an afternoon of carrying my 20 lb toddler. My Ob-Gynecologist wanted to do a hysterectomy, but I refused. I saw seven more Ob-Gyns who told me the same thing. Finally, I found one who was willing to try to save my pregnancy. I was on bed rest until my Ob-Gyn found an Ob in London, UK who had designed a collapsible O-ring which, when inserted in the pelvis, would prevent the uterus from falling out. I kept this O-ring in until I was 32-weeks. My baby came early. I had postpartum reconstructive surgery of my vagina and rectum. I still have a vaginal mesh.
