The incident where the 'Aussie Guy' fell to his death was due to him having his arms over the hooks. So, when he moved his arms underneath he must have accidentally unhooked himself. Personally, it was a bad design. You just need that one time out of 1000 to realize there is something wrong with the design. They were quick release hooks as opposed to locking carabiners that would have prevented his accidental release. There is a reason rock climbers use carabiners. Those morons just attached the quick release without giving the gentleman any instructions. He placed his trust in them thinking they new what they were doing.
