Dr. Ebraheim’s educational animated video describes fracture injury conditions and treatment associated with the pelvis.
Stability of the pelvis. Integrity of the posterior weight-bearing SI complex.Transfer of weight-bearing forces from the spine to the lower extremities.
The pelvis is a ring.
Anterior injury
•Anterior injury is obvious.
•Usually noted on radiographs
•Usually associated with posterior injury or fracture.
Posterior injury
•Could be occult
•Always look at the back of the pelvis.
•Posterior gap or comminution = unstable.
•Posterior fracture is serious.
•Needs significant blood transfusion.
•Need surgery
•Unpredictable outcome.
•Posterior fracture with disruption of the posterior ring complex is a serious injury as it leads to instability of the pelvis and can cause profuse bleeding.
•The resultant instability if not fixed surgically can cause late deformity, limb length discrepancy and pain.
Types of pelvic fractures
1-Anteroposterior compression (pubic rami fracture)
2-Anteroposterior compression (open book pelvis)
Treatment:
•“close the book”: pelvic binder and sling.
•Surgery if symphasis pubic diastasis is more than 2.5 cm.
•Anterior pelvic surgery such as external fixator or plates.
3-Lateral compression
•The most common type of fracture and is usually impacted and not displaced.
•Has good prognosis and usually does not require surgical fixation.
4-Vertical shear fracture
•Bad fracture
•Needs surgery anteriorly and posteriorly
•Needs blood transfusion
•Initial external fixation may be needed to control the bleeding.
•Traction in the ER may reduce vertical displacement
•Decreases motion at fracture site
•Pelvic binder may be helpful.
Vertical shear fractures are stabilized surgically by
1-Anteriorly internal fixation with plates. Reduction of the fracture before fixation.
2-Posteriorly: posterior screw fixation

It was a year ago today that I came out of surgery to fix my open book / vertical sheer fracture. Six screws, a plate and two bolts later I’m almost back to where I was before the accident. If it wasn’t for you and Doctors like you, I wouldn’t be here. Thank-you for all the people (including me) that you have helped!

I am just watching this because I fell onto my hip, and I have some pain in my pelvis with walking, though I don’t think I fractured it. I saw your comment and just wanted to encourage you for all your hard work! My best friend was in a motorcycle accident several years ago, going about 40 mph when a deer leaped into them from the side. Her pelvis was fractured in three places and her femur broken. It was several years back and she is good now, but what a terrible injury! Glad you are better now.

Wonderful video.....very detailed....my cousin was in a motorcycle accident today and he broke both arms, pelvic bone and had internal bleeding...not to mention most of his skin was completely skinned off...your video explained clearly the type of fracture he got...thanks

Pubic symphisis here, fixed with APC 1 ORIF symphysis plate. I better understand the operation now, thank you.

They can dance again. But it will take months depends to the fracture. They have to learn walk first.

it will be painful though if it's bad fracture even after fully recovering, don't ask me how i know? it's my personal experience 😞

I am recovering from pelvis break. The most difficult thing is bowel elimination. I keep it soft but can't pass stools except repeatedly enter a finger then wipe it clean until feelings of bloating stop. Do not suffer a broken pelvis!

I had surgery to fuse left SI joint. New experimental procedure. Surgeon tried to fuse the joint thru the back. He broke apart the si joint area. He focused on blood loss. He packed the area he broke in half using pig bone. He didn’t know he’d broken my pubic bone in half. Is this a vertical sheer fracture? He didn’t repair the pubic bone. On X-ray one side is higher than the other. X-Ray shows the bottom corner of higher side touching the upper corner of the lower side. The surgeon never changed my chart. He sent me home after 2 days. One metal bar with 2 screws he designed “for an average size man” held my sacral area together. He told me & my x husband I’d walk out of the hospital. At home i saw my left buttock & thigh was black (from blood loss?) at 6 week post op i asked him why my groin & inner thighs hurt so much. His response “you have a dislocated pubic bone, have it fixed during PT visits” i repeated his lie 2007-2014. 2012 i had a pain pump put into my abdomen & it delivered dilaudid into my spinal fluid. 2014 i brought my x-Ray to pain doc & repeated the lie. He said, “there’s no such thing as a dislocated pubic bone!” He put my X-ray on the window. “Omg, you’ve been walking on a broken pelvis for five years!” I asked him what do i do. He knew my xhusband was abusive, Doc said, “If you were my wife I’d send you to a trauma surgeon at Tulane, Johns Hopkins or Vanderbilt” at Vanderbilt surgeon specialized in pelvic injury. He said “I’ve never seen a patient with a pelvic like yours walk into my clinic” i knew it was 50% mortality rate to break it apart. He sent me to have a CT. Day of surgery he asked if he can open original side due to an anomaly. Woke up 4 days later. He found osteomyelitis. He removed dead & dying bone, flooded me with antibiotics & i went into renal failure. Bed rest 4 moths. Two screws to hold left leg onto body. Picc line from bicep to heart for 30 days. Pain doc said I’d die suddenly & to tell family etc..he refused to tell me how i would die. That was 7yrs ago. Now my right SI joint is failing. Incredible pain again but new leg. I cannot have it repaired. I look normal. I can walk & sit an hour or 2 but need a week of rest to recover. I’m so tired of making family & friends understand. What will make them stop making me feel guilty for not wanting to socialize?

So I got in a fight with 3 guys and one slammed on on the floor real hard I went to the hospital because I was limping and my rite thigh hurt a little well somewhat a lot they said that they had to do surgery rite away but I was scared and never got it , and now two years later my rite thigh feels like it’s on fire and it’s numb and today at work I tried to walk fast and it was a weird pain like my leg was pulling out what is going on doctor ?

I was ran over by a car and left with two titanium pins on the right side and a titanium rod that went from my back left side to my right side. The rod was removed a few years later but they won't remove the pins on the lower right inside of my pelvis that cause more discomfort than the 4inch rod through my back side did. They won't seem to return my calls on answering my question of why can't they be removed...

so when my wife gave birth she fractured her pubic symphysis, doctor said the ligaments came apart in between. she had surgery, a plate on that area with a piece of her pelvic bone placed in-between the pubic and 3 bolts in her back. my question is, its been almost 6 years since the Operation, will she be able to remove those bolts in her back and maybe the plating on the pubic symphysis without major issues? i would love if someone can answer this please.

this is sooo f up..i'm sorry if u r going thru it now..couple things that may help u; have a back scratcher - u won't be able to bend for couple months! have a bag- if u drop, say a paper, it'll be nightmare picking it up. u may have to lay on your back for couple months...sorry for your pain..perhaps u can escape the pain/bordom by learning a language!

I had motorcycle accident and open book fracture. total of 6 breaks when including the public bones which is still not connected but had external fixate for 12 weeks. having pain now in rt. side down middle of rt butt cheek. gained 40 lbs and want to learn sum exercises to that I can tolerate to lose weight. accident was March 2015....so I'm good as I'm going to get wanna lose weight. anyone got suggestions would like 2 hear them thanks

I was hit by a car and had to have an external fixater for 6 weeks. 12 weeks must have been tell for you. I lost a leg as well and yesterday I tried running on my prosthetic and now feel discomfort pain in the front of my pelvis. It was 4 months since my accident And suggestions to lose weight I guess. Just stop eating so much. Get a plate and only have 1 helping. Only eat once a day.

Assalamualaikum Dr ... i just got an bike accident... n the doc say my pelvic i broke... until now i hard to get up... my question... why the doctors say i didnt need any surgery ... this is already 11 day after the tragedy’s

I also didn't need surgery, but if my pubic ramus fracture and sacroiliac joint had been shifted for a 5mm more, I would have been operated. It depends on the situation. Now I'm recovering okay and the bones are healing in the right way. For a month I was forbidden to move, just have been laying down on my back, so don't worry. It's been almost two months after my car accident. Sending my regards, get well soon.

@@nazniel31 If you're asking me, pretty much okay. No painkillers in general. Been doing therapy at home, still on the crutches, but can walk on my own if needed (at short distance), like while I'm cooking or smth like that. Thank you for your concern. I would like to know how Ariff is doing...
