True, it is very pleasing visually, but if we're talking comic book panel, we definitely need to leave more room to the letterer, which often isn't compatible with that composition 😅

While there may be a large figure or object placed in one portion of the frame, text boxes and bubbles can overlap low interest areas in the bigger object, work around the layout and even sometimes add to the flow and composition of the panel and page. Newer artists and letterers can step on each others' toes or not know how to work around an inconsiderate collaboration partner but the rule of thirds doesn't automatically make it unworkable for a letterer. You can see it used in just about every issue of comics you read.

Do you just instinctively draw the concept figures or follow a certain pattern like some sort of gesture?

The figures in the scene will be dictated by the intention or narrative of the scene. This "rule" just gives us a guide on where to place them in frame.

Can this be combined with foreshortening where the "bigger" figure is for example an extended limb of the "smaller" character figure further back?

The rule of thirds has been debated and disproven as a useful rule by many... I think placing subject on a third is not a bad idea, but placing something else on a third to balance it just makes it boring. It's better to be aware of the spacing between objects and vary them according to big, medium, small rule.
