Some students are mixing up the names of internal pudendal arteries and pudendal nerves, so let's have a run through these structures and see where they are and what they do.

I injured the pudendal nerve mtn biking. Like he said, the seat is a cause. It’s not a small type of injury-- it’s devastating. Unlike any pain you’ll experience. When seated, you feel like there is a hard, sharp rock under your bum (or in it !). There is also a constant burning / binding sensation in the fold between the leg and pelvis- like wearing underwear 5 sizes too small. I’m now 4 years into this. Apparently, you can damage this nerve by cycling, squatting weight, falling, or by childbirth. Damage can also be a complication of surgery (prostate or hernia). Injuring this nerve does not always mean numbness, it may mean hell on earth due to unremitting pain.

@@alexriley6276 Thank you for your reply. I will research him. I’ve gotten decent results from Gabapentin and Duloxetine….and time, I suppose. No loss of function, thank goodness. Just pain and loss of my passion- biking. Decompression surgery might be my last resort as the risk:reward ratio is poor. I think pudendal nerve blocks , infrared heat therapy, and perhaps stem cell therapy are my next moves. How did you sustain your injury?

10 years ago I felt a little bit of pain in the premium ( The pain Seemed so insignificant at the time that I can actually remember for certain where it was located) after getting up from the wrestling mat, and that night I lost almost all feeling in my genitals and my nipples. The next morning I had pain in my feet which has yet to subside.I can get an erection currently, but I still have loss of feeling and often pain during ejaculation. I am seeing a pelvic specialist who did her first internal exam recently.None of what she did actually hurt. I’ve seen several neurologist and had my spine fused at L5, S1 eight years ago. I haven’t given up on my Johnson yet though.

ZOMG, Sam. Because of my last “doctor.” a D.O., I had to cram your videos in order to get a leg up on her. That is, because my ALMOST COMPLETE LACK of medical knowledge, my last “doctor” had a HUGE leg up on me and was whipping me around with skills. Thanks in part to your STUPENDOUS videos (in particular the ones on the inguinal canal on both the male and female pelvises) I was finally able to even the score and wrap her around the flag pole. She quickly DUMPED me from her practice. So now I’m with a new doctor, an M.D. who’s TERRIFIC. Whereas I had thought the first “doctor” was very smart, this new doctor puts her to shame. What I’m saying is that along with a lot of other STUPENDOUS med school type videos, I had to turn all of you here on KZfaq as a method of survival against a predatory “doctor.” Now that I was able vanquish this evil spirit, I could finally turn my attention back to my _actual_ vocation which as nothing to do with the medical field. So, in some ways, I feel like you’re a true and trusted friend and I missed taking in your STUPENDOUS content. Thank you, Sam. 👍 🙏 Oh, and by the way, my dad is from Cardiff, so you just scored EVEN MORE points! 🏴󠁧󠁢󠁷󠁬󠁳󠁿 👍

Would you say for cyclists the narrower, firmer bike seats are actually better as the weight is more on the ischial tuberosities and hence less soft tissue compression?
@seymourtompkins
@seymourtompkins 4 жыл бұрын
i injured the PN on my bike seat. From experience, I'd say it's critical to support the ichial tuberosities (but avoid too hard or too soft and also avoid too narrow or too wide). Also, remove all pressure from the perineum. However, if you have problems in the so-called kinetic chain, you'll develop a problem anyway, sooner or later. So many things can affect the PN too (like childbirth, constipation, etc). It's a whole subject in itself, as I've learned.
@rashad1saifullah
@rashad1saifullah 4 жыл бұрын
@@seymourtompkins Thanks! Hope you recover well. I was planning to angle the seat downwards so pressure is off the perineum and sort of just sit with the ischial tuberosities on the back part of the seat. what do u think about this ? www.amazon.ca/gp/product/B016KI2QLG/ref=ppx_yo_dt_b_search_asin_title?ie=UTF8&psc=1
@seymourtompkins
@seymourtompkins 4 жыл бұрын
@@rashad1saifullah There really are no guarantees with this. No magical seat. I actually got injured upon switching to the "perfect' seat (it was supposed to be safer). Yep, i had angled the nose downward too. The issue is that the seat is only part of the problem. The answer is to pay close attention to any odd sensations in your pelvis, hip, or inner thigh; if these occur while riding, stop riding. You can't 'muscle through'. P.N. pain feels like you're sitting on a very hard, sharp rock (or lego piece) 24/7. Its quite scary.

@@seymourtompkins Thanks alot. Im just wondering how those marathon cyclists do it. If I stop riding and take lets say a 5min break everytime I begin to feel some discomfort starting do you think any damage will be done?
@seymourtompkins
@seymourtompkins 4 жыл бұрын
@@rashad1saifullah I'd say the marathon riders are more at risk. The term Cyclist Syndrome actually comes from cases of distance road riding ! Ive heard of fewer mtn bikers who get it. Watch out for numbness in your genitals while doing either sport.. Bad sign.

Hello Sam I have a question you may be able to answer. With the dorsal arteries in the penis that run parallel to the right and left of the dorsal vein and they branch outwards towards the penile head. My question is that I can feel these arteries with my fingers they're very hard like cords/strings more prominent during erection. Should these dorsal arteries (not deep ones) be easily felt by touch under the skin of the penis as hard corded structures or should they be soft that you shouldn't be able to feel them besides a pulse? Thanks your answer would be helpful 🙂
