This is incredibly useful. I'm so mad at myself not finding this video before. A lot of women get interstitial cystitis as their diagnosis and it is not. It finally is pudendal neuralgia. I've been coming and going with this awful condition for 13 years before I got this was the problem. No doctor could make the right diagnosis in 13 years, gosh. BTW I'm a medical doctor too, I have a lot of contacts and even though with that advantage, none of the specialists thought about this.

I have this problem. I first started having the symptoms after having a hysterectomy. I already had scar tissue from having a traumatic childbirth in 1988 with subsequent fistula repair. I feel like I have more problems post hysterectomy than I did before. About a year ago, a new gynecologist had me get a pelvic MRI. It was discovered that I have a Tarlov cyst on the S2 nerve plexus. Many doctors don’t even know what those are, and sometimes radiologists don’t report them. Those cysts can also cause pudendal and pelvic pain depending on the location. It’s a miserable situation with not a lot of treatments!

I have Tarlov cysts and I am an MRITech I try to do additional images through the sacrum when I see them and get asked why I did extra images by the radiologist. Some don’t even mention them in the reports. I want to say, because I have them and they hurt like heck!! Now they’re thinking I have PN. I basically diagnosed myself from researching my symptoms. Now referred to a specialist.

I hurt my lower back lifting case of water, pain and radiated to front thigh with burning and then much numbness in groin area. I do have a hernia there and afraid it will burst but after 3 days no results with mri of pelvic and hip but will that show lower back too?

My wife is having severe shooting pains in her right upper inside thigh and genital area. She recently had her left iliac vein stented for may Thurner syndrome and pudendal nerve started about 3 weeks after surgery.....is there a common correlation with this surgery and issues with pudendal? Dr. Hauser, why don't you use prolozone instead of prolotherapy. My doctor David Palacio in Portland Oregon at lake oswego says it covers more area and you would need less shots. So l am curious about what you think. I have had this pelvic pain for 17 years and finally lost my career education job 34 years. I have been to Mayo clinic around 2005, Virginia Mason several times receiving a ganglion impar nerve block which worked a couple times for a year or 6 months. Also to Jason Attaman a pain doctor who gave me Pulse radiofrequency, Botox, and PRP. This worked a couple times but 3 times it did not and at $8,500.00 per session l feel l was taken advantage of when I was and still am at my lowest point. Very expensive when done 5 or 6 times. I don't want to think about it. Please give me your thoughts about the differences between prolotherapy and prolozone. I am going back for my 5th round of injections. Dr. Palacio really seems to care about his patients. I just want to get well. It has been a long hard road. I hope this time will do it.🙏

Thank you for the wonderful information. Can SI joint dysfunction cause pudendal nerve pain from the pain radiating to groin or would that rather be a piriformis muscle type pain issue because patient has Ankylosing Spondylitis and bilateral SI joint dysfunction with pain radiating to groin. Also has 3 herniated lumbar discs. No radiating pain down the leg so sciatica can be safely ruled out. Would really appreciate your medical advice on this matter. Thank you so much 🙏🏼

This was very informative. I actually have another joint condition called arthrogryposis. For me my left knee will not straighten so I put more pressure on my right side of my body. Booked in later this week to have a steroid injection on my left side in vagina

I don't see how his explanation gels with the surgeons who say that the ligaments are forming a sling like the nerve goes through them. They cut the ligaments and pain is gone. He suggests prolotherapy which might tighten the ligaments but would that not make it worse? Also, if you have SI Joint fusion, then wouldn't it make it better? Not in my case it didn't make it better.

I pulled my obturator internus in 2018 and it caused pudendal nerve pain that I don't even think I can describe without getting flagged by KZfaq. Took years to start getting better. Pelvic floor PT and pudendal nerve block injections was the treatment

I had surgery 2020 and didn’t help. Still have the pain at my tip Trying. This. Wave therapy. Now. Been two weeks now. Can you tell me why this pain is so Bad when I lay in bed on my stomach. When I get up it goes away and feels better after I go urinate.

Hi, Dr. H. Thank you especially for your video on this topic and education on I instability in your neck and pudenal nerve. In 2014. I hurt my left pne in the gym performing a stand up reverse hack squat with heavy weights. I was bed ridden for 4 years. Like you mentioned PT can help tremendously but it does not resolve the issue. I had the most supportive PT. But, I struggled with regular doctors and pain management clinics wanted to just give me steroids shots that only worked for that day. I finally found that daily cryotherapy and local cryo for flair in 2019 and am was able to get 70 percent better.. Could this affect my Vagus nerve left side one? I also believe I have cervical instability that effects my nerve. I just started to walk 5 miles and have flaired my pne from fast walking hit training. I am an exercise physiologist so when I had to stop exercising 4 years. It was devastating to me because exercise was my whole world. I have been diagnosed with migraines, inflammatory arthritis because of these symptoms with my stomach. I started to get burned vision, double vision, dizziness, stomach inflammatory arthritis. Feels like I have methane sibo. My jaw, migraines, eye pain, ear pain fullness, nausea, gad burning if the skin. Burning of tge skin when I sweat. I have been using the infated sauna. I ueve I have cervical and pelvic instability. Not sure if it's the left cervical with my symptoms. Cryotherapy works along with using vagus nerve techniques. Excitement ringing in the ears and whe. I feel flared really exhausted or muss cryotherapy for a day. I can't function. Chronic stress since 2014 4 due to pne. Now I have developed rashes on my skin, scalp. The blurry vision does come and go with inflammatory arthritis. The skin burning started this year with tiny red rashes. Cryotherapy makes me be able to feellike myself. Feels like a bullet whole went thru the one as you described. I just want to exercise but these symptoms are hard to come with. I feel tgat burning of the nerve similar to my skin at the left sidled pne. Really extreme insomnia with methane sibo and now left pne. Ice is my friend and cold therapy. I love to explore therapies. Wrmt thru a horrific divorce and have noticed more flaring. In your expert opinion coukd these 2 instability be causing both left pne, neck pain, stiffness, inflammatory arthritis, migraines. I went to cryotherapy but I tweaked it by pushing myself to walk 5 miles a day recently. My neck has been injured since a whiplash injury at 13 in a car accident? Look into "auto-inflammatory" disorders or MCAS (mast cell activation syndrome). I an in the process of possible diagnosis with MCAS and much of what you listed, I'm experiencing. . GI symptoms, migraines, cycles or flare-ups, rash, splotchy skin, flushing, sweating, hot/swollen joints and currently dealing with failed SI joint fusion, pudendal nerve pain, and sciatic pain.

Hi John, Thanks for your question. We would look one step further and want to look at what is causing the herniated disc. The disc herniation is an effect vs a cause and is related to ligament laxity/ spinal instability which can be associated with pudendal nerve pain and pelvic floor problems as well. They may all be connected and, our office, we determine this through a physical exam and ultrasound of the tissue as well as a medical history review, and then create an individualized treatment plan to help restore the ligament strength which we do with Prolotherapy. Hi Dr Hauser, thank you so much for the explaination. I’ve had penis pains on the right side of the penis glans. Was diagnosed as balanitis by a dermatologist because some of the symptoms. Went to urologist and he wondered why I’m using all these creams and said just leave it. Now this has brought to my attention I have pain down the shaft of the penis and have had perineal pains in the past. I’ve recently been diagnosed with ankylosing spondylitis. Could my problem be both balanitis and pudendal neuralgia, or it’s caused by ankylosing spondylitis? Your help would be much appreciated, been trying to get to the bottom of it. Thank you.

It is common for urologists to start you with antibiotics. Many people who had this problem were initially misdiagnosed with prostatitis. Eventually, the antibiotics prove useless and the urologists will try rounds after rounds of it, believing it will finally work even though all the blood work and test show no sign of prostatitis. This is a common tale. There are reddits groups you can join that talked about pudendal neuralgia. Everyone's treatments varies. Some tried PT and it worked. Some, it gets worse. It varies so much with different people. Stretches could make it worse too. So it all depends. A retired doctor said 1) not sit at all to rest the nerve and let it repair 2) take benadryl, it has an ingredient that is similar to prescription meds that helps relax the nerve (I personally think could replace with quercetin), usually some percentage of people heal months later after these two. 3) pudendal nerve block every month for 4 consecutive months, another percentage of people will heal. I suggests changing diet, no artificial sugar, milk, cheese, beef, pork, processed food, moonshade vegetables like potatoes and tomatoes, egg plants... because these food increase inflammation, which increase pain. I'm not a doctor but I hope this help. This is a horrible pain to have.

how can you diffrence between perineal tendinitis and pruduniel neuralgia? i have L5-S1 with similar symptoms with footdrop and calf muscle atrophy imbalance walking so how can u diagnose correctly

I was in the hospital for almost 2 years from an autoimmune disease and ended up getting a coccyx wound which then I needed to get surgery on it. it was like the size of a baseball and about 4 inches deep. I really hope they didn't damage any of my sacral nerves, especially my pudendal nerves.

They removed my coccyx and surgeon did not say any nerves are harmed despite all these nerves in the area. I just think small nerves that are cutaneous can easily get cut without any issues. If they are not just sensory nerves which overlap from many places, then you usually have no issue with feeling. If it is motor nerve, then you have a real problem.

@@fdimichele I prayyy to God no motor nerves were damaged. Because right now my legs are paralyzed from my autoimmune disease, so yeah damaging my motor nerves would not be good. I did ask my surgeon if any important nerves were damaged and he said no.

I have had PUDENDAL NERVE ENTRAPMENT FOR 8 yrs, WHEN IT FIRST OCCURED AFTER HAVING BACK SUGERY TV AD SURGEONS, I truely didnt think I WOULD SURVIVE 1 WEEK, I live in contant severe vibration, severe burning, it could all end in less then 3 seconds, but I PREY SOMEONE SOMEWHERE WOULD HELP, BETWEEN PAIN MEDS, HIGH DOSES OF LYRICA AND STILL THE OLDER I GET,THE WORSE IT IS CONTROLLING MY EVERY THOUGHT.

This is the reason for CPPS also. I am pretty sure it has something to do with Pudental Nerve irritation.

The reason it falls under CRPS is that about 5 peripheral nerve disorders are associated with Pudendal Neuralgia. If you luckily just had one of those five, then you should just treat that condition and not implant simulators that need to be removed before MRI which could be needed for other problems.

I’ve been told I have an extra bone that has to do with the sacrum and I deal with intense pain daily that irritates it with certain stretches mainly left side.. I have cystecele and rectocele stage 2.. I have been seeing a pvpt for 6 months and feel like I’m much stronger but my pain and prolapse is still very apparent.

Hi Kristina, while the prolapse is not something Prolotherapy treats specifically, there are cases where the pain is not related to that but is related to the sacroiliac joint or another joint instability and causing a radiating pain. Known joint instability is more likely a Prolotherapy case, tho we cannot tell you if you would be a good candidate for our clinics without reviewing your case in more detail, including your MRI report. @@CaringmedicalProlotherapy I've been dealing left sided PN for 18 months after a fall to my bottom. I've had blocks, cryonuerolysis and PT. My PT and I have honed into my sacrotuberous ligament. It is so so tight and painful and won't release. We do releases but it doesn't last. Any suggestions?

I am so glad I found your video! I hope you read this and give your opinion pleasee. And I am sorry for my English. Is it possible that at my 19 years old I can have this? A few days ago something started to happen to me, a symptom that I think it has to do with this. And I think I've been with that for years, but I never paid much attention to it until this year when I started to feel that this had to do with this. For example, when I was little, in elementary school it was when I realized that I could not sit a certain way because of my left side of my gluteus, as if after a few minutes it felt as if my nerves there or ligaments began to tire, I don't know how to explain it, but it not a comfortable feeling and is a bit painful if I sit for a long time. Then later this year, I tried to be more flexible, but my left leg cant, I feel the same pain as when I sit a certain way. But it got worse, I realize that when I throw my head back, like when the girls take their heads out of the water to throw their hair back hahaha. I feel a sharp feeling inside my belly, almost near the entrance of the vagina... it is very confusing, and disturbing. Days ago I got up because the left side of my vulva began to feel too sensitive and really painful. Its really confusing and scary. I would not like it to be anything serious, and for my doctor to mistake it for something else, that I will never be able to heal well. Or be treated the right way.

hi doctor Hauser for more than 7 years I have obstructed defecation, I don't have pain or anything related to it, after seen several specialists and done multiple studies they concluded is pelvic floor dysfunction, physical therapy is doing little to help my problem, at this point do you recommend the botox injection or trigger point injection? Hi S Seif, Ligaments are the tissues that hold our joints together throughout our body. Joint instability due to ligament laxity/damage can cause nerve compression because the bones are able to move too much and press on nerves (among other joint structures). Treating the ligaments to stimulate them to tighten and stabilize the joint motion pulls pressure off the nerve so the nerve has the normal space it needs. Prolotherapy is the regenerative treatment of ligaments.

I had a bladder lift in 2010 they used mesh had severe pain immediately. I had all of the mesh removed but the anchors the doctor wouldn't remove them. Still suffering from severe pain it's coming from my right side on the right pelvic bone I've told the doctor over and over he keeps saying it's a pelvic floor problem i went to Pelvic floor therapy and it didn't help she's the one that told me it wasy pudendal nerve. After going to several more doctors taking more pain meds nerve blocks ablations etc finally i tried a Medtronic nerve stimulator it gave me 100 percent relief but since he couldn't suture the leads to my sacrum they moved and it quit working. After he kept trying different settings there was nothing else he could do. I was introduced to another pain doctor he tried to revise the leads when that didn't work he installed a Medtronic pain pump inside me which gives me some relief. I'm better than I was but I still can't sit. I stand all day which has cause problems in my legs and feet now. My pain level stays at a 5 it's all vaginal and rectal constant pain. No matter if i lay stand sit etc i can't get relief. I have gone to every kind of doctor. I have come to realize that I'm going to have to live like this for the rest of my life . I'm fixing to turn 62 I've lost a lot of my life already. If i could just find something to make the pain go away for a few hours a day i would be so grateful. I want to be able to enjoy what life i have to be able to do things with my grandkids that I've already missed. I don't know what to do anymore. I don't have any support at home.

so sorry to hear. get one of the medical medium books by anthony william. they are about true causes of mystery chronic illnesses. the nerve pain could be viral. strains of a shingles virus like to get into the sciatica, pudendal, tibial nerves etc. you’ll actually get answers that make sense unlike the doctor merry go round and their guessing games. good luck.

I watched your video 5 times to understand all the terminology; i actually think i've been dealing with this issue since 2016 when i had my 2nd car accident. Ever since then, my right leg is just not right; there is pain in the lower back on the right side; my right leg is becoming weaker and weaker and harder to control; and i often lose balance. There are a couple of other physiological symptoms I have in a pelvic area that indicate an issue with this pudendal nerve, so i'm going to take this info and take it to my doctor. However, what exercises do you suggest? I just did one exercise that is recommended, it brought me quite a bit of relief; so if you know of any other exercises, please include them with your videos.

I would see a surgeon not for surgery but the pelvic floor surgeons would be able to guide your care better. I even saw pelvic floor therapist and didn't help me. Luckily, many ob/gyn now cover pudendal nerve for women to get care. Men, we are out of luck as most urologists who should cover that area are not trained in how to treat pudendal nerve pain.

I was bed ridden for like 8 months. Didnt walk normal for a year and a half. Ive had every test in the book. All negative. I feel like my lower anterior abdominal muscles could be weak, i have a belly and am 250 lbs. My pain in one sided. Do you think my pelvic floor muscles could be compensating for weak abdominal muscles from being inactive last year. It just seems to be getting gradually worse. Month by month and over 2 years and some change.

I have suffered from Chronic pelvic pain for 3 years now. I have tried every type of PT, stretching, internal (rectally) trigger point massage, every medication know to man for nerve pain, and nothing does anything. The pain in the worst when I lie down or any activity that “stretches” my body, like swimming freestyle is one of the worse. I know I already have cervical instability, could the same thing be causing my pelvic nerve pain??

I have severe pain in my perineal. Its a burning pain and inflammation pain. I hurts after ejaculation and bowel movements. sometimes I feel like I have a swollen burning big golf ball. i feel like my spine is compress and is causing all kind of pains but most of the pains is burning and this happens also in my upper abdomen area. From two months I deal with intense pelvic burning sensation, that is moving from urethra, rectum and vulva, sometimes tailbone. I’m pretty healthy, slim and with good diet. One doctor said that it might be pelvic nevralgia, the other said might be Tarlov cyst. It is weird that there is cyclical behaviour of the burning - the burning moves, the intensity changes, and every 5 days there is a day that I feel quite normal. Biggest burning and tension is when squating. What could this be? It came suddenly after a year with frequent cystitis, but by urinaty tract is free of any bacteria and it definitely is a nerve burning. Also, I deal now with a loss of a loved person, which definitely make things worse 😔

I spoke with my PCP a few years ago about not being able to feel much pleasure during, well....you know. Everything functions just fine, but no feeling. I feel this may be tied in to lower back issues I've had since I hurt it jumping and twisting not long prior to seeing a doctor. He told me that its probably in my head, but this is persistent. Hi Wars7498, Thanks for the question. Yes, there are other symptoms that are common from joint instability other than just pain, including numbness/numbiness, burning sensation, tingling, are among common ones we see.

I’ve been experiencing tingling and itching in my genitals,penis and sometimes a burning and pins and needles feeling. Not much pelvic pain. I’ve been tested for all stds and I’m negative. Could this be pudendal neuralgia.

Yes. That was me for 15 years. Although mines I believe was triggered by a stubborn yeast infection. Once the infection was treated the nerve pain got real bad. Itching, burning, numbness…

Hello doctor. Has pundendal nerve compression ever been confused with prostate problems? I’ve been having all the symptoms of pundendal issues but am being treated for prostatitis even though I have no bacteria in my tests. Thank you in advance.

Yes, it is a common thing urologists will give you, antibiotics. Many people who had this problem were initially misdiagnosed with prostatitis. Eventually, the antibiotics prove useless and the urologists will try rounds after rounds of it, believing it will finally work even though all the blood work and test show no sign of prostatitis. This is a common tale. There are reddits groups you can join that talked about pudendal neuralgia. Everyone's treatments varies. Some tried PT and it worked. Some, it gets worse. It varies so much with different people. Stretches could make it worse too. So it all depends. A retired doctor said 1) not sit at all to rest the nerve and let it repair 2) take benadryl, it has an ingredient that is similar to prescription meds that helps relax the nerve (I personally think could replace with quercetin), usually some percentage of people heal months later after these two. 3) pudendal nerve block every month for 4 consecutive months, another percentage of people will heal. I suggests changing diet, no artificial sugar, milk, cheese, beef, pork, processed food, moonshade vegetables like potatoes and tomatoes, egg plants... because these food increase inflammation, which increase pain. I'm not a doctor but I hope this help. This is a horrible pain to have.

Stop taking those antibiotics it's not an infection it's neuro. Most urologists are not trained enough to know about pudendal neuralgia. Prostatitis is a general pool of could be a 1,000 things you need pudendal nerve decompression

Hi John, Thank you. Glad it was helpful. The ligament laxity of any surrounding joint can cause the bones to shift and compress nerves and close off or reduce the space of nerve pathways. The actual joint instability/ligament problem does not need to be exactly where the person feels pain. There are referral pain patterns through the body. Wish you all the best in health!

Emg with needle electrode with sphincter ani externus on the left normal resting activity clear activity increase during crossing and coughing activity search in connection with crossing and coughing but also moderate activity increase in connection with crossing Normal emg images with sphincter ani externus on the right side normal resting tone activity normsl activity increase during pinching and reflex activation as well as slightly moderate activity increase even when crossing normal emg images Assessment examination shows signs of n pudendus sksds bilaterally of a relatively pronounced degree and most likely of a predominantly demyelinated nature, substantially lateral Emg showed essentially normal images in pudendus innervated muscles. However, it may be seen as a secondary manifestation. A tendency to paradoxical sphincter reaction at crossing. However, activity increase in the external sphincters during crossing does not exceed 50% of activity increase during pinching No effect on thin nerve fibers perianally or in the anal canal In addition to this, irregular and occasionally slowing function of internal sphincters is somewhat more distinctly left side. The examination of internal sphincters is, however, very difficult due to increased tension in external sphincters. Examination shows signs of n pudendus bilateral injury of a relatively pronounced degree and most likely predominantly demyelinating character substantially lateral

Ok so my my nerves that connect from my meat to the back side hurts causes penis shaft tenderness, groin pain, and i know its back tok because whenever i make my tailbone movement, it hurts. at first it was only like when i quickly standup using my pendualdmcniencje whatever it hurt so i tried a bit of exersises i found from internet it kinda calm down but i think later it got worse because i sat all day due to covid 19 and schools aint open

He's saying that because the Pudendal nerves travel through already right places to get from your spine to the inside of your body that some people's bone connective tissues are not as strong as they should be which causes the bones to pinch the ligaments where the nerve travels. He sort of drops it there and now those of us that have no way in hell of ever visiting this doctor are left up in the air with no hope.

try soil. put your bare feet in soil for atleast 30 min or walk for 30 min, it will restore your pelvic muscles back to normal and also fasten healing. it will the best if you put your feet atleast 2 feet in soil. it will heal you completely for sure
