This 3rd Part of the video by Aspire 32 is on Cavity Liner, a part of an ongoing series for dental students' with an aim to help them understand the various situation for application of Dental Base, Dental Varnish and Dental Liner. This video concentrates on a specific clinical situation where Cavity liner is indicated, It also discusses the role of calcium hydroxide as a cavity liner. Pulp Protection is an important topic from Endodontics and Operative Dentistry. We have to extend the cavity atleast 0.2mm in Dentin so it will be complete thickness of enamel i.e 3 ..hence answer is 3.2 And vanish only will work for the amalgum restoration as the rdt is sufficient

Why should the cavity be extended 0.2mm into the dentin? Why can't we just make a cavity of 1.5mm in to the enamel till where the caries extends? Please reply

Sir... In deeper cavities having RDT less than 0.5 varnish doesn't provide full protection to the pulp from the components leaching out from base?? I mean if we apply varnish in such situation couldn't it seal the dentinal tubules fully??

first of all i really need to thank you for these videos they are really helpful and useful for us interns answering your question ~ I would extend the cavity to 2mm and hence its on enamel i would still use varnish with no base hope you answer

It will be 3.2 mm since amalgam floor has to be in dentin.

Depth of the cavity preparation will be 2.5 mm for better mechanical retention of the restorative material , Then varnish will be applied for pulp protection and also prevent the corrosion products (discoloration of tooth)which is released from the amalgam.

Sir a cavity prepation with a depth of 1.5-2mm and since still intact enamel would be left i would prefer goin with varnish esp to protect dentin from the corrosive prdts of amalgam.

dear doctor suresh , for a shallow amalgam that i cant use a base under and i have to use varnish under it , lets say that i dont have varnish then can i use etch and bond ? ( only on dentin just to prevent post op sensitivity )

Yes. The latest guidelines say one should use bonding agent instead of varnish

For amalgam we will take support of dentin, so our cavity will extend 0.2 mm in dentin and then varnish is applied and restoration is done

Removing caries, cavity should be prepared. It's depth may be 1.5 to 2 mm. If composite restoration is done no need of pulp protection. If amalgam restoration is done, cavity varnish should be applied.

Sir as the cavity preparation depth for amalgam restoration is 0.2mm from DEJ to PULP so here the enamel depth is 3mm so the required depth for amalgam restoration will be 3mm of enamel+0.2mm from DEJ to PULP=3.2mm And the second part that is required pulp protection wud be only varnish .

Deep of the cavity will be around 2mm to reach 0.5mm into the dentine this is because dentine is a resilient material it can withstand amalgam restoration so we need to go into the dentine and can't stop till enamel this is because enamel and amalgam both are brittle material And we'll only go for base and restoration no need of application of liner

I said varnish should not be applied below gic but can be applied after giving the gic base. Apply over it. Can you understand it now ?

About Your Question! 🙂 In some coments, someone said Varnish and no base then you said correct! But i have Question Why use varnish when caries didn't arrive to dentin? I think we just use base like composite and so...! Pls answer dear doctor ❤

Varnish prevent discoulration of enamel anr dentin because of corrosion products of Amalgam. Its not given for composite. Have you seen all 4 part of this video ?

Answers 1)Sir i think here caries extended1.5mm then i extended 1.5mm more to reach the thinest diameter of dentinal tubule. Then i goes to option BOTH VARNISH &BASE 2) If i extend less 0.5 mm in enamel Then i goes to option NO VARNISH BUT BASE APPLIED

Amalgam Restorative should always extend in dentin even if there is caries only in enamel. The minimum depth is 0.2 mm below dej. If you have extended minimum depth in dentin then there is no need for base and only varnish is sufficient.
