@@JessTheMD thanks for noticing what I said. Took me a while to come back, because, you know how it can be. I picked up on Cox saying that because way back when, I had a mentor who would work me to hell and back all hours, but always stood up for me when needed. Even 20+ years on i wouldn't call us equals on a professional level but I have his trust in my actions, which in my industry carries a lot of weight.

Let's also talk about the MASSIVE compliment Cox gave JD at the end. When JD said that he'd have made the same call Cox did, Cox looked at him and said "...yeah?" as if begging him to say that he wasn't just saying that. This shows that regardless of Cox's hazing, he has ENORMOUS respect for JD.

Seeing the unshakable Dr Cox break and lose it as he trashes the room is honestly one of the most emotionally brutal scenes in TV. The other was how he and JD reacts to the loss of Ben. Creating a temporary delusion to cope.

Credit to the actress who plays Carla in this scene. She's always so competent and collected, but in that scene with Cox she's so very sad and a little bit scared when he loses it. Heartbreaking.

This episode was based on a tragedy that happened in 2004, 3 patients died of Rabies. Obviously they weren't all in the same hospital but the race to get the organs postponing testing was true

Yeah it was a patient in Oklahoma who died. They misdiagnosed his rabies for cocaine overdose. Interestingly some of the symptoms are the same. They sent two organs to different patients in Texas, one organ to another hospital in Oklahoma and I think one to Oregon or Washington. And they died months apart too. One of the 4 patients was saved.

The thing that always gets me in this episode isn’t the way that Dr Cox reacts to his patient flat lining. It’s the way that Carla reacts to him losing his cool. Throughout the show Carla has always seemed to have the answer to Cox brash and fiery side but when Cox loses his temper due to this you can see that even Carla is stunned and doesn’t know how to help her friend.

I noticed they actually got something right there that a lot of medical dramas screw up. The patient wasn't flatlining, Dr. Cox said he was in V-tach. V-tach, or ventricular tachycardia, is a situation where you'd use a defibrillator, unlike flatlining, where it would have no effect.

This is one of the episodes that FUCKED ME UPPPP. Dr. Cox's acting was amazing, the song selection was perfect, obviously its not medically realistic but this one hurt. And then when he said his patient could've lasted another month without a new kidney, DAMN.

Truly excellent episode!! Yes definitely some liberties taken on the medical accuracy, but it was such a good episode (and show, honestly!) that it’s forgiven 😊

The situation is based on a real life story. In 2004 an Arkansas man came into a Texas hospital presenting with low grade fever and mental status changes. A diagnosis of subarachnoid hemorrhage was suggested following neurologic imaging and was finally the reason attributed to his passing, After his family agreed to donate his organs, three days later they were transplanted into new hosts. Over the next three to four weeks all three transplant patients passed due to complications from rabies. Pretty sure they all didn't happen in the same facility, but regardless, what a tragedy. After an intensive investigation it was finally determined that the original donor had been bitten by a bat. To make matters worse, a fourth death has since been tied to the same donor. It was from a stored segment of iliac artery, used in another transplant and ultimately claimed another individual's life. This is one of the most heartbreaking episodes in the series, but it's also one of the reasons Scrubs still remains my favorite TV show of all time.

@@SSJasonMsame. Not sure if The Frey quite knew just how much this song would affect Scrubs fans for years to come. It’s been 17 years since I first watched this episode and still this is what I think of when ever I hear this song.

The episode that comes after this one, My Fallen Idol, is incredible as well. It's a direct follow up to this one and shows Dr. Cox struggling with severe depression following the events of this episode. I HIGHLY recommend that one.

Truly a great episode. The fact that is was based on true event makes it even better. They change some stuff, like making all recipients be patients from the same hospital, but its still relly cool. The show is not really medically accurate, but its accurate enough. You feel that they cared about accuracy but they wanted to make a show that is still enjoyable by non doctors.

I've watched this scene a million times and another thing I noticed before his friend died that Cox was REALLY close to getting over it...because JD said he'd have made the same call he did Even with the emasculating comments and aggravated reactions and rants and proclamations of how annoying he finds him he deeply respects JD and it makes their dynamic so layered and loveable

This is the episode that makes my eyes tear up b/c as much as we know how unbreakable Dr. Cox is as a character, watching him break while his kidney transplant patient is flat lining is heartbreaking. Not to mention The Fray playing in the background adding to the sorrow!

Seen this episode and some medical professionals reactions to it a lot and it always hits me. Scrubs is my favourite medical show and these reactions are a great way of going back to it. I actually live in a country where unfortunately Rabies is relatively common. I never go near stray animals for this reason. Such a terrifying illness.

My brother has been a chiropractor for 40 years, leaving me with a deep distrust of them. Some, like my brother, consider subluxation a central principle, but other chiropractors have told me subluxation was never a thing. It would be like GPs disagreeing on whether germ theory is a thing or not. You don’t have any idea what you’re going to get when you visit a chiropractor new to you. Plus I can tell you first hand the chiropractic community is a cesspool of conspiracy theories.

Dr cox flipping over the machine in anger after the third patient died was such a raw & iconic moment that's still etched in my memories. Fun fact about "How To Save A Life" by The Fray here. Their use of the song came in April/May of 2006 4 months before it became a hit on the radio. This show always had a knack for finding songs that go with specific moments.

This episode is so intense it's almost as strong emotion as a real life event. The music really fit as well. It talked in a very real way about trying to save an addict and most of us have experienced something like that.

As a massage therapist, the chiropractic scene made me laugh because it reminded me of being in massage school and learning cervical manipulation and passive stretching techniques. One student asked the instructor, "Have you ever broken someone's neck doing that?" Fortunately, the instructor assured us that it wasn't actually that easy to break anyone's neck with the kind of things we were doing. ...I think the Janitor's methods have a much higher chance of doing damage, especially since he didn't properly warm the soft tissue first!

Hey Dr. Jess, I found the original article, this episode based on (N Engl J Med 2005; 352:1103-1111) . Unfortunately, this wasn't an isolated case, rabies infected recipients from corneal transplant donors earlier (see references of the N Engl J Med article, 14-17) or Clinical Infectious Diseases, Volume 50, Issue 8, 15 April 2010, Pages 1112-1119 (youtube doesn't allow links, even of scientific articles). I accept the reasons why checking all donors for rabies wouldn't solve the problem (rabies is very rare, too much time for the tests). But vaccination the recipients against rabies and/or check their vaccination history, prior the transplantation wouldn't be an effective approach?

Thank you for the link to the article!! Interesting read! Certainly it looks like those that were previously vaccinated did do well. The only issue is that it does take time to launch an immune response to a vaccination to subsequently develop immunity, so potentially vaccinating all people who are on a transplant list is an idea, but it would have to be well in advance of transplantation. The other consideration is population health and pros vs cons analysis. Meaning - is this a relatively common enough occurrence to necessitate large scale widespread vaccination. Does the risk of potential side effects of the vaccine outweigh the benefits based on the rate of rabies occurrences? I do not have the answer to this, and it would be best answered probably by a clinical trial! But very very interesting thoughts and certainly a potential mitigation strategy!! Thank you for this! 😊

​@@JessTheMD I checked the vaccination guidelines for organ recipients of some countries, Australia, U.K., U.S.. The organ recipients usually get at least 10 (!) vaccinies before and after solid organ transplantation, for example against Varicella, Hepatitis A and B, measles-mumps-rubella, etc. So a lot of data should be collected and analyzed before including vaccine against rabies to the guidelines.

@@JessTheMD im not a medical professional and maybe the infection gets fasttracked because of suppressants for the transplant and such, but isnt the rabies vaccine usually given to people after a suspected infection?

@@Phoenix-zu6on yes, but the problem here was awareness. in the show the timeline is probably too fast, but if the patients already show symptoms, it is too late to give the vaccine. Also if I would have to make a guess (also no medical professional), the virus probably also had a faster progression because it was already spread out in the transplant (instead of being injected in a bite)

Love Scrubs...great writing and just has so much heart to it. And as it turns out (based on the many YT reactions from doctors and nurses) I have to give them kudos for the amount of things they actually get right in that show....which is both impressive as well as, I think, a good thing

I completely agree! I believe the show’s creator based JD on his college roommate (or friend or something - can’t remember exact details at the moment), but long story short: he based a lot of these characters on people he knows, and got a LOT of input from them regarding accuracy and all! Really cool and something I love to see! ❤️

​@Jess The MD fun thing about the show, it was created with John McGinley in mind, I might have screwed up his name, could be McGinty. The show revolves so much around his character. With my age, it's much easier for me to relate to the older characters than the younger characters. Love Cox, Kelso, Dr Jan Itor, and Jordan. Grew to love the whole cast though.

oh shit! This video made me think of one ofthe last human interactions I hade before I shattered my elbow in Vegas. We were in a grogery store buying liquor and someone with meth teeth who claimed to be a nurse (was wearing scrubs) started telling us how she avoided hangovers by plugging IV's full of Gatorade into her arm. Was a good prelude to a night where I shattered my elbow and kept bar hopping for 10 hours after.

John C McGinley is an absolutely amazing actor, and I want my doc to have that big Cox energy when trying to save my life. This episode still wrecks me, just watching the reaction was hard enough.

God that part at the end where Cox so succinctly throws JD's words back in his face to demand responsibility for his friend's death. I got chills. It kind of makes me sad the only morbidity and mortality review the show did came early and provided no comfort because I'd love to see what dispassionate professionals have to say about using that kidney without confirmed cause of death. Weighing the time a kidney remains viable outside of the body against the timeline for receiving an expedited autopsy to establish cause of death and that even against the odds of getting a different donor within the month or so that man could have continued to survive on dialysis. Cox very well makes the point that this isn't as cut and dried a case as the prior two patients, but that still doesn't mean he made the wrong choice and I think this might actually be some sort of comfort to him. Or not, the guys a bit of an egomaniac.

I will tell you - it's shocking. It's a lot to handle. I still think of one specific patient that I was absolutely NOT expecting to lose, and coding the person and all. Some of those things just stick with you. The sheer terror in the person's eyes when they knew that they were going into cardiac arrest - I'll never unsee it. Super heavy stuff happens in the hospital! This episode was a wonderful episode. Scrubs is just a phenomenal show!! ❤️

The end of this episode was possibly the most emotionally devastating moment in the series to me - definitely in the top 3. I only wish this video had included the full minute or two that closes out the episode, continuing from when he's using the paddles on the third patient. When he starts yelling out in frustration and knocking things around, I always tear up. And then walking out in the middle of his shift, after J.D. echoes the warning that once you blame yourself, there's no coming back - to which Dr. Cox says, "Yeah. You're right" and leaves. Man, that's a weighty ending.

That's an amazing one as well. This one's ending hits hard and is more memorable to me because of Cox's raw emotion at the end, but the next one deals with the aftermath of his breakdown in a moving way - particularly when J.D. finally visits him at the end.

This is one of the saddest episodes, next to the ones in which Ben dies and the one with Laverne’s death That show was so amazing, i binged it over and over again, to the point that I could enter the room where my roommate’s were watching an episode, I watch it a couple of seconds and could tell them season and episode. At least for season 1 to 6

OMG I was the same way back in the day! 😝 It's been long enough now that I've forgotten a lot of the details. But my shows that I binged like that and could quote most all of it were: Scrubs, How I Met Your Mother, Big Bang Theory and Friends 😝 Laverne's death was also such a tragically sad episode 😔

Now if JD had reported Jill for what he believed was a mental health crisis. What is the likelihood they would have recognized that her symptoms would have resulted from rabies? If they were able to recognize that was the cause would it have been too late anyways to save her?

1 think i find so wierd when seing rabies symptoms is that fear of water. How can u get scared of water from contracting a virus. I understand that u can generate a laster fear of something from an event, but how is a virus infection giving u a fear u never hade before makes no sense.

Nevermind the work/hime separation (going out with scrubs) what about hygiene?! Both what you might take out from the hospital and what you might take into it to the next patient? Is it normal to leave the building wearing scrubs?

In regards to chiropractic treatment, it feels really good when done by someone who was properly trained. Spent some time in the birthplace of American chiropractic treatments, Iowa, and the students needed people to practice on. When looking for a chiropractor, be careful. Many who go to the Putnam School in Bettendorf, Iowa are trained to regard chiropractics as pretty much a cure all. There are other schools that teach chiropractics and basic medicine. Either way, being adjusted by a chiropractor feels good and can loosen the stress in your body.

Bypassing the obvious flaws in the medicine in the episode, the barely there code procedure for the kidney patient, the fact that one donor would never be an exact match for several transplant recipients, the fact that Jill exhibited no symptoms of rabies prior to losing consciousness and the fact that Cox likely would be in extremely hot water with the medical board for patient abandonment at the end of the episode, it's still one of my favorite episodes of one of my favorite shows lf all time. Scrubs has a reputation for being one of the more accurate medical shows, creator Bill Lawrence employed his college roommate who was an actual doctor, as an advisor, but it gets plenty wrong. However, even when it's technically incorrect it still feels very accurate for life in a hospital. Btw, Grey's came after Scrubs and borrows from it considerably at times, a fact that Scrubs itself makes fun of in an episode. There's also an episode that features 3 guest stars who play doctors who were also doctors in one of the more famous medical shows of the 80s, St. Elsewhere.

Hmmm. Not sure I'd use the word "boasts" in that situation, or maybe I'd use it in the opposite way you did. I think 2 things when I see a Dr in scrubs outside of a hospital. Either A, they ARE trying to "boasts" and be cool about being a Dr, or B, they're tired and don't give 2 sh1ts what they look like as long as there isn't blood on them. I assume the latter, but I know a few of the first category. They're the guys who wouldn't be Drs if daddy didn't have deep pockets for repeating tests and courses lol. Enjoyed the reaction. I had an ear infection and needed my diabetes prescriptions renewed and my toenails fell out and it took me six months to find a doctor. It would be nice if I could have had someone to talk to when my father died (my fault) but no, medical care in the US isn't always available to everyone. Sorry to say.

I have no knowledge on medicine at all. I'm curious, how does it work if the people receiving rhe organs are all vaccinated against rabies? Im assuming it makes no difference as the complications are already there but just curious. 😅

That’s a great question - I don’t know the answer for sure, but I’m thinking that the vaccine is meant to prevent overt infection. If the organs are already infected/diseased, I’m not certain it would have actually made a difference… but truly not sure about the answer on this one!

I know that Jill Tracy was often used as a punchline, but she was actually such a tragic character. The first episode she was in the show she was constantly overworking and losing herself trying to live up to everyones expectations AND her own expectations (which seemed to be more what she thought she should want from life than what she really needed) and it caused her to stumble in that downward spiral that she never really managed to get out of again. And then she tried to overcompensate her depression and mental struggles with this bubbly, over-the-top behaviour that just irritated the hell out of everyone. She was the personification of "the road to hell is paved with good intentions" and her death and organ donation causing three other deaths was the tragic pinnacle of that.

You kinda missed the point of the Todd segments, Turk was warning them that even if he does turn out to be gay, he's still the Todd, he's still a poon hound; and it bit them in the ass when Todd used that to harass them since he's bi, something they didn't even think of. There's another episode where there's a gay couple and they're portrayed pretty fairly.

You made a very touching PSA about mental health at the end, but i would like to make another one about rabies… and a more personal one about Lymes Disease. If you get bit or scratched by a wild or strange animal , get checked out. And check for ticks when in an appropriate environment , especially if you find a living tick in your home or on a pet. Last summer my brother got lymes disease… after he was EVENTUALLY tested for it…. He was sick for several weeks before his doctor finally tested him. And he forgot to mention he had been both in a heavily wooded area (campground turned into RVcabins) and had found a tick on the back of his dogs head

I am not sure how you feel about criticism. This is just a hobby and I am learning from your channel, which is nice, honestly. I'm a jobless person who did video editing as a hobby so feel free to take my words with a grain of salt, tbh. My critique on this video, which I think applies to other videos, is to not make so many cuts in your videos and let the video play out longer DURING the more emotional moments only. In my experience, people want to experience these emotional moments with the person who is narrating the video. In this case, those scenes with Dr. Cox saying: "He wasn't about to die, was he noobie?" and ending the video there makes me, as an audience member, feel like I have been cut short of experiencing a "moment" with the doctor experiencing this moment with the video because that is what I, as an audience member, want and expect to experience with this video. BUT, if it is a personal choice to not expose vulnerability on your platform, then I respect that choice. However, if the reason for cutting scenes and cutting the length of the end to shorten the overall video... Then, I would advise against this for harder-hitting emotional scenes. You want to extend the emotional parts out because that is how an audience can "relate" to the narrator of the video more because of the emotional impact and emotional connection those moments will have with the narrator. Anyway, just my two cents. I'm not saying your video is bad (It is a good video), but I am giving some critique that could, hopefully, improve the quality of your future videos. Best of luck! I know there are a lot of gray areas and I have a hard time with those. Yet I would think that JD does deserve blame. Doctors are mandatory reporters. Even though Jill did not seek out treatment at the hospital, she did open up about her struggles and that he had not seen her therapist in a while due to certain things and being stood up, should have definitely been a red flag for him. He was right to feel guilty and even though ultimately she did not take her own life as the episode later showed, he should not have been relieved, the bare minimum for him would have been to suggest for her to go get some help or to talk to somebody and offer her resources that could help her. If Jill had a physical issue instead of a supposed mental one, there is no question that JD would have recommended going to the emergency room to seek treatment. Of course that is also not always the best place to send someone who is having a mental health crisis, at least that is what I found out from my own experience when I was teenager.

Chiropractors and Osteopaths can do miracle work and solve some really spooky inexplicable problems (unexplained nausea for example). But many have no talent and can't fix your problem. You have to keep shopping until you find one of the less common talented ones. I had a rib out once- the chiro was basically doing generic work on me that had nothing to do with my stated problem and after 15 sessions I went and found an office chair with a webbed back and adjusted my own rib. Oh- I'd be *real* careful about music. So many youtubers get burned for any clips over 12 seconds long. It's always very upsetting to them. And it usually hits when they just start to gain momentum and pass 10 to 20 thousand subscribers.

When it does, please don't get upset (since you know it's coming, right) just edit out the music and reupload. The strikes have come over as few as 3 seconds of music . Technically not legal. But the KZfaq appeal system is mostly automated.

Todd is bi, and it’s not an implication, he admits it, and no one in the show at all thinks or says it’s a big deal in any negative way, they’re not negative about it at all, just mentioning or questioning the possibility someone might have a different sexuality than they’re presenting based on certain sexual behaviors that are not typical of heterosexual people (ie, BEING ATTRACTED TO ANYTHING OTHER THAN THE OPPOSITE SEX AS IT IS DEFINITIONALLY CONTRARY TO THE WORD HETEROSEXUAL (LIKE TODD VERY CLEARLY IS)) isn’t homophobic or offensive. It might be in bad taste nowadays, and sure that’s a reasonable and fine point, but acting like it’s like homophobic or vitriolic is just stupid. Other than that, wonderful video though, and I appreciate your insight otherwise…

you did not cover one of the most important parts of the episode it showed Cox's weekness in full, he leaves though his shift and gets told by JS "remember what you told me start blaming your self for patient deaths that a door you can never close", Cox responds with "i know" and walks out, showing that weakness in cox and that he's human and has flaws and he was talking from experience with JD.

That’s a great question and I don’t know the answer for sure! Where I trained, we didn’t have a robust transplant service, so our hospital didn’t really do much of that… the only experience with a patient that I remember vividly is a patient who passed away after a high spinal cord injury - and was otherwise healthy. 😔 so you definitely bring up a great point! I’ll ask some colleagues about it 😊

very sad ep, but at the same time, isn't rabies not only quite rare but apparent when symptoms start showing esp the neurological symptoms? I mean it's not called "zombie virus" and "hydrophobia" for no reason? I feel like an actually responsible hospital would catch that, but idk. Rabies and all the lyssaviruses are very scary, considering they're almost always fatal after symptoms show, and it's really sad in pets like dogs, although they've done a really good job of making it very rare in western countries in the last few decades.

Yes, it can be apparent but I believe in this scenario, the patient was already completely unresponsive, so there’s not a lot of neurological symptoms that are evident at that point, unfortunately. 😔

This episode was based on a real life incident. It's dramatised of course, in real life the organs went around the country and there wasn't the emotional "I needed the organs so bad I fucked up" angle, but there was a real life case where someone died of rabies and because it's so rare it wasn't caught, their organs were donated and transplanted, and all the recipients died (I think one died during surgery rather than from rabies and the other 3 got rabies).

And since they were all transplant recipients/patients, then they were all on immunosuppresants... So rabies would run it's course in them MUCH faster. I don't even know if prophylaxis or any vaccine would have made a difference even if given at the time of them waking up from the transplant. IMO

I do understand what you're saying. I would also argue that not everyone who is morbidly obese would be in the category of abusing their heart. Some people have metabolic issues that may be genetically inclined, and have a lot of trouble losing weight, despite healthy lifestyle choices. Obesity is such a multifactorial disease process that it's hard to just generalize "obesity = body abuse". But I definitely can understand your point, especially if you're not diving deeper into how complicated obesity is as a disease 😊

What yes he did....he should have tested those organs before he put them into the patients ....I mean they were willing to be reckless with these patients who to say they wouldn't be reckless with us or if not us you or if not you then your love ones.....rushing into a surgery to replace the organs with damaged organs is not a good job or a good trade off especially if it hurts or in this case kills numerous people in the process ....thats not a good look for the hospital or doctors in general because it make you look like murderers....you didn't save there lives....you just prolonged there deaths....and possibly made it more painful for them and there family.....i dont care what anyone says your the ones in control not the ones Going into surgery ....they're lives are in your hands they trusted you....and you failed them....and you wonder while people don't like to go to the doctors ....its because of things like this .....and if you still defend these doctors negligence then I wouldn't want you to be my doctor or be my families doctor in fear you would make the same mistakes....you should be ashamed of yourself for says it's ok to screw up ROYALY ....ITS NOT OK

Just to make sure you realize… rabies is an extremely rare disease. It is NEVER routinely tested for before transplants, surgeries, etc. Of course, this situation is absolutely horrific. However, the patient that came in unconscious had zero indication of this being rabies (or at least from what they showed in this episode - and remember, this is a TV show…), and this is quite literally never a routinely tested lab prior to surgery. Now, if, for example, this patient had hepatitis B or C or HIV, and those organs were transplanted in this particular situation, I would 1000000% agree that this is negligence. But rabies? It’s SO rare that it’s like saying you must test for every disease know under the sun prior to transplant and that is quite impossible given the time constraints for transplantation…. We work via evidence based medicine. We test for certain more common pathogens prior to a transplant, things that we would be more likely to potentially see. I would agree with you if this were a different blood borne pathogen, but not rabies. “Damaged organs” - I assure you that if the organs visually appeared damaged, no surgeon would ever transplant them. So, in a real life situation, if the organs looked visually healthy, and the pathogens that are routinely tested for came back negative, with no other indication of an infectious disease being the cause of death of the donor, there would be zero reason to doubt the situation. This is a horrific situation, but there is only so much someone could prepare for, and a diagnosis of rabies is far from that, given the entire presentation.

Also to add, this story is BASED onna true story, of course IRL the 3 patients were on 3 separate hospital all across the US, the donor was a cocaine addict died of brain hemorrhage, they all thought it was because cocaine overdose, turns out his dilapidated house has a bat roost in which he contracted rabies from, it took the CDC far longer than this show's episode to actually found out it was rabies that caused all of these donor recipients to die. As in this episode, two of the 3 patients are dying and literally clinging to life, the test would've prolonged the operation and they could very well died before the test results came.

@@JessTheMD Best to ignore that trolls. They will eat away at your self esteem and joy in your channel. And it literally doesn't matter what you do- they will attack you. Joe's Blogs is really struggling with this right now- he isn't even talking on camera any more due to personal insults by russian and chinese trolls. Also- I think you can moderate your own forums so you may want to simply delete toxic comments like this (including this encouraging post). As you get bigger, get an editor to do it for you so you never see them in the first place.
