Dr. Ebraheim’s educational animated video describing the condition known as radial nerve palsy.
The radial nerve originates from the posterior cord of the brachial plexus. Radial nerve injury results from acute trauma or compression of the radial nerve.
Condition is often referred to as
•Saturday night palsy: alcohol is sometimes a factor as person falls asleep with the back of their arm compressed by a chair back, bar edge, etc.
•Honeymoon palsy: from another individual sleeping on one’s arm overnight, compressing the nerve.
•Crutch palsy: compression on nerve from walking with crutches.
The radial nerve supplies the extensor muscles allowing for extension of the wrist and fingers in addition to supplying the triceps that extends the elbow. If the muscles are not working properly, the patient will experience the condition known as “wrist drop”.
A radial nerve injury classically occurs by fractures involving the distal third of the humeral shaft. The patient will be unable to perform the “hitch hike” sign.
If the nerve is compressed at the axilla, the triceps muscle will be affected. Patient will not be able to extend the elbow in addition to wrist drop.
Treatment of radial nerve palsy
•Brace, cockup splint
•Physical therapy to help maintain the muscle strength and avoid contracture.
•EMG and nerve studies:
Brachioradialis is the first muscle to recover.
Can be done as early as three weeks and follow the progress.
•Nerve exploration, repair or graft: nerve exploration:
Nerve exploration is indicated in open humeral fracture with radial nerve palsy or in conditions that have had sufficient time without nerve recovery.
•Tendon transfer if nerve cannot be repaired or the function cannot be restored.

Thank you so much for these videos. Your videos are very helpful as an OT student. I appreciate that you include physical therapy in your recommendations for treatments of a variety of conditions; however, it would be amazing if you could also mention the importance of occupational therapy in treating these conditions, especially upper extremity conditions. I think OT is often overlooked and it is a vital part of a patient's recovery. Thanks again for everything!

After four monts I lost the wrist drop. I can move my wrist up now. I imagine I will have a full recovery within the next two months.

@Sailor Moon No it will not be sudden. Signs of recovery will depend on your habits. I use a computer all the time, so typing and using a mouse has become easier. From a mental perspective, you need to accept the fact this is going to take a long time to heal. Nerves are a bitch if they get bruised. Or if you have a bone fracture in your radial nerve area that is suppressing the nerve, meaning the nerve is bruised. Another sign, which will sound weird, but if you scratch the side of your head with the hurt hand, it should feel really weak, and difficult. Signs of improvement will show that you can do this more normally and with more strength. Cutting fruit is also a sign of improvement if you feel you have more power to cut said fruit, like a lemon or orange. Either Way, assuming the nerve was not severed by a bad cut, or some other trauma you WILL HEAL. Like I said I can finally move my wrist up, and that is the sign to full recovery. I don't have wrist drop, but I am not fully recovered. My hand is still fairly weak. Meaning I could not lift a weight without a wrist splint yet. The bad news, this could take up to six months to heal, the good news you will heal. I like this channel, so I can tell people they are not screwed. When I first went through this, it was scary, so I want to tell people not to freak out. Others on this channel have reported full recovery as well. Hang in there, wear your wrist splint all the time. And try to use your hurt hand as much as possible for normal activities. and I do not advice seeing a doctor anytime soon. I did, because I was scared and they did nothing for me, they did not even know what they were looking at. Then I got slapped with a $3,500 hundred dollar bill for nothing. This heals on it's own. Just get a couple of good wrist splints. And use your hurt hand as much as possible, to a comfortable degree. Last thing I will say, if you have numbness on your thumb, upper hand or anywhere due to the injury, do not worry it goes away. That scared me alot.

@@jasonaquino8232 thank you I have had wrist drop for 7weeks now after a humerus fracture but I have had some minor improvements compared to my first week so that's a good sign...also the burning pain I had in my hand is gone just the tightness and numbness is what I feel around the fingers. I use a splint and I have therapy balls I got off Amazon. My strength is pretty weak though.

Thanks for the reply, glad you're nearly all better. Here's a little tip I found has really helped me since starting it yesterday for anyone suffering from it. Most mobile/cell phones you can set multiple alarms for. Set an alarm every few hours throughout the day and evening to do some wrist exercises and get in to a routine.

Mine develpoed as a post operative condition following carpal tunnel repair on the radial nerve using a radial axillsry block as anesthia. I woke up in severe pain in the recovery room. My surgeon had already gone and left me in his partners care. I was in so much pain they could not release me from the hospital and it was supposed to be an outpatient procedure. No one seemed to know what was wrong. When my durgeon returned to work he knew immediately what was wrong. The pressure of the wrist drop was pulling on the axillary section of the radial nerve causing severe pain. He orderedca brace to be put under the cast my wrist was in and the pain stopped almost instantly. It took almost a year of intensive therapy to regain 90% use of my hand . There are still certain movements I can't make, like the boy scout salute or counting to 3. This can be a debilitating nerve injury and should be monitored if it doesnt heal after a few days.

Tendon transfer is not the only option. A nerve transfer is an option. Two branches of the median nerve to radial nerve. FDS to ECRB wrist extension, and FCR to PIN finger extension.

Three weeks ago, I had a horrible snowboarding accident that lead to a broken humerus. After a 3.5 hour surgery, I was unable to feel or move my wrist and fingers. The doctor diagnosed it as radial nerve palsy. I’ve been going to physical therapy and doing exercises every day. The feeling and movement in the wrist are slowly coming back, but the fingers… not so much. i can’t make a thumbs up, or extend my fingers at all. It’s been hard, and there’s been some depressing moments. Reading the comments in this section makes me realize that I’m not alone. Just gotta keep working hard and being patient.

Hey Luke, I'm in a similar boat. I fell asleep on a computer chair with the arm rest jammed into my arm pit. I woke up with my right arm feeling numb. I was unable to move my wrist upwards or extend my fingers. Yeah it's been insanely depressing man. It's been a month and I can now extend my fingers. My wrist still has the characteristic "wrist drop". I've been doing physical therapy twice a week. The neurologist is like 3 months out. I have been trying to find an answer online to no avail. How is your hand now? It's been like 7 months since you posted

It is controversial subject on whether or not the nerve will recover on its own. After three weeks you should have an EMG (nerve conduction study). If the nerve is not cut, there is a good chance of recovery.

Wanted to share that I got Saturday night palsy. Can initially be scary when you read online it takes months of recovery in even mild cases. Not necessarily true, there is a huge variance in recovery times. For me it took 4 weeks for full recovery. Was dominant hand and I live in China so couldnt eat with chopsticks or cook for a week and a half. But gets better!

thank you for the video, what happens if its a low lesion? would the patient get wrist drop? and what about the interrosei nerve that branches off at the elbow, would that not allow a patient to retain function?

I got a touch of that Saturday night palsy. Only been about 4 days. Ordered a splint and am doing exercises. Trying to use the affected arm often. It’s also my dominant hand, makes things quite difficult.

In high lesions, such as in fractures of the humerus (particularly mid-shaft fractures where the radial nerve lies in the spiral grove), or due to prolonged tourniquet pressure, there is weakness of the radial extensors of the wrist numbness over the anatomical snuff box. They are also seen in patients who fall asleep with the arm dangling over the back of a chair ('Saturday night palsy').

This happened to me April 13th, as of today june20th, about 9 weeks I can extent my hand with fingers straight parallel with arm but I still have no strength to my wrist. I've read that the full healing starts at about 3 to 4 months

i still have sensation over the area of the arm/hand that is controlled by the radial nerve, just no motor function. wrist movement is coming back very slowly. it isn't level with my arm yet, but it does flicker up a little.

hi , I wanted to share my bitter experience. I had suffered from radial nerve palsy after I fell down from my motor bike and broke my right hand humerus. It was in Last august 2014. It was so painful both physically and mentally as doctors have not given any time frame of recovery. I was worried, and thinking what if I can not recover. This video here gave my insight about my injury and the comments the hope to fght back. I had gone through physio therapy for 3 months and finally recovered from it and able to work normally. I even started driving my bike in due course. I really feel grateful for my physio for his help and positive talk. I had to use dynamic cockup splint though it looked very awkward when I was using for typing on keyboard and doing exercise . Advise all the fellow injured to keep your spirits up and believe that you can recover fully from this.

Hi Basav, it can be anywhere between 2 months and 6 months depending on the extent of nerve injury. Also I came to know that one will recover completely even if the nerve is damaged about 70%

i am diagnosed wrist drop by my orthopedics surgeons. he prescribed me some medicine i.e. inj. methycobile, syngap and cockup splint. after 2 weeks passed no improvement found at yet. should i go for surgery or wait. please anybody help me out.

@@Nada-wq7tb i started seeing results around the 3 month mark i believe it was. Make sure to take b vitamins (spirulina is a good option) and also if you can , use electric stimulation on it! I had just severely pinched my nerve - not a full blown sever. Stay optimistic! Everyone's journey is different! It's one of the hardest things you'll go through! I have 100% full function of my wrist again.

@@fikrihelminugraha I'm sorry, just now seeing this. I used multi B vitamins and electric stimulation EVERY DAY! Mind you, i just severely pinched the nerve, not severed. 6 months later i had a usable wrist and now several years later i am back to 100%.

Anybody watching dis n having doubts keep pushing I made a full recovery frm dis only thing I can say now is my arm get tired real fast then normal, n for the people who freshly broke it keep to ur rehab everyday put ur wrist over a chair n try to lift it everyday u will see progress it may hurt at first dem sharp pains u gon feel frm ur nerves feel like ur arm on fire but keep fighting, it started wit my thumb which came back first then so forth

I’m experiencing this right now, I don’t drink but fell asleep on my desk chair for hours and woke up with a left arm wrist drop and went to the ER the day after Easter. I’m also left handed so hopefully wearing the brace and self therapy helps with recovery.

Went to ER, told it was Saturday Night Palsy. at first, I was unable to move my wrist upwards or extend my fingers. It's been a month and I can now extend my fingers! My wrist still has the characteristic "wrist drop" though. I'm being told that after the nerve decompresses I will see signs of recovery in my wrist. Are you unable to move your wrist/fingers too?

I had tendon transfers 10 years ago…. Are there any treatments to try a nerve graft, transplant, etc to try and revive the nerve or is the tendon transfer what I am stuck with? 😢

Injury to the radial nerve sucks. I went through surgery on mine in January (5 months ago), I'm finally able to hold my wrist up and minimal extension of my fingers has returned. I'm still praying for full recovery.

Hello Dr. I was shot in my upper left arm and I have to go for this surgery this Tuesday. I Was shot 4 times with a high velocity gun on the 2 September 2015. I had the emg test and the dr says I should have the surgery. What are the risks of the surgery? also what do you sacrifice by doing the transfer? I am scared and wonder if I shouldn't wait? How long after the trauma must this surgery be done? I would appreciate your opinion? Thank you so much.

i had someone say they fell on ice and hit there wrist no swelling or burising but had wrist drop no fracure is that normal

nice educational presentation.but posterior interosseous nerve injury is not included.it will be better if have similar presentation on posterior interosseous nerve injury also.

Hi. It´s been 8½ months since I fell and "did something" to the radialnerve. Just shortly before going into surgery for pancreas cancer. I told the hospital about my issue, but nothing was done at first. It took them about 7 or 8 days to finally take an x-ray and have a doctor helping me. I know from reading on the internet that it´s important to take action as soon as possible or else you will risk malfunction. Well, this happened to my right arm/hand/fingers. I was going through hell healing from the whipple surgery and a right hand not functioning at all! The muscles in my arm disapeared completely and I´m now trying to build up muscles again. The process is real slow and I wonder if I`ll ever succeed. I have partial function of my fingers and can now use a pair of scissors ubt with lots of difficulties. Everything I do is with difficulties….

Hie.. Im sorry this had to happen to you. Hope you recovered and are much better now. As a physical therapy student I'm keen to know your progress and what you think worked well or didn't during your course of treatment.

This just happened to me two weeks ago after waking up in the middle of the night in my computer chair. I have right wrist drop and have gone for CAT scan and MRI. Dr. has scheduled me for an EMG and physical therapy. Your video was extremely informative and I appreciate the time and effort and of course your experience and knowledge. Thank you very much.

damn same exact thinh happened to me! i spoke to a doc on the phone first (cuz COVID) and he told me i had to go to an ER for brain scans or possible nerve/muscle dmg. i saw my co-pay was $150 + $200 deductible, so i was like f***. then i decided to just go to urgent care and surprisngly i saw a doc like 2 hours later. the doc i saw said the same as on the phone and sent me to a specialist. the specialist's nurse gave me x-rays (they say they have to do x-ray before MRI). 2 minutes after my x-ray, the specialist walks in and immediately tells me i have Radial Nerve Pallsy and give me a wrist brace/ splint. The fact that your doc's didn't catch this before you had a CAT & MRI scares me

same thing happend to me. I can feel my fingers ect but cant extend fingers or lift wrist up.... did u recover
