In 2019 I underwent a lumpectomy and radiation for DCIS, grade 3, ER+PR+; I continue to suffer from fat necrosis (severe) as a result from the surgery/and/or radiation. I currently have some type of breast infection (open and draining!) on the same breast-2 weeks of oral steroids and antibiotics have not helped. Culture being done today, probable IV abx once culture is back (?) So when people say "oh, you 'only' had DCIS, lucky you" I cringe. Thank you for your informative video..some of the information was NEW to me...and I have numerous doctors. Knowledge is power!! Update: I had a midline IV placed, 6 weeks of mulitple-times-a -day IV antibiotics and then ended up having a mastectomy. Still doing reconstructive surgery--two surgeries this past year, another 2-3 scheduled for 2024. Again, there's no such thing as "easy" breast cancer--for me anyway.

I was diagnosed with DCIS in September 2019, medium grade. Had lumpectomy in October 2019, re-exision in November that year. Hot clear margins. 20 rads in January 2020. I was also 95% ES+ so therefore I am on Letrozole. Doing great! Your video was very informative & at terms we can understand! Thank you

I was recently diagnosed with DCIS stage 0 ( size was 11 mm or 1.1 cm and Low grade 1 Margin 5mm from DCIS ). The doctor did a lumpectomy on Sep 15 and recommended radiation for 5 days for 5 mins. I have to take the antihormone pill Armidex (which I am paranoid about due to side effects). Starting radiation on Mon Oct 31st.

@@katiedemingmd2226 when having DCIS is the breast painful to touch? I have a cyst in my nipple and is painful for some reason i have been having a feeling that i have this.

@@FanDebronco97 DCIS or breast cancer does not usually cause pain. Cysts are more commonly painful. Of course, you definitely want to check with your doctor or have a mammogram or ultrasound if there is a concern.

Would love to understand the implication on how radiation could potentially affect fertility. And how long after radiation does patient need to wait before trying to conceive? There are so many considerations and it's giving me so much unnecessary anxiety.

I was also diagnosed with DCIS, stage O in July 2021. I am expecting to have to my lymph nodes (3 of them) taken out to be sure that it did not spread anywhere else. I’m glad I saw thIs video.

I had my lumpectomy five weeks ago.....6mm tumor..... My margins were clear and the sintenel node and the adjacent one were both clear. My radiation treatments are, as my oncologist explained, " The Condensed Version," as I have only 16 total to take. Thank you, Dr.Deming, for your explanation. I know more than I did, and I feel even more certain about my future.

I was just diagnosed 3 weeks ago with stage 0 DCIS and going to all the new Dr appointments in the next week. Alot of information thrown at you. Hopefully I can treat it and not have reoccurrence....this video helped me to understand it and I don't feel so scared anymore. When you hear CANCER it's always a horrific feeling.

Hi Nikki. I agree about so much info being thrown at you. So many things we neer knew until we were diagnosed,right? My head is still spinning, but I'm thinking positive thoughts. I know stage 0 is cancer technically,but I focus on the terms"precancerous " and "noninvasive" as it calms me down. I wish you well and understand how you're feeling. I take comfort knowing my surgery in 3 days is a "same day" affair,and that is great news. Hugs to you.

@@katiedemingmd2226 You are so kind. I will do exactly that on my long evening walk. I find walking/w music is a good way to up the endorphins. I was a nursing assistant in 1969,when breast cancer was a whole different "situation." I focus on this being 2021,and all the magnificent technolgy and advancements. English study found one week of radiation as effective, and actually had less recurrence than 3-4 week so I’m wondering why most of the world does one week radiation but USA does 3-4 weeks.

I had a lumpectomy for a DCIS cluster, path report showed IDC, DCIS in the margin, DCIS still scattered throughout my breast, DCIS is high grade, ER Positive, HER negative, BRCA negative. Prior Stage 2 cancer in opposite breast in 1996, chemo, radiation, tamoxifen…no DCIS or cancer in that breast now. Age 72. Would you recommend a mastectomy with reconstruction or just radiation? Aunt on my mother’s side had breast cancer twice, both with mastectomies. Other female family members had lung cancer, stomach cancer. Thanks. Good video.

@@foxyred1015 hello sunny this is Debra Hunter, I just had lumpectomy Monday Nov 7. Last night I got preliminary results on my patient portal. As much I can understand it stated the boundaries were completely clear of malignant. 2 lymph node were removed both completely clean. They also did immediate IORT. Have appointment this Monday, I'm hopeful no more radiation needed. Oh yes got immediate reconstruction with gel implants. I changed my bandages today my new breast are beautiful although swollen. We get our of this depression and learn to live again. 🙏

I was told by the surgeon who did 2 lumpectomies on me, that most plastic surgeons will NOT do reconstruction after radiation. I have so far had one plastic surgeon who wouldn’t do it, but referred me to another plastic surgeon.

I did 16 sessions of radiation. I was going to stop at 27 Grey but was talked into finishing. I wish I’d stuck to my guns. Radiation burns don’t start until after 27 Grey. The radiation medical people don’t see the results because it just keeps getting worse and worse after you stop. Two weeks after radiation, I feel like an extra crispy fried chicken breast, but at least they kill the chicken before frying it. The burns aren’t really down near the nipple, but mostly above the bra line and on the side. First it got red, then redder, then blisters, then fluid from the blisters formed crystals on the skin, then the whole burn formed a crispy shell, now small pieces of skin keep catching on T-shirts, is cratered, lines, crusty, brown, and extremely painful! Drs don’t see this because you’re cut loose as soon as you are done with radiation. They wanted to do an additional Boost week. I am so glad I refused it. I wouldn’t have a breast left if I’d done that

You are correct that the skin reaction typically peaks around 1 week after radiation is complete. I have close follow up with my patients to check on their skin reaction. One piece of information that may help to understand is that it is better to complete radiation than to stop part way. In order to get the anti-cancer effect of the treatment, you need the full 16 treatments (42Gy). When you stop radiation early ( 27 Gy as an example), you will receive the side effects (skin redness & burning) but not the anti-cancer effects. That’s why it is always better to complete the treatment than stop midway. Your medical team wanted to make sure your received the benefit and not just the side effects. Hopefully your skin is healing. A calendula based radiation cream like Miaderm is the best way to prevent radiation burns. But, Aquaphor can be helpful in the healing phase (after treatment is complete).

I’ve been using calendula the whole time, also Manuka honey, ceravee healing, Ora’s Amazing Herbal Touchy Skin, Aloe Vera, and everything else recommended. It has been 2 week since I finished radiation and I’m still in lots of pain. I finished, and hope it did some good.

Whole breast is the most common type of radiation for DCIS, although occasionally accelerated partial breast irradiation (APBI) can be used.

I just finished radiation. I didn’t want to do it but they insisted. I had two lumpectomies with the last being clear margins of 4 mm. they originally told me grade 3 but paperwork said grade 2.

Hi, thank you for your informative videos. I had 12 centimetres of DCIS intermediate grade with 10mm margins. I had a mastectomy 6 weeks ago. Is there any indication for radiation given the size of the DCIS? Mine was hormone positive 50-60%. Thank you SO much:))

Dr. Deming. Thank you. Would you kindly address the recent (June 2022) conference where it has been shown that early stage BC can be equally be treated without radiation. Women who chose not to do radiation had no adverse outcome or nearly none. It seems that with improved diagnostics and other tests that will be required after BC, any reoccurrence would be detected much sooner. Why continue advising and practicing radiation in the early stage cases?

Hi. The study that was presented at ASCO in June of 2022 showed that women 55 years or older with grade 1-2 tumors that are 2cm or less, ER+ and no lymph node involvement and low Ki67 score can avoid radiation assuming they take anti-estrogen pills for 5 years. Previously, we had data that women older than 70 years could avoid RT if they had early stage disease and agreed to anti-estrogen therapy, but this study shows that women 55 years to 70 years may be able to avoid RT also. But, one thing I want to point out is that it assumes you will take anti-E for 5 years. Some women choose to have RT instead of the anti-E because of side effects and duration of treatment. To make this decision, you should meet with both a radiation oncologist and medical oncologist (prescribes anti-E). They can explain the side effects and risks associated with both treatment. This will help you make the best decision for your body.

Is radiation therapy needed after surgery for removal of nipple and areola because of Paget's with no underlying lump? I was just diagnosed with Paget's after punch biopsy of the nipple.

Very well presented by Dr. Denning. But, just one correction-please remember your Latin, and say “In See-too,” not “In Sight-too.” Please help spread this correct pronunciation in the medical community. Our verb “to sit” in English comes from the same Latin root. DCIS is still “sitting” within the duct, maybe think of it that way, to help remember the correct pronunciation. Thank you for this good, clear explanation regarding radiation for DCIS.

Thank you Anglo for mentioning this. i studied Geology and in our profession it is pronounced correctly, "in-See-to". I was recently diagnosed with DCIS. They kept saying it wrong, and so I almost started to pronounce it wrong. I'll stand my ground now. LOL!

thank you for this video. i was dx with high grade dcis with necrosis. my lumpectomy is scheduled for 11/22. my surgeon told me i may be a candidate for an abbreviated radiation course, where i would have 5 consecutive days of radiation for one week only, instead of the normal 4-8 weeks. also, the radiation would be localized to only the area where the dcis was, and not the whole breast. thoughts? what makes someone a good candidate or a bad candidate for this?

After my lumpectomy ( 5 weeks ago), I was told by radiologist I needed a full breast radiation ( 3 weeks) plus a booster ( 1 week). They told me it’s because my report came back as high grade ( grade 3) dcis and I’m also relatively young ( 41), so chances of it coming back would be 40-60% for me.

I am just recovering from radiation therapy for my DCIS. My DCIS was diagnosed in early January, After the lumpectomy pathology indicated the CIS.and we begin radiation therapy after the lumpectomy. I started radiation in mid March, after doing some due diligence to learn about what I was entering into, and within the timeframe that I was advised would be acceptable. So I, theoretically, still have about a month of recovering time post radiation treatment. I was hoping you would have included a bit more information about the skin recovering. But also addressing other side effects. I had radiation for four weeks. About five weeks in, 7-10 days after final radiation treatment, I began having intense itching that was almost pain and also some shooting pains, all within the breast itself rather than on the surface. I spoke with my radiologist about this and she confirmed that this is a possible response to radiation, although apparently not very frequently seen. I was hoping for, or would be hoping for, more information about post radiation symptoms and responses, besides just the skin itself.

Thank you for your feedback. I will do a video about the expected effects of radiation that occur after treatment. Look for that video in the next week. Just to answer a couple of your concerns, it is common for the skin redness and irritation to peak 5-7 days after radiation. And the itching you describe sounds like folliculitis (irritation of the follicles in the skin by radiation). Sharp shooting pains are often related to nerve fibers in the breast that were severed at the time of surgery and are now recovering. The recovery of those nerves causes sharp, shooting pains that last a couple seconds and then go away. Radiation irritates the nerve fibers and can increase the sharp shooting pains. Hope that helps!

What if after surgery they also remove 2 lymphs because it was effected the first one 2.5 cm and second 1 mm . Is both chemotherapy and radiation required. When they said no more left in the body . Also asking to have pet scan done ???

What do these words indicate on MRI report: "There are innumerable additional foci of enhancement throughout the lower inner and lower outer quadrants of the left breast which demonstrate predominantly progressive kinetics."

Hello Dr. Deming. Thanks for your informative video. FYI, I just had my lumpectomy 2 weeks ago and my oncology had planned for radiotherapy after the surgery. I wondered when my radiotherapy will be held generally..since my onco keep telling me it will take sometime after the surgery and he is not explained to me in details that make me worry and anxiety 😅. By the way I am now having this seroma pockets fluids under my right armpit which I need to go the hospital to drained it out every 3 days.

Typically radiation starts about 6 wks after surgery, although it could be anywhere from 4 - 8 wks. Seromas often reaccumulate if they are drained. Sorry to hear about that. Wishing you the best in your healing and radiation.

I was diagnosed with DCIS in August. I will have my appointment with the surgeon this Friday. I was told that she might recommend freezing to remove it, but I haven't seen any information on that method. What do you think? Mine they told me s is a stage 0. My family wants it surgically removed. That is what I think I will ask for.

I have not heard of cryotherapy (freezing) for breast cancer. Sorry, I am not sure.

Thank you. I dont want radiation for my DCIS. Had lumpectomy, feel it will do more harm than good! Low risk. What is the most accurate way to detect recurrense..ultrasound,MRI or Mammogram? Do you think every six months would be good?

A month ago I was diagnosed with breast cancer. My tumor was 1mm. It found very early so they could not stage it. Yet they removed my breast. My lymph nodes tested negative

I had a bilateral mastectomy (nipple sparing with recon) for a 1.28 mm tumor (invasive ductal carcinoma). Zero regrets! I also had LCIS four years before that, and family history. Cancer is cancer no matter how small.

I wish drs would talk more comprehensively about the progressive damage that radiation inflicts on the skin. It is not the benign treatment as it’s described as according to my surgeon who has to reconstruct irradiated breasts which often have wound healing issues. Also I could not have radiation per my rheumatologist as I have scleroderma and radiation for scleroderma patients has been directly implicated in the development of a different cancer down the road. I opted for a bilateral mastectomy and I don’t need to ever have another mammogram. I did immediate flap reconstruction.

@@mhygdistribution3207 I just realized this comment was for DCIS, not invasive cancer. The only indication for radiation after a mastectomy is positive margins. If you have a mastectomy for DCIS and have clear margins, radiation should not usually be needed. Hope that helps.

My radiologist said the risk of reccurence after radiation is 5%, my surgical oncologist said she is seing the risk of reccurence after radiation for DCIS is 1%.

My wife is diagnose by DCIS, now need to do radiation can you please tell me. which radiation technic is good for DCIS between IMRT and IGRT. because as per my understanding IGRT is more used in tumor and DCIS is not tumor. so i can go with IMRT. Please suggest.

You don't need IMRT for breast cancer. It would be 3D conformal planning. IGRT is not a type of radiation. It just means that you are using imaging to guide the therapy. The main think I would ask about is if she has left-sided cancer. If she does, I would make sure to ask if they can do deep-inspiration breath hold treatment to spare her heart from radiation. What about a grade 2 tumor that is ER+ and larger 4.5 - 5 cm but hasnt spread to any lymph nodes? with the DCIS we are doing chemo now in hopes to shrink the cancer. You mentioned the size over 2.5 would maybe require radiation. What if the chemo we are doing shrinks the mass to below the threshold for need of radiation based on size?

This video is for DCIS. Chemo is not given for DCIS. So, I am wondering if you have mixed invasive carcinoma and DCIS? If so, this video does not apply to you. In the setting of invasive cancer and DCIS, radiation is usually recommended.

I found radiation awful and it left my breast area, shoulder, arm and under arm weak and painful. Or was it the lumpectomy? From tumour testing my tumour was low grade and indolent. I wish I skipped radiation. Nobody offered me options. Also the radiation givers have terrible people skills. It was just so awful everyday dealing with disorganised people who made it clear they did not know or care about my experience. I hope it’s better than my experience with the Epworth cancer factory in Melbourne.

The recommendation for margins on DCIS is 2mm. So, most surgeons would recommend another surgery for 1mm margins on DCIS.

Yes, there are ways to stay off your heart. Ask your doctor if they use any techniques like deep inspiration breath hold to minimize dose to the heart.

What if it’s high grade but a small area? None of the videos are addressing this. I guess it doesn’t matter if it’s high grade? No matter what you need radiation despite gene testing and oncotyping?

Get the radiation. It’s very low risk to your health. It doesn’t change your odds much at one year out, or five years out. But it increases your odds of being alive ten years out, by 25%, which is significant.

I had 4 lumpectomies to get clear margins of 2mm DCIS. Then radiation. Nervous that recurrence risk is greater with 4 attempts.
