It kinda speaks to her talent as courtesan, I think. Like, she had some cues, namely Sakura and Saki's rhythm when talking, and Kotaro's beatboxing, to go off of, and improvised something based on only that. Which I find impressive. That kind of feat requires analysis, attention to her surroundings, and creativity.

This might be the first time a freaking _shamisen_ music instrument was included in a rap piece. A shamisen is as traditional as it gets, dating from the early-mid Edo Period (17th century). Definitely a Shamisen. Biwa's are typically 4-stringed, while Shamisen's are 3-stringed, and as you can see, there are only 3 tuning pegs which means 3 strings.
