I did once on a second playthrough with the same character because I used that Blood Pack pistol that shoots only one bullet at a time. I got only two shots on her, that wasn't enough. I had to reload an older save because I was stuck. They fixed that in the LE.

Imagine being on the team of a cutscene so easy to avoid you know that, unless someone intentionally fails it, no one will ever see it.
