Physiotherapy exercises and techniques can help rectal prolapse treatment at home when the rectal prolapse is mild to moderate (mucosal prolapse or inner lining) and after rectal prolapse surgery to reduce the risk of recurrent prolapse. When the rectal prolapse cannot be reduced (repositioned) this condition usually requires rectal prolapse surgery.
1. Stool Consistency for Rectal Prolapse
Managing stool consistency in a priority for rectal prolapse treatment at home to avoid straining and constipation. Ideally your stool consistency on the Bristol Stool Scale should be Type 3-4 (soft and well formed).
Type 1 stool (Bristol) is hard and lumpy increasing the likelihood of constipation and straining. Osmotic laxative and/or dietary management can improve bowel emptying.
Type 7 stool (Bristol) is liquid increasing the likelihood of straining and bowel leakage. Medications including antidiarrheal and natural products such as psyllium husks (1-2 teaspoons daily) can improve stool consistency and reduce fecal incontinence.
2. Bowel Emptying and Rectal Prolapse
Avoid straining with rectal prolapse. The brace and bulge bowel technique reduces straining provided the stool consistency is correct (Type 3-4).
Position
* Sit and lean forwards
* Keep the inward curve in your lower back
* Legs apart
Technique
* 5-6 relaxed deep breaths
* Brace and make your waist wide (say 'M')
* Bulge your lower abdomen forwards (say 'OO')
* To push further say 'M' again
* Lift and squeeze your pelvic floor muscles
* Make your last wipe a wet wipe
3. Rectal Prolapse Exercises
a. Technique
Kegel exercises train the muscles that hold the rectum in position. Start these exercises lying down or sitting. Before starting reduce your prolapse internally.
Step 1
*Imagine you’re trying to stop passing gas. Squeeze the back passage (anus) and lift inwards.
*Relax these muscles.
Your buttocks should stay relaxed throughout.
Step 2
*Squeeze and draw inwards as if to stop the flow of urine. Men try to imagine you’re retracting your length just like a turtle pulling its head back into its shell. Women try to lift and squeeze inwards around the 2 pelvic openings at the front at the same time.
*Relax these muscles
Step 3
Combine steps 1 and 2 and breathe normally with rectal prolapse exercises.
b. Rectal Prolapse Exercise Guide
* Contractyour pelvic floor muscles using the correct technique
* Hold your contractions for the length of time you can manage
* Hold each exercise for up to 8-10 seconds
* Perform up to 10 exercises in a row (1 set of exercises)
* Perform 3 sets of exercises daily (up to 30 contractions)
Progress the strength of your exercises with rectal prolapse exercises when standing.

Thank you so much, I suffer from constipation and a lot of stomach and pelvic issues. I recently started leaking from behind, like a colorless odorless fluid n sometimes blood. Endoscopy was fine🤷🏻‍♀️ I’ll just see what my doctor says but this really SUCKS!!! I’m sorry to anyone going through this 🙏🏼

Thank you for sharing. I'm curious if anyone explained leaking liquid? I had endoscopy and colonoscopy didn't reveal anything major. I have hemorrhoids and pelvic organ prolapse and bowel movements are a challenge.

As a retired health professional, I really appreciate your straightforward sensitive approach, and am sharing your videos with everyone I know who could benefit from them! Hi--I've been to Australia while in the army ! You are so informative & not shy in expressing some things non-meds can't talk about , although needed to know ! The topics you explain are very important to people . You never mention the causes of these maladies though ! In todays cultures sexual practices , such as anal sex , fisting , FGM , pressurized enemas etc. ; that In my opinion , add to the increases of future pain & suffering to patients lives ! Many people think only surgery is the answer ! I was always a fan of natural & healthy nutritional cures , & still am ! Never give up--- Lots O Love- 💖 Ed !

Hi I’m sorry your going through that, I have muscular dystrophy and Severe constipation, I thought I had hemorrhoids for awhile but I recently pressed on it and felt like my intestines moved, very strange feeling, I looked up the difference between hemorrhoids n rectal prolapse.🤦🏽‍♀️ making appointment right now. Thank you for this video. I've been dealing with a mild prolapse for a while, exacerbated by bleeding hemorrhoids on occasion. Not a fun experience. Will practice these exercises and hopefully minimize these conditions.

It did help me immensely!!! I didn't know why my bowel movements changed after discovered i had a mild prolapse in my older years. Never heard of prolapse before in my life. so after seen the doctor she told me something about my uterus bends to the left towards the last part of the colon. . I didn't understand anything as the word prolapse was the only thing on my mind. She did tell me i was lucky to get it so late on life as young ladies have it... ... I went to see my gastroenterologist he recommended a colonoscopy so I did ... passed with flying colors and gave me peace of mind. your information was right on target great exercises...👌👌

@@Yashsleeps to be brutally honest I’m 54 and I still have trouble with it to this day. The way it started is pretty much unbelievable to some. I saw a full apparition of a ghost in my sisters house. It shocked me so much that I ghasped so hard and my abdominal muscles were stuck. Tense and all lifted into my sternum area. I started having trouble ‘going‘ straight away and my confidence was knocked out of the ball park. All my peak years I spent stuck indoors cos I wouldn’t socialise. I couldn’t have a relationship because of it. My best advice to you is don’t shut down and go into yourself because of it. Open up. Talk to your doctor. Get it sorted right away cos the years I missed I could cry. I even tried to take my own life because of it. Pls pls see your doctor asap. It’s a simple operation done by keyhole surgery. Nothing to fear and nothing to be ashamed about. I wish you all the best and pray for you that you get it sorted. God bless. 🙏🏻❤️🙏🏻

Hi I’m glad these are helpful. Splinting is fine for added support, I don’t see an issue with it provided this is accompanied by abdo bulge to relax the anal sphincter. Foot stool is very individual and height dependent. Really only of benefit with short legs to get knees higher than hips. The same benefit can be gained with a couple of large books too for those small in stature

Thank you so much! I always come to your videos for my problems. Always helpful. Since about a week, I am facing some rectal discomfort and pressure, very slight pain but it bothers me quite much now, even more when I think about it. Now I'm trying to find what it is and I hope it gets fine. No blood in stool or anything else but I do have constipation and stomach problems. Do you have any advice regarding this problem?

Hi Fatima try to keep your stool consistency manageable through your diet, plenty of fluids, avoid straining to empty your bowel and see your doctor if this doesn't settle down within the next week.

I have suffered this to. It went away completely last year after realising I had been sitting too long daily on a hard chair. I’ve just been suffering from it again, in a different house, after a time of terrible stress and again sitting around too much on hard furniture. Sometimes pressure on the tip of pelvic bone can cause pain to come out in the rectum (don’t ask how! I was told this by a doctor). Hope this helps!

PLEASE hear my story. I had rectal prolapse caused by IBS and bad guy bacteria. I was told there's no cure and I need surgery. I then reduced my Wheat foods and sugary foods. Increased vegetables and sour fruits. For Carbs I eat Oatmeal, Potatoes and Rice. IMPORTANT. I'm drinking 2 Tablespoons of Pure Olive Oil first thing in the morning. I make sure I drink warm water after every meal. It's been 6 months and my Prolapse is completely gone. Thank God.

I think I might have mild prolapse or maybe I do have it fully and don’t know but when she explained how to go to the bathroom, I already do that… like that’s how I go to the bathroom and hav even for a while now cause I’ve been dealing with constipation and issues with pooping and that’s literally how I poop

I find that I’m most often straining trying to fart rather than trying to pass stool. Would you recommend that same sitting method for each time you pass gas?

The problems I have is that I have nerve damage that affects my pelvic floor. I use the wet wipes. But there are parts of that region that I cannot feel, thus cannot contract. I’m 2disabled due to a 16ft fall. I’ve have been battling this for just over 10 hrs...The Kegeals aren’t cutting it.

Sounds good but what if you have rectal prolapse with internal and external hemmroids? I was told to use sugar down there to shrink the prolapse? Also I have gastoprisis and spinchter of oddi dysfunction. So I can’t have normal foods. I’m nutrition deficiency. Fish chicken turkey baked potatoes butter rolls. Water that’s about my limit on food. Can you help me with it still? Thank you. 😢

Surprised you didn't mention bidet water cleansing, or at least using a perineal care squirt bottle after BM's. Very soothing not abrasive like a wet wipe. Not everyone can afford a bidet, but a portable perineal care squirt bottle works very much the same. Just something to really consider

Very helpful. I have mild prolapse and definitely do not want it to worsen. It's odd how something that isn't technically life threatening, is even treatable if need be with surgery, can be such a source of anxiety! I actually suffer more discomfort from sciatica, but I'm way more obsessed about this, lol.

I have the same problem and wondered if it was possible the sciatica was actually causing the condition as I didn't have regular bowel problems before the nerve pain came along. It drives me insane as i struggle to sleep because it always feels like I need to go for a number 2 (but can't) as soon as I lay down to sleep

Is there a relation between anterior pelvic tilt, pelvis behind centre of gravity, and a weak posterior pelvic foor? Where could I find more images, literature, animations about the biomechanics or how this affects movement.

Yes Kti there is a relationship indeed - check out Ruth Sapsford's research into spinal position and pelvic floor activation. Having anterior pelvic tilt decreases the effectiveness of pelvic floor training. Neutral spine is optimal

I don't know what's wrong with me, please I need your help. This is what happening to me for the past few days. I can't poop peacefully, like when I poop I can go all out and I feel it's stucked inside. I remember holding my bowel for a while, and when I stood up I had an unbearable pain but it went away.

I have a prolapse for 2 years and have been trying to make it better with exercise and diet. I am at the stage where I need medical intervention, which I'm very afraid because I don't know what to expect. I was also told if that doesn't work, I might end up having my bowel removed and replaced with a bag. I'm so worried about it, I also think I've been doing exercise that's making it worse.

Hi Michelle, I am facing some rectal pressure, discomfort and slight pain since about a week. (I earlier mentioned my problem here in the comments and I don't feel this problem when I sleep). Now this new problem is making me quite worried and much stressed as I've been dealing with numerous health issues for years, adding on another now. I'm hoping this goes because I already have constipation and stomach problems, occasionally blood in stool and it gets hard for me to manage. Please help /or any suggestion...

Hi Fatima yes I recall reading your earlier comment. It sounds as though you need an endoscopy to check out your bowel - this is the best way for you to know if there is any problem and then how best to manage it if there is one. Stress and worry can make bowel problems worse too so having a medical assessment will take the guess work out of this and ease your mind too, all the best

Hello & thank you for such a informative video. I have pressure and pain. I have not been diagnosed but I feel around my anus what could be my rectum Protruding( almost lumpy ) but it’s not out. I sit all day at my job had a big baby almost 10 pounds 30 yrs ago and it also burns when I move my bowels. No blood. Never had hemorrhoids. Could these things cause prolapse ? And will exercises and soft stool help my condition from worsening?

I was recently diagnosed with a rectocele and was recommended for pelvic PT but there is such a waiting list for an appt in my area. I'm looking for helpful exercises for rectocele and hoping your tutorial is an answer.

Rectocele is a prolapse or bulge of the rectum into the back wall of the vagina. In contrast, rectal prolapse occurs when the tissues inside the rectum protrude out of the rectum. Yes D R this can definitely be part of the problem. Try to keep your belly bulged and pelvic floor relaxed throughout plus the stool needs to be not too hard

When I sit down to poop, blood comes and at times I feel something hanging down my anus, I thought maybe it's a poop unwilling to come out, but it used to come daily and when I pull it goes back inside.

Michelle, is it possible that a retrograde uterus could be responsible for a lifetime of difficulty passing stool? No amount of fibrous veggies, psyllium husk, or stool softeners have helped. One successful year of dieting low fat/high fiber (~70 grams day, mostly from beans) was the only time I had relaxed bowel movements. I'm starting with chia seeds now, but would be really interested in maybe solving this personal mystery. Thanks!

Hi jk retrograde uterus can be associated with an increased risk of vaginal prolapse because the uterus sits above the vaginal opening however I don’t know of any association with constipation. Have you ever had a stool transit test ? Some people have slow motility through the bowel which can be caused by a number of factors including elongated bowel. Chia seeds will bulk so from what you have said I’m not sure this is your solution. What if you don't get normal urges anymore, laxatives don't work, make you worse, you have almost all the types of stool in the chart with one stool, you have stool trapping pieces that are flat hard and ridged in jacked up shapes getting stuck from what it appears somewhere in the inside prolapsed rectum, also dealing with other types of prolapse in pelvic region, also having issues with ileo cecal valve, cant find a Dr to listen or help

Hi Michelle, I am having a combined hysterectomy (probably total) and an anal sphincteroplasty in 3 week's time. I can't find any advice on exercise etc after the sphincteroplasty. I am worried about coping with bowel movements afterwards. Can you help me?
