Reiter’s Syndrome
Reiter’s Syndrome is a type of reactive arthritis that happens as a reaction to a bacterial infection in the body. It is characterized by urethritis, conjunctivitis, and join arthritis. Urethritis is inflammation of the urethra with the most common symptom being painful or difficult urination. Conjunctivitis is inflammation of the outer membrane of the eyeball and the inner eyelid. The eye itself is not infected, however the immune system is acting as if the eye is infected. The knees and the joints of the ankles and feet are the usual targets of reactive arthritis. Fluid culture is negative for infection. The infection enters the body and triggers the immune system. The immune system creates antibodies. These antibodies attack the infection. These antibodies then cross over and attack the normal body cells. This is why they call it reactive arthritis. It reacts to the infection that the body already went through. It usually is associated with infection after the initial infection is gone (residual immune effect). GI infections such as salmonella and shigella and GU infections such as gonorrhea and chlamydia are often infections associated with Reiter’s syndrome. Some people are more susceptible to get Reiter’s syndrome than others. Most cases resolve within weeks but may last longer. Recurrence occurs in about 50% of the patients. Reiter’s syndrome usually starts 1-3 weeks after the original infection (up to 45 days). The patient is usually seronegative spondyloarthropathy which is in the same family of disorders as ankylosing spondylitis and psoriatic arthritis. 75% of patients are positive for HLA-B27. Remember that in a test exam, they may test you on a patient with Achilles tendon pain (Achilles tendonitis) with heel pain (plantar fasciitis) and it’s associated with Reiter’s syndrome. Also remember that Cipro (antibiotic that is used to treat bacterial infections) can cause rupture of the Achilles tendon. Do symptomatic treatment and observe. Use antibiotics for the underlying infection and steroids for severe cases. Reiter’s syndrome is not gonococcal urethritis.

This is likely what I have right now. It started with an intestinal infection, then I had trouble urinating. Three weeks later and my knee became inflamed, along with my big toes and tendons. Swollen eyes in the morning. Thankfully, after 5 months, it's subsiding, and isn't constant anymore. It tends to go away after 3pm, and each day shows improvement. So if any of you have these symptoms, know that there's hope! Note: It may appear to be symmetrical at first, but symmetrical swelling can be caused by compensation injuries accompanied by lesser initial swelling. That has been my experience.

Been struggling for +6 months with severe attacks of oral ulcers & red eye attacks, along with bilateral head/face/neck/intraoral edema+ worsened joint pain & stiffness. 30yoF with existing complex medical history & immunosuppressed from steroids- I’ve been completely fobbed off, told it’s my ‘normal condition’- which I was born with. I’ve never had these symptoms. Why now at 30? Attacks first started >12 months ago with a markedly high ESR. For the last week I’ve had what I thought was a severe UTI. Woke up at 5am with gross haematuria including large blood clots, & the most horrifying pyuria that I didn’t know was humanly possible. I haven’t been sexually active in years, & had STI testing after my last partner. Today I saw a different GP as I couldn’t get in to mine short notice & after an extensive history, two urine samples including mid stream, & a vaginal swab, he asked me “you don’t get mouth ulcers do you?”. I told him I do- I’m actually finally seeing an oral medicine specialist this week for that very issue. “What about red eyes?”. He then mentioned this. I’ve begun antibiotics in the meantime, & will receive initial results later this week. Thank you For this video for the information & reassurance. Thank you also to the other commenters, I feel much more hopeful & less frightened. I’ve left this comment in the hopes that I can also become a story of hopeful recovery to others.

Thank you for your video, it was very informative. I strongly believe I have gout, but my doctor tells me I have Reactive arthritis. I started noticing symptoms when I ate red meat for more than 3-4 days in a row, my joints hurt. As soon as I stopped eating meat the pain went away. This went on for approximately 1.5 years until my Dr. finally believed what I was telling him. There was something you said in this video that was quite interesting, the heel of the foot. My heel and the calcaneus cartilage hurt me. One day, I could not sleep because my right hand on by my thumb started to itch, I scratched it all night, I thought something. It me. Two weeks later the same thing happens to my right hand in the same area. At this point I thought it was something else. Since this occurred, when I go to sleep my hands cramp up causing me to wake up and lift them and open and closed my hands to get some relief, but when I'm up my hands are fine throughout the day. I recently had a herniated disc with a pinched nerve (sciatica). I don't believe my doctor's diagnosis because, the blood work that was done came out negative for both arthritis and gout. Yet, the ankle on my left food swells and now after eight months the top of my write foot became, first itchy and then red, hot and also swelled up. Leaving me with a mark of where it was swollen as though I had been burned. Do you have and explanation for this?
