The Airvo 2 from Fisher and Paykel is an excellent device for delivering high flow, heated oxygen therapy. It is can be used in all areas of healthcare from the intensive care unit to the patient's home.
Heated high flow therapy is becoming much more prevalent in the management of dyspnea in chronic obstructive lung patients. High amounts of oxygen can also be delivered with this device non-invasively to treat early hypoxic respiratory failure.
--Keys to therapy--
1. Document the FiO2 (most important for stability)
2. Document the total flow from the Airvo screen.
3. Wean FiO2 first
4. Wean Flow last

We recently got this device as well. How is it working out with your oxygen consumption in relation to long distance patient transport? The suck around the office it's going to drain tanks like crazy. units having to do road side main tank changes. Any information is appreciated.

I had a pt with acute respiratory failure on ABG who was DNR and wasn’t a candidate for BiPAP, so I tried AirVo and 3 hours later his ABG was PERFECT 🤩

That's great! It's hard to explain how it ventilates a patient, but it may assist resps on a patient in resp distress.

Please review a very basic PFT test during normal inspiration, an adult routinely breathes at a peak inspiratory flow of about 1L/sec (60L/min). I agree that the average inspiratory flow is about 20 - 30 L/min but if we need to exceed the peak inspiratory flow we will need a higher flow rate?

These are nice units to have at home as a patient, although they are quite expensive. Very simple to use, and helps the work of breathing and the humidity is nice. You must make sure to keep your mouth shut so the flow can do its thing, which is difficult in the first few minutes when you're SOB but just stick it out and you're good.

Excellent. Very relevant in 2020 A practical aspect is the management of oxygen supply which is very high when large number of patients are connected in a Covid-19 hospital. One can refer to a video " HFOT Calculator" on KZfaq as a supplement to this nice video.

On the AIRVO 2 there is the alarms menu where you press and hold the alarm silence, up and down arrows at the same time. Are there any other extended menus, and how do you access them?

studies show that HFNC is not inferior to CPAP. Some of the studies go onto to say that that one has not been proven to be better than the other. Per Fisher Paykel

He didn’t say it was inferior to CPAP. He was comparing to Bipap to decrease CO2. I’ve heard those studies from the reps and I think it’s a fantastic device, but it has its limitations. I think that this device really shines in situations where you have patients who cannot come off bipap to a high flow cannula due to extreme flow demand. It gives them the ability to take breaks to eat and drink.

Saddle PE, that's a serious diagnosis. It probably prevented some of the thoracic pressure changes that may have happened if you were on CPAP.

How would the Airvo 2 cannulas adapt to the max Venturi circuits? We are having issues adapting our Max Venturi for to the airvo 2's that are becoming more and more popular in the hospitals. Any ideas/products you could point me to? We are ordering RAM cannulas now to try to get a better fit for some of the patients. Any experience with the Ram cannulas?

We use RAM cannulas for neonates on a F&P circuit here. As you have found the airvo circuits have a proprietary adapter for the cannulas. There are a few different types of cannulas from Resmed that hook to the regular max venturi circuits. They are comparable to the airvo cannulas.

So what is the difference between Airvo2 and giving our patients plain oxygen, usually from the wall, at the hospital. I am a new nurse with COVID patients who are given this machine and I am finding this concept difficult to understand. Thanks very much!

You are in the same boat with many clincians. High flow oxygen therapy is a very new concept. The Airvo and Vapotherm devices deliver very high flows (20 to 60 lpm), but they also heat and humidify the flow to 37 degrees and 100% humidity. This allows for the toleration of this high flow by the airways. These high flows also create a small amount of CPAP which helps to increase oxygenation.

My husband is currently a covid patient. Is it supposed to fill with water? It’s making a pumping sound.

The heater area is filled with water to add humidity. In the hospital, it can be feed by a bag of sterile water or a port that opens up for a manual pour of sterile water.

I leave the cap closed. I've not tried using the Airvo with it open.

Student here, Love the video, we are just learning about this in class. Sounds like a stupid question, but I get FIO2 is O2 but the flow number next to temp, (LPM) is what, room air ?

That is the total flow of to the patient. FiO2 is the amount of that flow that is oxygen. The rest is assumed to be nitrogen or room air.

When you wear it, you'll believe it 🤣 Beside expansion of the circuit, I'm not sure how it could lose flow from the machine to the cannula.

@@rtclinic thanks for the reply, can you tell me what would be the minimum flow that's supplied to patient at the end of the cannula when the machine is set at 60LPM. Or since there is a gap between the cannula end and to the nose there will be a venturi effect and thereby supply of more air to the patient

Is there any update on this?? Hehehe this has been our problem fio2 limit is just 21 to. 95%, and our patients needs 100% tho the manual says if this alarm is activated its either your fio2 is bet 95 to 100%

You will access the advanced menu screen by holding the following (3) buttons at the same time for 5 secs: alarm mute, up and down arrow. Use the menu button to cycle to the third setting, hold the up/down arrows to unlock, while the values are flashing press the menu button 4 times to progress to the flashing '95'...press the up arrow to change to '100'. Hopefully this helps!

Love the videos, Jimmy! Question: Used a 10 L concentrator with the Airvo but as I turned up the flow, the FIO2 wouldn't increase. It stayed at 21%--even at 10 L. Do you happen to know why? Thanks, man.

That doesn't make sense to me either. When with a total flow of 60lpm you should still see a bump in your fio2 with the concentrator... I would check two things... analyze the fio2 on the concentrator to verify it is working then change the o2 sensor on the airvo.

There is an optional backup battery and mounting bracket. Its around a $2000 upgrade but offers nearly 2 hours of transport time

What is fault E157 and how do I fix it?

The temp at the patient end of the circuit is too high. The manual recommends turning the device off and restarting, if that doesn't fix it change the circuit. I would first check to see if there is a fan blowing on the circuit causing it to heat up too much.
