Sacroiliac Everything! SIJ pain and dysfunction treatment options explained in every detail. Today I take apart the sacroiliac joint and explain it to you. I’ve suffered from the problem myself I put thousands upon thousands of needles into this joint and around it. I’ve done surgery to try and transfix it. I understand this joint better than perhaps any treating physician because I myself live with sacroiliac joint dysfunction. The purpose of this video is to take the relatively naïve suffer and educate them about how the joint works how to use physical therapy and chiropractic Manipulation to get out of pain, how to shore up the joint and strengthen the muscles surrounding the joint and then what are the risks and benefits of all the procedures that treat the joint. Hopefully this helps!

I have been fighting this problem on both sides for over 2 years. I’m scheduled for a fusion in 3 weeks. Your video was by far the best information I’ve received from countless medical specialists. I've become a victim of Doctors I have these symptoms including the sciatic pain down to my foot so after 2 discectomies and one Foraminotomy a 2 level fusion with no relief and all the doctors that i have seen not a one of them have ever suggested or even looked for SI joint problems . So last week at a year follow up dr appointment I suggested an injection to the SI joint so I'm still looking for answers. This a great in depth video that i wished that I could have seen 5 years ago. Getting the right diagnosis for your pain is so critical !!

I was looking for a second opinion before rushing into steroid injections and surgery. This is the video that sent me across the state to see Dr. McRoberts. This is exactly how he is. He is patient, thorough, and explained my MRI to me in a way that I could understand. Never rushed me and didn't encourage the use of steroids or other injections. He gave me exercises to do at home and a Rx for PT. He was so encouraging. And I really appreciate that he takes the more conservative approach of helping the body to heal itself before heading in to more agressive treatments. I can honestly say he relieved my fears of what was happening to me. If I ever have other spinal issues he will be the first doctor I see.

That's what I thinking about 7 Ortho appointments later I'm Googling my MRI....I left every appointment with more questions than Answers😔 not only do i have scrorilitiis i have a lot going on in my spine especially L4,L5 year's of disc high loss and bone spurs ect..I was thinking last night i wouldn't want to tell someone all this and can't help..I'm very happy you got exceptional care and are doing better!!!!

Wow, thank you! The nore we know, the better we can describe our pain to our doctor. We can even figure out our triggers, which also helps tell our doc what to look for to get us better.

Thats exactly what Baptist Health Hospital in Little Rock Arkansas did to me back in 2015 They fused L5 To S1 Now i i have a whole new set of pain-related issues, Thats why ive been binge-watching your videos

Sorry to hear this. Did you ever have relief after the surgery? I know that this seems to be the problem with fusions. They eventually cause problems above or below the fused area, and then you have to have another surgery. I was wondering how long of a time one gets relief from pain before another surgery is needed. Really sorry you are having this.

35 year firefighter/EMT. Recent fusion both sides. Still have issues. SI injections even after fusion. The injections so far are giving relief.

I have had arthritis in my SI joints for over 15 years, and get bilateral injections once or twice a year. I have had 3 spine doctors, and this is the best explanation I have ever heard about this condition, so thank you!

8 month's😢...When you just said you understand the pain, Debilitating in every way possible i felt that no one understands what I'm truly going through I've had my MRI but they think it's bilateral it's bin my R side since August 2023 am going for a CT But NOOO relief from the SI targeted injection and my insurance is not great i just wanting to stop and say it was nice to hear a Doctor who understands!! Back to the vide.

Thanks I have a better understanding of this and can speak to my Orthopedic doctor we are working together to find the best treatment for me I am a healthy 75 and still active and we all agree it’s the quality of life

Thank you for making this video. I just had L5-S1 fused. We knew I had hyper unstable SI joint I have had injections and they help from the lidocaine. The neurosurgeon said we would wait until March 25 to do my fusion. They have ordered a SI joint injection. I don't know why PRP hasn't been mentioned. I'm going to PT to try to avoid this SI fusion. My last ablation in January didn't help at all this time.

Best descriptive SI joint knowledge I’ve heard. After two laminectomies and two lumbar fusions, I now am suffering with debilitating leg pain from the knee to the ankle only when I first wake up!

Wow. Doc, I just ran across this video by accident. I've been suffering with SI Joint issues for years now. And, it truly IS debilitating. I'm about to have another ablation. Last one I had lasted for quite a while. I'm about to the point of asking my neurosurgeon about fusion. Unfortunately, my whole spine is a mess. I had a multi-level lumbar laminectomy several years back due to extreme stenosis and am now losing some stability there, so looks like a multilevel lumbar fusion is the cards. I also just had a multilevel fusion of about all of my cervical vertebrae. I think I'm truly on the way to becoming a $6 Million man. Anyway, I haven't had it suggested to inject the lubricant into the joint. I believe I will ask about that next. This was a great video, and I really appreciate you posting it.

This all describes me and I have a scoliosis and leg length discrepancy. After a total knee replacement I exhibited all those symptoms- the MRI show’s stenosis etc BUT thanks to you I will see if I can get a differential dx . Thank you SO MUCH!

If Only. Unfortunately, I have experienced just about all. Mine is a complicated 45 year history. Successful surgeries, but time has created its own problems. My issue is my right S1 joint has acted up. Very painful groin pain and numbness at the front of leg. You have given me valuable information😂 Thank you.

You have answered so many questions that I have. Very interesting about the sleep issues and pain in shoulder etc. I have all of this. Four years ago I had L2-3, L3-4 laminectomy and L2-3 discectomy. In January 2024, I am now getting L2 thru S1 fusion, spacers with a cage. Degenerative osteoarthritis has made a mess of things.

Pelvic instability is so bad that I'm using a wheelchair. It buckles with the smallest movement. It feels like I'm on a boat just sitting. Car accidents (tboned 2x), falls. It buckles and I fall. Usually on my butt. That's why I use a wheelchair. Stabbing pain used to cause my legs to buckle making me fall. I decided to use a wheelchair to avoid the pain and falls. I've done a year of PT, pain shots, imaging. And the doctor who was going to do ifuse just left the hospital. So I'm waiting for a different neurosurgeon to evaluate and schedule. It's like my upper half and lower half work like normal independently - but they don't feel connected in the hips.

Very enlightening. After many back treatments followed by 2 spine surgeries, finally getting a better idea of the importance of what PT I should have been getting much earlier. I can no longer live independently and use a Walker to get around. Been taking daily injections to increase bone density for over a year with intermittent physical therapy thru out. Still found that my hips were no longer in the right place to hold up my back after surgery. Trying to have intense physical therapy now, to see if I can ever be ready for surgery.

Omg it was very good explanation. I was told i need L2 to L4 laminectomy with right sided fasectectomy, but my extremely pain is from my SI. I have exactly all the symptoms you mentioned and going to the bathroom is my big nightmare....

If it is possible for the SI joint to be out of place, how does a surgeon make sure it is correctly aligned before fusion? Also, the pigeon pose (Yoga) that some people recommend for SI Joint relief would be moving the ilium in the opposite direction it should be going, right? I get some relief by doing the opposite. Forcing my knee behind me, hips as far out in front of my knee as possible. Great explanation of this painful condition. Thanks.

Fantastic!! Been watching various utube vids on low back pain and SI pain from Chiropractors. But this explains it so much better. Having rodeoed professionally, ive used DCs for many years. Most really helped various pains. But, old age has resulted in a very painful hip that i just feel is from lower back or hip muscles rather than actual hip problem. This was a great explanation of how things work together. Also wathed your utube on lower back probs. I wish you could demonstrate on yiur table what not to do and what to do when sleeping. Same for the hamstring stretch. The language is a bit confusing ie, "throwing leg over", "sleeping on side with knees not together", etc. And for hammy stretch, "back in extension" does that mean concave or convex?

Ive been suffering from this horrible pain for 14 years. All my Mris and X rays come out clean. I just want this godawful pain to go away. It's robbed me of my 20's, a mans prime. My injury happened after lifting something. Life is too short for me to live it this way. Its 3am and cant sleep 😔. My mental strenght to go hold on is chipping away.

Have you been to an Orthopaedic Doctor? I go in July to ck out my options. I TOTALLY UNDERSTAND about quality of life. I've been living like that for over 15 yrs. Gets old!!! I had to pursue things myself and totally bypass referrals.

@Merri1961 yeah I went to 4 already. None have figured it out. My chiro saw my mris and they're pretty normal. He mentioned that I have Si Joint dysfunction. I always have weird pops I'm my low back when shifting positions.

Thank you so much for this video, my doctor wants to inject the si joint to see if pain goes away well a month ago I just had an epidural which has helped me so much in getting rid of inflammation, I’m still not feeling a lot of pain except for on each side of groin still pain and a little achy back. I’m asking how will I know if it is the si joint if I’m still not having pain except for when I get in and out of car, going up stairs and standing too long. I guess that is enough to know if pain goes away then.

Again excellent! Thank you. What if you have pain in the area you are describing and sciatica. 24/7 . And, for 3 years non-stop. I have also been diagnosed with “severe facet degeneration of mid and lower spine” now. Two years ago mild. Haven’t been able to exercise due to this constant pain in left butt. 6 months PT- did not help. Certain exercise made butt pain to intense so stop trying to strengthen butt muscles. Over 60, and last year I can hardly walk when I get up in morning. Legs so hard to move. Gets better after 2 hours but pain in butt is so bad I have to sit on ice and heating pad 24/7. Go grocery shopping and very limited errandS. Can no longer do my 2 mile walks that I use to do everyday. It came on gradually after I prevented my mother from falling by holding her up when she passed out. Not sure if that caused it. Been to spinal surgeon. 3 years 3 MRI, XRAYS. Last MRI a 3 months ago ruled out stenosis, but have mild disc disease, last Xrays 3 months ago showed that Facet degeneration had gone from mild to severe over last 2 years. Please can you tell me what you think the non-stop pain in left butt in area you describe is? In this video, sacroiliac joint pain comes and goes when you move it out of place. Right? Not, constant. I had 3 epidurals 2 1/2 years ago: L3/4; L5 S1; Pimaformis. None worked. I just finished 4 weeks PT - did not help,and because I can’tdo exercises to tighten/ strengthen butt muscles they said I need to go back to the doctor. I am in the process of seeing another spinal surgeon. Before I could see him I had to have XRays, MRI, PT. -- should I insist on CT scan or is that necessary? And, do you think I have a problem with sacroiliac? Thank you kindly. I am scared I will end up like your mother. My physical condition is going down hill each year the last 3 and still haven’t been diagnosed as far as the left buttocks pain and sciatica left side. Would facet degeneration cause this. Usually, the pain in my lower back from it is more of a burning pain. Your opinion would be greatly appreciated. I wish I lived closer to you. I would come to you. I live on West coast. You are so good! So kind of you to do these videos. I have watched others of yours.

When i hurt my back in 2013 heard a pop in lumbar when i bent backwards real fast came back forward then knelt down to the ground and waited a little bit never felt a pain like that ever b4 i finaly stood up slowly and i seemed okay and was until 3 days later my (back felt hurt on lumbar left side) right leg drew up at my groin toward my chest and stayed that way for about 14 days before it finally released on its own the whole time oxygen being cut off to my bones anyway tho two days later after that release of my leg cramped up was my mri appt so i went and the report said dx Anterolateral (as one word) cramping. And i dont know what that means still to this day. Can you tell me what that is!?

Would there be any physical difference in feeling between a SI joint and a Facet joint? I have arthritis of the L5 facet joints, but only have one side having pain, so I’m thinking it might be SI joint (or both’) dunno

What does anterolateral(as one word) cramping as on my mri report dx mean. It happened in 2013 after i hurt my back real bad and i still don't know what that means exactly.

Do you know a doctor that you could recommend in the San Antonio or Austin area that has a practice like yours?. I have osteoarthritis & bone spurs in my S-I joints & getting the injection you described Synsec (sp)? or rooster juice is much more appealing than the fusion I have been recommended. Thanks again.😊

Try Dr. Kancherla: He is an interventional physiatrist specializing in the non-operative management of low back and neck pain, focusing on individualized treatment plans and therapy in Austin. I've heard good things, but don't know personally.
