It is common for patients to inquire if bracing can be used for adult patients, and the answer is that traditional bracing is not used on adults because these braces are designed to stop a curve from progressing over time. For adults, progression happens very slowly over their lifetime.
The only appropriate time to use traditional bracing is during rapid growth times, which applies to children and adolescents to slow down progression.
For an adult to actually benefit from bracing, the type of bracing used should be corrective. Corrective style bracing in adults works by pushing against the curve that's developing to reduce the progression of their scoliosis.
Corrective bracing can be compared to teeth braces. People use them to have straighter teeth and correct their smiles, and these braces work by pushing and pulling on their teeth to correct their position. This is the same thing that the corrective scoliosis brace does for patients. For example, traditional bracing is just like a retainer for somebody's teeth. It holds teeth in place but doesn't affect any change.
For corrective bracing to work better, it also needs to be paired with intensive care. A multimodal approach delivered in a condensed format of high dose and low duration is the best approach. This could be from 30 to 60 hours of care in a period of 1 or 2 weeks to get a rapid change.
Knowing the curve has reduced allows for more aggressive bracing, which equals a greater reduction of the adult patient's scoliosis. As the adult scoliosis is reduced, that also reduces pain.
Even though adult scoliosis studies are more limited as far as the origin of scoliosis than children and adolescents, we know that curves progress in the adult stage.
The two biggest factors for adult scoliosis progression are:
-How big your curve is
-How old you are
As adult scoliosis progresses slowly with age, what we see is what Dr. Tony Nalda calls ""A Dragon's Tail"". A curve that starts off relatively flat and increases faster and faster as patients get older.
We can decrease the effect of gravity over time by using the corrective brace along with intensive care. This will reduce the pain and the curve, and wearing the corrective brace in a full-time or part-time status to hold the spine down to prevent any further progress.
Patients are weaned out of their corrective bracing as their curves continue to reduce, similar to people who wear braces for their teeth. They go from full-time corrective bracing to part-time to just using it at night until it can be fully removed.
