Patients often have questions about scoliosis surgery, which can also be called spinal fusion. It fuses the affected vertebra into one solid bone using rods and screws to hold the spine in place so that when the bone grafts can fuse and form one solid bone.
The goal of scoliosis surgery is to stop progression. Results in terms of reduction are not guaranteed, although in most cases, they are making the spine straighter while putting in the screws and rods to hold it in place because scoliosis is progressive.
The most rapid phase of progression is during the adolescent stage, as they are in a growth phase. The smaller the curve is in the growth stage, the slower the progression will be in the adult stage. In adolescence, as the curve becomes bigger, it is likely to progress faster as you go through your growth stage.
Scoliosis is treatable but not curable. For conservative treatment to be successful, the smaller the curve and the younger the patient, the better the chance of positive results.
The goals of scoliosis treatments are to stop progression. The traditional approach funnels patients to this surgical option, whereas proactive approaches focus on nonsurgical treatment options to prevent a curve from becoming severe where to there is a risk that a patient needs treatment.
Patients in the traditional approach of treatment are told to watch and wait until the curve becomes severe enough that surgery becomes an option. The only time that it is considered treatable in the traditional approach is as an adolescent going through a growth spurt and if the curve is between 25-40 degrees. If you meet these conditions, they may recommend a Boston Brace to try and slow the progression. Other than this situation there are no other treatment options other than medication to help with pain but no action is taken to reduce the curve,
In conservative treatment, the curve is actively being reduced before they get to the severe stage so that surgery doesn't need to become an option because there are risks associated with scoliosis surgery. These risks are why the curve needs to become severe enough before surgery is recommended. The risks include excess blood loss, nerve damage, infection, and pain at the fusion site.
Post-surgical effects can include loss of spinal flexibility and range of motion, adverse reactions to the hardware, weakened spinal structures, and hardware malfunctions.
The worst case scenario in conservative treatment options is that the curve progresses and surgery is recommended, but the worst case scenario after surgery, the effects can even limit your quality of life.
In the best-case scenario in conservative treatment, your curve is reduced, and you never need to consider surgery as an option. However, in the best-case scenario for scoliosis surgery, you still have had surgery and need to deal with it for your entire life.
Trying conservative treatment to prevent the risks associated with scoliosis surgery would make sense. Even in the best-case scenario post surgery, you now have something to deal with that may impair the quality and function of your spine.
