Knowing your “lymph node status” helps determine which combination of therapies are best for treating your unique cancer. Only about 30% of all patients diagnosed with invasive breast cancer are found to have cancer in their lymph nodes. If cancer travels to the lymph nodes, these cells typically go to the axillary lymph nodes under the arm on the same side of the newly diagnosed breast cancer. These cells usually lodge in the first 1, 2, or 3 lymph nodes (known as “sentinel nodes”) and grow there. Research suggests that cancer typically spreads to the sentinel nodes before the other 10 to 20 axillary nodes everyone has under the arm.
If you are found to have cancer in your lymph nodes, you will likely be offered chemotherapy if you can tolerate it. It is less likely you will need chemotherapy if your lymph nodes are “negative.” Your lymph node status is one of many factors in deciding your treatment options.
“Sentinel Node Biopsy” vs. “Axillary Dissection”
A sentinel lymph node biopsy has replaced the more extensive “axillary dissection” for most early stage breast cancer surgeries. A sentinel node biopsy is easier to perform, is just as accurate, and causes fewer side effects than an axillary dissection. An axillary dissection is a more extensive surgery that removes all of the axillary lymph nodes and results in more armpit sensation loss and an increased risk of lymphedema than the less invasive sentinel node biopsy.
There are some situations where an axillary dissection is still clearly needed. Having detected cancer in the lymph nodes before surgery is usually an indication for an axillary dissection. In some instances, if you are found to have cancer present in the sentinel nodes, you might need an axillary dissection. Our goal with this course is to give you an outline on axillary surgery so you can better make these decisions with your breast surgeon.
How is a sentinel lymph node biopsy performed?
Hours before your breast surgery, you will likely undergo a small injection into the skin of your breast of a mildly radioactive “tracer.” This tracer slowly filters through the lymphatic system of the breast to the first one or two axillary lymph nodes (sentinel nodes) under your arm. These are the same lymph nodes that breast cancer cells would first travel to from the breast. A blue dye injection is also commonly used in addition to the radioactive tracer.
Your surgeon will use a small probe (similar to a Geiger counter) during surgery to find your sentinel nodes. The dye can also turn the same sentinel nodes blue in color, assisting your surgeon in finding them. The term “biopsy” implies taking just a piece of these nodes. In fact, these “sentinel nodes” are removed intact. The average number of sentinel nodes removed is only about three of the 10 - 20 lymph nodes normally present under the arm.
Are there side effects of a sentinel node biopsy?
Sentinel node surgery is a much less invasive procedure than an axillary dissection. The risks do include pain and discomfort in the armpit that does improve over time. You may have some permanent, partial sensation loss in the armpit and upper, inner arm. There is a slight risk of mild lymphedema. If your surgeon uses “blue dye” during the surgery to help find the sentinel nodes, there is a 1 to 2 % chance of having an allergic reaction to the dye.
Does a “positive” sentinel node mean an axillary dissection?
Until recently, surgeons would remove the sentinel lymph nodes and immediately have a pathologist evaluate the nodes under the microscope to see if cancer was present. For years, if any cancer was found in a sentinel node, surgeons would go ahead and remove all the nodes during the same surgery. This “axillary dissection” results in more long-term side effects than a sentinel node biopsy surgery.
In 2011, the ACOSOG Z00011 clinical trial showed that carefully selected women with early stage cancer undergoing a lumpectomy can now avoid an axillary dissection if only one or two sentinel nodes are found to be involved with a small amount of cancer.

That injection is extremely painful! I had 19 nodes that were positive with cancer. I have done really well. I had double mastectomy with reconstruction done during the same surgery! It has been one year. I have very little swelling and numbness. I did not do chemo as I am HER2neg. I did 27 radiation burns. I will be 70. I am on anastrozole 1mg. qd . I am a devout Christian. I was a hospice RN for 17 years. I know where I will spend eternity but until then I am truly living a wonderful life!!

I am 37 years old with stage 2 breast cancer. I’ve done Adriamycin and Taxol for Chemo. I’ve done 1 of 2 mastectomies so far, and I’ve had axillary node dissection total of 18 lymph nodes removed 4 came back cancerous. I’m getting ready to start radiation treatments. Then will follow with the other mastectomy and both reconstructive. I’m on a estrogen receptor now. And to be completely honest I don’t know how older women have the strength needed to go through all this. Meanwhile I’m still raising a 2 a 3 and an 11 year old at the time I started my journey. We are some powerful women that’s a fact and those injections hurt like hell btw. I wish I would of watched this before my process it would of helped tremendously.

I am 59 years old. I found a lump at the beginning of May on my chest. The size was 1.5. It had grown to 1.7 when it was removed last Monday. I have an appointment with my oncologist on tuesday but I couldn't wait so I found my lab results on my patient portal two days ago. Six nodes biopsied, one with involvement, grade 3 with extensions. The ultrasound post mammogram showed nothing in the nodes. That is why a nodal biopsy is important! The Huntsman Institute is very close to where I live. I'm going to call in the morning and ask for a doctor there. Thank you for these videos. I find most women don't want to know and don't know anything about breast cancer until they or someone close to them are diagnosed. Having all the information gives you the ability to make better choices. Stay positive! By the way the lumpectomy and nodal removal was a piece of cake. Stopped pain meds night of second day and started taking Ibu 800's hardly notice it now. I've never had any other surgery except intestinal repair from a gunshot wound. Now the tumor biopsy that hurt but only for a minute and sore for a week after. I guess were all different. Crazy, both my mother and her mother died very young of heart attacks. I always thought that would be my demise. Breast Cancer never even entered my mind. I am a genealogy addict and there is no history in our family that I have found. Don't take it for granted.

I’m 3 weeks out from a mastectomy and sentinel lymph nod biopsy and an expander placed to start te instruction. The stingy and burning is still there but not as strong. It’s annoying pain. The arm pit hurts and I wasn’t warned about that. The expander outline is visible and that actually adds to the pain. It wasn’t horrible like when muscles are involved but be prepared for the arm pit pain. Pillows have been my friends. I still carry a pillow tightly under my arm pit. Important to sleep in a reclined position.

Im 70...I do not want to deal with lympha dema from the node biopsy and that is a side effect. If there is cancer any where else...i will just accept and let nature take course. Really not that thrilled with life these days.

@@kandacepatterson7965 you won't get lymphedema from the biopsy but you can but not always get it from removing lymph nodes. I've never had a problem with it. The risk increases the more they remove . I seriously encourage you to rethink your decision dying from cancer is ugly.

@@charlotteruse158 something is going to kill me. Studies have shown 80 %of biopsy are negative. They can do pet scans dont need to out people through all that...just because its cheaper.thanks for the advice but Im going with my gut. I really dont want to be here that much longer any way.

I just had my lumpectomy a few days ago along with the dye injections. The injections did not hurt at all, he did four injections into my breast, one at 12, 3, 6, 9 o’clock. He said the needle was equivalent to an insulin size needle. ❤

I just had this procedure the other day and know it may vary but wanted to add my experience if anyone is scared. Make sure you use the lidocaine cream as directed. Putting it on and covering it with saran wrap. The cream goes around the areola 1 hour before procedure. It helps. A LOT! Luckily, I had a very fast Dr who told me exactly what was happening. It was only 4 tiny shots. Felt like a bee sting then a burn. Sting then burn. Etc. The pain doesn't last long at all. I had a lumpectomy and it's mostly under my arm where the nodes were taken out that hurt.

I had a 1.3cm ductal tumor with skin involvement. Because of the skin involvement I went from stage 1 to stage 4. I went from lumpectomy to mastectomy and sentinel node biopsy. I had the surgery a month ago. Thankfully they found no cancer in my skin, the margins or the nodes so I went back to a stage 1. Let me tell you though a month later I still am sore. I have to sleep with a small pillow under my arm so be prepared for that. It really helps! Also the nerve pain and a general overall feeling of fullness/tightness will take a while to go away. BTW I am male. I didn't even know men could get breast cancer but we can and do.

Hi, I'm new to my surgery, diagnosed in Covid19 but I called mine C20, I didn't have time to think about it or any choice but go straight into it. I have breast cancer so I had mastectomy and sentinel node biopsy plus axillary node dissection. Did you got a breast reconsideration? Did you got inpatients? I got my doble maseptomia 3 weeks ago and i got implants in my reconstruction but now the want to give me radiation im very scared of the side effects for my chest and inplants and end needing many more surgeries if the radiation burn or make a hole in the implant. 🙏🙏🙏 I don’t know what to do

Thank you so much for that last part explaining the rationale for not testing and removing lymph nodes. I'm nearly 70. I decided on the lumpectomy only after a diagnosis of DCIS. My housing situation is precarious right now. Anyway, if the lump comes back in 20 years, if I'm still on the planet...

I had 2 of the radioactive dye injections and it was very painful but it didn't last long. The nurse squeezed my feet which helped since I couldn't move. Be prepared for the area where the injections were will turn blue afterwards and your urine will also be blue. Ice packs and pain pills helped.....oh and pillows for comfort.

These videos are great. I am a 52 year oil family NP dealing with a new diagnosis of intermediate DCIS. My area is small but the decision between lumpectomy with radiation and unilateral mastectomy is a big one. These videos are a great adjunct to my discussions with with my breast surgeon. I am taking 4 weeks to consider my options and goals of therapy for my quality of life moving forward. They are very helpful to share with my husband so we can have good discussions.

My mom had a seed localization the other month and now there suggesting this biopsy. How are you doing after 2 months. Was it a quick procedure. What kind of surgery were you recommended after. I found this site after my surgery .... they give great info ..... I have Papillary breast cancer .... and was recommended a sentinel node biopsy .. At 4.40 it says a small injection (EXTREMELY PAINFUL) made me cry for half an hour after ..... the radioactive tracer had blue dye in the injection ...it was not done during my surgery like it says at 5.15 my dye was injected when they injected the radioactive tracer... I will NEVER have it done again.... it is so very painful.... so worse than any of my 5 surgeries .. don't know why they didn't do it when I was under the general anesthesia .. I hope no one has to go thru this . Be sure you ask your doctor before your surgery ....... I didnt know how this was done .... I thought they would get axillary nodes for biopsy while I was under....

I have papillary breast ca. Too.. have lymphnode biopsy scheduled in 2 days. Did yours involve the nodes? Hope not. Thanks for warning about how painful it is. I go in for radioactive injection tomorrow..hope you have well recovered

I will not let been sent to MRI. It seems the tech doing it was new and I was her guinea pig. I was inside the machine for approximately one and a half hour when they said that I would be for 30-40 minutes. Position was extremely painful for me due to the cervical pinched nerves. When mu oncologist sent me to to two different mammograms and an MRI, I only did the standard one since I have to pay a high co-pay for each breast done.

Susie, I have a similar history. I had breast cancer but I was HER-2 negative. I had a sentinel biopsy which was really painful (I would not go for another) and had mastectomy and reconstructive surgery (in two stages because I tend to encapsulate the implants). I have minimal inflammation and my right arm tends to swell some days. Besides, the scar tissue under my right arm causes pain. I do not what to do because, due to CoVid-19, I have not been able to see the oncology surgeon.

Well explained. Both RN’s, spouse had lumpectomy, with methylene blue. 13 nodes removed, chemo, radiation, arimadex. 10 years free now. I/she still worry. Yes, tingling, mild lymphedema etc. Trying to stay current, interesting Z-11 trial

I had the sentinel biopsy also before my left mesectomy and it wasn't bad had 3 shots the first 2 I didn't feel the 3 felt like a bee sting not bad

I got a doble maseptomia i got told i need radiacion but I already got my reconstruction and im a little scared i got told there’s side effects that damage the implants and your breast skin so im scared out of my mind of end needing many more surgeries to fix what the radiation damage.. who has experience anything or something like that after radiation surgery pictures please

I had lumpectomy with sentimental node biopsy. Prior to surgery I had the radioactive tracer placed a couple hours prior. Also had the blue dye injected - report shows Hot blue biopsy- does this mean the tumor has cancer cells? Or does it just ID the sentinel node?

Dr John first i would like to thank u I made ultrasound to my left breast ciz i was feeling some burning sensation and sometimes a pain in my neck and when i palpat above my my breast till my axillary i felt like something firm mass thing only in my left side But the result get out normal which makes me confused And it say breast parenchyma displays dense glandular tissue Please dr i need ur advice what shall I do

So if my surgeon took all nodes unnecessarily am I screwed? I couldn’t get a straight answer from him. First he said 2 nodes had cancer and had to take all the nodes. Then on next breath he said he only took 13 nodes. How do I get a straight answer from him?

Ok, so I had a auxilary lymph node Biopsy on November 21, 2019. I developed a seroma 5 days later and was prescribed bactrim as a precautionary. I believe it wasn't due to infection because 2 weeks later I had it aspirated, 90ccs of clear lymph (fluid) was extracted and that felt great. Within 3 days 65 percent or so of the swelling returned. The seroma was putting pressure on a nerve or something because my arm started to feel tight and tingling. Per my surgeon I may have developed the seroma because I am a little heavy set and there is extra spacing in cavity where they took the lymph node from. He said when the patient is slimmer seromas are rare. He recommended a drain but I haven't got one put in due to not getting a clear answer on what charges I'd be looking at. (My deductible is back at 2600 due to it now being a new year) I don't know what to do I can't straighten my arm out without feeling some slight pain and discomfort. There is a little more pain and tightness feeling when I try to straighten my arm and tilt my wrist back. My forearm looks a little fatter as does the corner part of my lower bicep. It's also sore! I feel like the trapped lymph fluid is moving around my arm. I get a little pitied edema in my forearm area. I haven't went back to my Surgeon since the end of December but when I was last there he said the soreness was from inflammation due to me not moving my arm as normal. "I was like really, what a croc of shit!" Please anyone with knowledge of these kind of issues (Preferably Doctors)help me out in letting me know what I should do.

It was a Biopsy, but yeah luckily everything looked good no abnormal cells/ no cancer.

@MK The swelling eventually went away probably at 6 mos post biopsy. My forearm does still get a little tighter than the right hand one from time to time especially during say lifting weights. I try not to lift too heavy as I don't want to injury my lymph nodes in my arm and axilla. I also avoid getting my blood pressure checked on my left arm also to avoid reinjury to my lymphatic system in the area of the surgery.To date it hasn't fully returned to normal as the lymph node was removed in all but it's manageable.

injection is to the tumor it self.......then taking camera...... the camera detect the first LN, that draines the tumor

@@niseylee7796 that's horrible. I'm sorry. My wife just through surgery for BC is doing the less is more route if not doing radiation, chemo, and other stuff they want. It seems like risk is there no matter what. :(

So if i need this biopsy does that mean i do have some form of breast cancer or no? This was ordered for me by the doctors. But he said it be 3 things Breast cancer, lymph node cancer,or maybe i have an infection. How would i know if i have any cancer im a little confused

Ok, so I had a auxilary lymph node Biopsy on November 21, 2019. I developed a seroma 5 days later and was prescribed bactrim as a precautionary. I believe it wasn't due to infection because 2 weeks later I had it aspirated, 90ccs of clear lymph (fluid) was extracted and that felt great. Within 3 days 65 percent or so of the swelling returned. The seroma was putting pressure on a nerve or something because my arm started to feel tight and tingling. Per my surgeon I may have developed the seroma because I am a little heavy set and there is extra spacing in cavity where they took the lymph node from. He said when the patient is slimmer seromas are rare. He recommended a drain but I haven't got one put in due to not getting a clear answer on what charges I'd be looking at. (My deductible is back at 2600 due to it now being a new year) I don't know what to do I can't straighten my arm out without feeling some slight pain and discomfort. There is a little more pain and tightness feeling when I try to straighten my arm and tilt my wrist back. My forearm looks a little fatter as does the corner part of my lower bicep. It's also sore! I feel like the trapped lymph fluid is moving around my arm. I get a little pitied edema in my forearm area. I haven't went back to my Surgeon since the end of December but when I was last there he said the soreness was from inflammation due to me not moving my arm as normal. "I was like really, what a croc of shit!" Please anyone with knowledge of these kind of issues (Preferably Doctors)help me out in letting me know what I should do.

The test the biopsy will tell you if is cancer.

Not going to lie, the blue dye made me cry. I don't know why they don't tell you exactly how painful it is going to be before the injection. I feel you should be sedated for this. Yes it needs to be done but if your going to be sedated for a lumpectomy anyway why don't they just wait until you are asleep?

Why does there have to be pain, any pain???? If I have surgery I want to be asleep!!!! This is why I’m not having a biopsy on the mass that was found! I see my Dr this afternoon and it’s going to be a talk that I’m sure she’s not going to like!!! I bet if it were men having surgery on their testicular you can bet they would be sound asleep!!!

I should have done more research before even getting the Biopsy which thankfully came back negative for cancer. I was scared because I have enlarged lymph nodes in my groin and chest area which my doctors still haven't found out why. I am just so frustrated by the lack of knowledge my doctors have. So I ask you what if anything can be done to avoid this from becoming worse? Maybe a drain then compression garment or dressing after drain removal?? Any help is greatly appreciated!
